Low-dose MTX administered weekly remains a mainstay in the therapy of RA. There is a belief amongst rheumatologists that RA patients taking MTX have both an increased risk and severity of infection. Here we review the published data on the risks of infection associated with the use of MTX in patients with RA and make some recommendations for managing MTX in patients with infection.

Cross With Caution: Antibiotic Cross-Reactivity and Co-Reactivity Patterns in Severe Cutaneous Adverse Reactions

Grace Thompson, Andrew McLean-Tooke, Michaela Lucas

Summary: Current understanding of cross-reactivity in severe cutaneous adverse reactions to beta-lactam antibiotics is limited, and the underlying immunopathogenesis of these reactions is not completely understood. Research has shown that cross-reactivity or co-reactivity commonly occurred among the beta-lactam antibiotic class, but further investigation is needed to understand and investigate patterns of cross-reactivity.

Treatment of pemphigus in Australia: Aligning current practises with global recommendations

Erin Somerville, Kurt Gebauer, Andrew Mclean-Tooke

Summary: Pemphigus is a rare and severe skin disease that can be effectively treated with the use of rituximab, an anti-CD20 monoclonal antibody. Despite overwhelming evidence supporting this treatment, Australia still recommends traditional therapies. It is proposed that rituximab be included in the formularies of Australian Public Hospitals and on the Pharmaceutical Benefits Scheme to improve treatment outcomes for moderate to severe pemphigus.

A Novel Targeted Amplicon Next-Generation Sequencing Gene Panel for the Diagnosis of Common Variable Immunodeficiency Has a High Yield

William Kermode, Dianne De Santis, Linh Truong, Erika Della Mina, Sam Salman, Grace Thompson, David Nolan, Richard Loh, Dominic Mallon, Andrew Mclean-Tooke, Mina John, Stuart G. Tangye, Michael O'Sullivan, Lloyd J. D'Orsogna

Summary: This study used a targeted NGS panel to identify disease-causing variants in a CVID cohort. Pathogenic or likely pathogenic variants were found in 27% of patients, along with variants of uncertain significance. The findings have implications for understanding the genetic mechanisms of CVID and potential targeted therapeutic strategies.
