Summary: This paper proposes a dual-arm robotic needle insertion system with the guidance of intraoperative 2D ultrasound (US) and preoperative 3D computed tomography (CT) fusion for percutaneous liver tumor puncture operation. By addressing the shortcomings of existing puncture robots, the system can provide real-time, clear and all-round guidance using real-time ultrasound and clear CT images simultaneously, with good accuracy demonstrated in experiments.

Summary: This article reviews the use of popliteal sciatic nerve block for foot and ankle surgery and how the anatomy of the sciatic nerve can affect block outcomes. The study found that the internal architecture of the sciatic nerve is more complex than previously described, which promotes the spread of local anesthetic to the internal aspect of the nerve trunk.

Summary: This study investigated the dislocation rate and postoperative pain effects of a novel self-coiling catheter compared to a conventional catheter. The results showed that the self-coiling catheter had a lower dislocation rate and better postoperative pain control. Further research is needed to explore the potential beneficial effects of the self-coiling catheter in other locations and application techniques.

Summary: Intravenous ketamine infusion is a safe and effective acute pain management method for surgical procedures, especially for patients at higher risk for opioid-related adverse events. However, restricted access to intravenous ketamine in the medical-surgical setting limits its effectiveness. In this report, the authors present their ketamine infusion protocol and demonstrate the safety and cost-effectiveness of ketamine infusions on the medical-surgical ward, highlighting the essential role of nurse practitioners in increasing patient access.

Summary: Interest in natural language processing, specifically large language models, for clinical applications has soared following the introduction of ChatGPT. These models are powerful but have their limitations. This article discusses the potential of large language models in perioperative medicine and highlights three areas where they can be beneficial: clinical decision support, research data analysis, and optimized documentation. It emphasizes the need to understand and leverage this technology for patient care rather than fear it taking over clinical decision-making.

Summary: Continuous interscalene nerve block is an effective non-opioid postoperative analgesia technique for shoulder arthroplasty patients, but it carries the risk of phrenic nerve blockade and respiratory complications. This retrospective cohort study analyzed the association between patient-related factors and respiratory complications in patients undergoing elective shoulder arthroplasty with continuous interscalene brachial plexus blocks. The study found that certain patient-related factors were associated with an increased likelihood of respiratory complications.

Summary: This study compared the analgesic effectiveness and motor impairment between ultrasound-guided supra-inguinal fascia iliaca block (FIB) and erector spinae plane block (ESPB) after total hip arthroplasty (THA). The results showed that there were no significant differences in morphine consumption and pain scores at 24 hours after surgery between the two methods. However, FIB provided more reliable sensory block in the femoral nerve and lateral femoral cutaneous nerve distributions, while ESPB resulted in better preservation of quadriceps motor strength. There were no differences in side effects or incidence of chronic pain between the groups.
