Predictors for distant metastasis in head and neck cancer, with emphasis on age

Martine F. van der Kamp, Friso O. W. Muntinghe, Rene S. Iepsma, Boudewijn E. C. Plaat, Bernard F. A. M. van der Laan, Ayat Algassab, Roel J. H. M. Steenbakkers, Max J. H. Witjes, Boukje A. C. van Dijk, Geertruida H. de Bock, Gyorgy B. Halmos

Summary: This study found that male gender, hypopharyngeal tumors, advanced T-stage, histopathological poor differentiation grade, regional lymph node metastasis, and extranodal extension are independent prognostic factors for the presence or development of distant metastasis in HNSCC patients, while age does not play a significant role.

Evaluating Laryngopharyngeal Tumor Extension Using Narrow Band Imaging Versus Conventional White Light Imaging

Manon A. Zwakenberg, Gyorgy B. Halmos, Jan Wedman, Bernard F. A. M. van Der Laan, Boudewijn E. C. Plaat

Summary: The study showed that the sensitivity of NBI during flexible endoscopy was comparable to WLI during rigid endoscopy, with higher tumor extension observed using NBI. NBI during flexible laryngoscopy led to upstaging and downstaging of T classification, improving visualization of tumor extension and accuracy of staging.

Cortactin expression assessment improves patient selection for a watchful waiting strategy in pT1cN0-staged oral squamous cell carcinomas with a tumor infiltration depth below 4 mm

Koos Boeve, Mirjam F. Mastik, Lorian Slagter-Menkema, Boukje A. C. van Dijk, Jan L. N. Roodenburg, Bernard F. A. M. van der Laan, Max J. H. Witjes, Bert van der Vegt, Ed Schuuring

Summary: This feasibility study evaluated the value of molecular tumor biomarkers associated with lymph node metastasis in OSCC. Tumor infiltration depth and pattern of invasion were independent prognostic markers for sentinel lymph node status. Cortactin expression showed promise as a marker to identify low-risk patients who may not benefit from SLNB or END.

High DNMT1 Is Associated With Worse Local Control in Early-Stage Laryngeal Squamous Cell Carcinoma

Leonie Bruine de Bruin, Martijn J. A. M. Clausen, Lorian Slagter-Menkema, Gertruida H. de Bock, Johannes A. Langendijk, Bert van der Vegt, Bernard F. A. M. van der Laan, Ed Schuuring

Summary: This study investigated the association between DNA methyltransferase 1 (DNMT1) expression and local control in early-stage laryngeal squamous cell carcinoma (LSCC) patients treated with radiotherapy. The results showed that high expression of DNMT1 was associated with worse local control. Patients with higher T-stage and positive N-status also had worse local control.

Age-specific oncogenic pathways in head and neck squamous cell carcinoma - are elderly a different subcategory?

Martine Froukje van der Kamp, Gyorgy Bela Halmos, Victor Guryev, Peter Laszlo Horvatovich, Ed Schuuring, Bernardus Franciscus Augustinus Maria van der Laan, Bert van der Vegt, Boudewijn Evert Christiaan Plaat, Cornelia Johanna Verhoeven

Summary: In recent clinical practice, an increasing number of elderly patients with head and neck squamous cell carcinoma (HNSCC) of unknown pathophysiology is observed. HNSCC patients can be divided into three subcategories: young patients with genomic aberrations and inheritable diseases, HPV-related HNSCCs with a more favorable prognosis, and HPV-negative tumors involving lifestyle factors. The proposed fourth subcategory is elderly (HPV-negative) HNSCC patients without a history of tobacco and alcohol consumption, which may have different pathophysiological mechanisms. Future research is needed to fill the knowledge gap.

SCC Antigen Concentrations in Fine-Needle Aspiration Samples to Detect Cervical Lymph Node Metastases: A Prospective Analysis

Jeroen E. van Schaik, Anna C. Muller Kobold, Bernard F. A. M. van der Laan, Bert van der Vegt, Bettien M. van Hemel, Boudewijn E. C. Plaat

Summary: The diagnostic value of measuring SCC-Ag and CA15-3 concentrations in FNA samples for the detection of SCC metastases in cervical lymph nodes was evaluated. SCC-Ag measurement showed comparable sensitivity to FNAC and could be used as a cost-effective screening tool in samples with nondiagnostic or indeterminate FNAC results or when multiple lymph nodes are sampled. However, SCC-Ag in FNA samples had lower specificity compared to FNAC assessed by experienced pathologists in head and neck oncology. The addition of CA15-3 measurement did not enhance accuracy.

Summary: This study investigated the association of frailty and geriatric assessment (GA) with radiation-induced toxicity (RIT) in patients with head and neck cancer. The results showed that age, regional radiotherapy, and concurrent chemotherapy were independent factors associated with increasing toxicity during the treatment period. However, none of the individual items of GA and the frailty screening instruments were associated with increasing RIT.

Narrow-Band Imaging in Transoral Laser Surgery for Early Glottic Cancer: A Randomized Controlled Trial

Manon A. Zwakenberg, Jeroen M. Westra, Gyorgy B. Halmos, Jan Wedman, Bernard F. A. M. van Der Laan, Boudewijn E. C. Plaat

Summary: This study aimed to assess whether the additional use of narrow-band imaging (NBI) in transoral laser surgery (TOLS) for early laryngeal cancer improves clinical outcomes. The results showed that the use of NBI significantly reduced the number of positive resection margins and potentially decreased the recurrence rate and improved recurrence-free survival in patients treated with NBI-supported TOLS. Further studies with larger patient groups are needed to confirm these findings.

Primer shot fractionation with an early treatment break is theoretically superior to consecutive weekday fractionation schemes for early-stage non-small cell lung cancer

Z. A. R. Gouw, J. Jeong, A. Rimner, N. Y. Lee, A. Jackson, A. Fu, J-j. Sonke, J. O. Deasy

Summary: This study investigates the effectiveness of non-uniform fractionation schedules in radiotherapy for early-stage non-small cell lung cancer.
