Cervical collagen and biomechanical strength in non-pregnant women with a history of cervical insufficiency

Background: It has been suggested that cervical insufficiency (CI) is characterized by a muscular cervix with low collagen and high smooth muscle concentrations also in the non-pregnant state. Therefore, the aim of this study was to investigate the biomechanical properties, collagen concentration, smooth muscle cell density, and collagen fiber orientation in cervical biopsies from non pregnant women with a history of CI. Methods: Cervical punch biopsies (2 x 15 mm) were obtained from 57 normal non-pregnant women and 22 women with a history of CI. Biomechanical tensile testing was performed, and collagen content was determined by hydroxyproline quantification. Histomorphometry was used to determine the volume densities of extracellular matrix and smooth muscle cells from the distal to the proximal part of each sample. Smooth muscle cells were identified using immunohistoche-mistry. Finally, collagen fiber orientation was investigated. Data are given as mean +/- SD. Results: Collagen concentration was lower in the CI group (58.6 +/- 8.8%) compared with the control group (62.2 +/- 6.6%) (p = 0.033). However, when data were adjusted for age and parity, no difference in collagen concentration was found between the two groups. Maximum load of the specimens did not differ between the groups (p = 0.78). The tensile strength of cervical collagen, i.e. maximum load normalized per unit collagen (mg of collagen per mm of specimen length), was increased in the CI group compared with controls (p = 0.033). No differences in the volume density of extracellular matrix or smooth muscle cells were found between the two groups. Fibers not oriented in the plane of sectioning were increased in CI patients compared with controls. Conclusions: Cervical insufficiency does not appear to be associated with a constitutionally low collagen concentration or collagen of inferior mechanical quality. Furthermore, the hypothesis that a muscular cervix with an abundance of smooth muscle cells contributes to the development of cervical insufficiency is not supported by the present study.

Evaluation of automated radiostereometric image registration in total knee arthroplasty utilizing a synthetic-based and a CT-based volumetric model

Summary: Radiostereometric analysis (RSA) is an accurate method for measuring rigid body pose in three-dimensional space. This study proposes an automated image registration technique using digitally reconstructed radiographs (DRR) and computed tomography (CT) volumetric bone models as substitutes for traditional markers. The results show that marker-free RSA using DRR and CT obtained volumetric bone models is feasible with similar accuracy as the gold standard method. Additionally, the use of synthetic volumetric implant models does not improve silhouette-based methods.

Summary: The objective of this study was to examine the factors associated with unexpectedly high rates of conversion to open label oxytocin in the CONDISOX trial. A secondary retrospective analysis of data from a prospective randomized controlled trial was conducted, and it was found that switching to open label oxytocin during induced labor was associated with nulliparity, an unripe cervix, larger babies, and higher rates of delivery by cesarean section.

Biomechanical properties of the stomach: A comprehensive comparative analysis of human and porcine gastric tissue

Summary: This study characterized and compared the hyperelastic and viscoelastic properties of porcine and human gastric tissue using tension and radial compression tests. The results showed that similar constitutive models can be used for both species, but there were significant variations in the constitutive parameters. This research provides insight into the biomechanical properties of porcine and human gastric tissue and can aid in the development of diagnostic or therapeutic devices with computational models.

Women and partners' experience of major postpartum haemorrhage: a qualitative study

Summary: This study aimed to examine the impact of major postpartum haemorrhage (PPH) on women and their partners. Through qualitative interviews, it was found that factors such as incomprehensible medical terminology, a tense atmosphere, and alarm calls increased concern in women and their partners. However, small gestures such as brief explanations from healthcare professionals and reassurance that they were in control of the situation can increase their feeling of safety. Furthermore, family bonding can be supported through measures such as positioning the partner at the head of the bed and keeping the baby on the woman's chest.

Polychromatic neutron phase-contrast imaging of weakly absorbing samples enabled by phase retrieval

Summary: This study demonstrates the use of a phase-retrieval technique for propagation-based phase-contrast neutron imaging with a polychromatic beam. This technique allows imaging of samples with low absorption contrast and/or improving the signal-to-noise ratio for time-resolved measurements. Metal and bone samples were used to demonstrate the technique, showing significant improvement in signal-to-noise ratios and the ability to separate bone and D2O in the bone sample for in situ flow experiments. The use of deuteration contrast makes neutron imaging an interesting complementary method to X-ray imaging of bone, without the need for chemical contrast enhancement.

Summary: Unique markers in fetal membrane cells may be key in the search for biomarkers for preterm prelabor rupture of the fetal membranes (pPROM) in maternal blood. Through transcriptomic analysis and immunohistochemistry, markers exclusively expressed in fetal membranes were identified and their localization confirmed. These findings contribute to the understanding of the biological significance of these markers in the maternal circulation.

T2*-weighted placental magnetic resonance imaging: a biomarker of placental dysfunction in small-for-gestational-age pregnancies

Summary: This study demonstrates that placental dysfunction is common in small-for-gestational-age fetuses with normal fetal Doppler flows. T2*-weighted placental magnetic resonance imaging is a sensitive biomarker of placental dysfunction, regardless of clinical manifestations. The study indicates a shift in the understanding of placental dysfunction, which may present with a wide range of clinical and subclinical manifestations.
