Long-term functional and radiographic outcomes in 243 operated ankle fractures

Background: Large comparative studies that have evaluated long-term functional outcome of operatively treated ankle fractures are lacking. This study was performed to analyse the influence of several combinations of malleolar fractures on long-term functional outcome and development of osteoarthritis. Methods: Retrospective cohort-study on operated (1995-2007) malleolar fractures. Results were assessed with use of the AAOS- and AOFAS-questionnaires, VAS-pain score, dorsiflexion restriction (range of motion) and osteoarthritis. Categorisation was determined using the number of malleoli involved. Results: 243 participants with a mean follow-up of 9.6 years were included. Significant differences for all outcomes were found between unimalleolar (isolated fibular) and bimalleolar (a combination of fibular and medial) fractures (AOFAS 97 vs 91, p = 0.035; AAOS 97 vs 90, p = 0.026; dorsiflexion restriction 2.8 degrees vs 6.7 degrees, p = 0.003). Outcomes after fibular fractures with an additional posterior fragment were similar to isolated fibular fractures. However, significant differences were found between unimalleolar and trimalleolar (a combination of lateral, medial and posterior) fractures (AOFAS 97 vs 88, p < 0.001; AAOS 97 vs 90, p = 0.003; VAS-pain 1.1 vs 2.3 p < 0.001; dorsiflexion restriction 2.9 degrees vs 6.9 degrees, p < 0.001). There was no significant difference in isolated fibular fractures with or without additional deltoid ligament injury. In addition, no functional differences were found between bimalleolar and trimalleolar fractures. Surprisingly, poor outcomes were found for isolated medial malleolar fractures. Development of osteoarthritis occurred mainly in trimalleolar fractures with a posterior fragment larger than 5 %. Conclusions: The results of our study show that long-term functional outcome is strongly associated to medial malleolar fractures, isolated or as part of bi- or trimalleolar fractures. More cases of osteoarthritis are found in trimalleolar fractures.

The impact of regionalized trauma care on the distribution of severely injured patients in the Netherlands

Summary: Since the implementation of a well-organized trauma system in the Netherlands, trauma care has become increasingly centralized, with more severely injured patients being directly presented to a trauma center. However, 30% of these patients are still initially brought to a non-trauma center. Future research should focus on improving pre-hospital triage to ensure swift transfer of the right patient to the right hospital.

More than 20° posterior tilt of the femoral head in undisplaced femoral neck fractures results in a four times higher risk of treatment failure

Summary: The study found that patients with Garden type I and II femoral neck fractures with a preoperative posterior tilt angle >= 20 degrees had a higher failure rate, with an odds ratio of 4.24, which should be considered a significant predictor for treatment failure.

Serum sclerostin levels in osteoporotic fracture patients

Summary: Through the study of osteoporotic and non-osteoporotic fracture patients, it was found that osteoporotic fracture patients had lower levels of serum sclerostin. Future research should focus on the potential of sclerostin as a biomarker for osteoporosis.

Trends in data quality and quality indicators 5 years after implementation of the Dutch Hip Fracture Audit

Variation in posterior fragment fixation in the Netherlands: a nationwide study

Summary: There is significant variation in the treatment of posterior malleolar fractures among orthopaedic and trauma surgeons in the Netherlands. The percutaneous approach and open posterolateral approach seem to be equally used, but there is no uniformity in the treatment, especially for certain types of fractures. Less than half of the surgeons consider reoperation in cases of persistent postoperative step-off of more than 1 mm.

The relation between surgical approaches for pelvic ring and acetabular fractures and postoperative complications: a systematic review

Is hospital volume related to quality of hip fracture care? Analysis of 43,538 patients and 68 hospitals from the Dutch Hip Fracture Audit

Summary: This study aimed to analyze the association between hospital volume and hip fracture care. The results showed no significant correlation between hospital volume and mortality or turnaround times. However, the provision of orthogeriatric co-treatment improved with higher volumes, but decreased above 367 patients per year. Further research on complications and functional outcomes is needed.

What makes fixation of femoral neck fractures fail? A systematic review and meta-analysis of risk factors

Summary: This systematic review provides an overview of predictors for failure of treatment of displaced femoral neck fracture (dFNF) with internal fixation and quantifies their risk of fixation failure through meta-analysis. A total of 16 articles were included, identifying 24 predictors and analyzing 7 predictors in a meta-analysis. Female sex, smoking, age > 50 years, inadequate fracture reduction, and fixation with cannulated screws or pins compared to fixed angle devices were identified as significant predictors for fixation failure.

Maturation of trauma systems in Europe

Summary: This study assesses the maturity of trauma care systems in Europe and finds significant variations in their level of development. Prehospital care has the highest score, while quality assurance has the lowest. Improvements are needed in quality assurance and key infrastructure elements.

Clinical implications of additional posterior fractures in combined anterior-posterior pelvic ring fractures in elderly patients

Summary: The clinical relevance of combined anterior and posterior pelvic ring fractures (PRF) in elderly patients was investigated in this study. The majority of elderly patients with anterior PRF also had posterior PRF. Patients with combined anterior and posterior fractures were found to be more dependent on walking aids at 2-4 weeks compared to patients with isolated anterior fractures, but there were no significant differences in pain scores and Activities of Daily Living scores.

Clockwise torque results in higher reoperation rates in left-sided femur fractures

Summary: This study aimed to compare the reoperation rates within 30 days after surgery for left-versus right-sided proximal femur fractures in patients undergoing cephalomedullary nailing. The results showed a higher risk of reoperation for left-sided fractures compared to right-sided fractures. This finding may support the hypothesis that clockwise rotation during implant insertion and weightbearing may lead to higher reoperation rates.

Rates and risk factors of complications associated with operative treatment of pelvic fractures

Summary: This study aimed to identify the most common post-operative complications and their risk factors following pelvic fracture surgery. The results showed that surgical site infections were the most common complication, and duration of surgery and obesity were independent risk factors for the development of surgical site infections. It is recommended to reduce the risk of surgical site infections by using less invasive and less time-consuming surgical approaches and increasing awareness and post-operative screening for early signs of infections, especially in obese patients.

Indications for surgical fixation of low-energy pelvic ring fractures in elderly: a systematic review

Summary: This systematic review summarizes the current literature on the indications and methods for surgical fixation of fragility fractures of the pelvic ring in elderly patients after low-energy trauma. The results suggest that surgical treatment is commonly performed after a period of conservative treatment, with persistent pain being the most frequent indication for fixation. However, the timing and techniques for surgical fixation vary widely.
