Indications for breast magnetic resonance imaging. Consensus Document Attualit in Senologia, Florence 2007

The clinical use of breast magnetic resonance (MR) imaging is increasing, especially for applications requiring paramagnetic contrast-agent injection. This document presents a synthetic list of acceptable indications with potential advantages for women according to evidence from the literature and the expert opinion of the panel that developed this statement. We generally recommend that breast MR imaging be performed in centres with experience in conventional breast imaging [mammography and ultrasonography (US)] and needle-biopsy procedures (under stereotactic or US guidance) as well as in breast MR imaging and second-look US for findings not revealed by conventional imaging performed before MR imaging. In our opinion, there is no evidence in favour of breast MR imaging as a diagnostic tool to characterise equivocal findings at conventional imaging when needle-biopsy procedures can be performed, nor for the study of asymptomatic, non-high-risk women with negative conventional imaging. After a description of technical and methodological requirements, we define the indications and limitations of breast MR imaging for surveillance of high-risk women, local staging before surgery, evaluation of the effect of neoadjuvant chemotherapy, breast previously treated for carcinoma, carcinoma of unknown primary syndrome, nipple discharge and breast implants.

Artificial intelligence for digital breast tomosynthesis: Impact on diagnostic performance, reading times, and workload in the era of personalized screening

Summary: The ultimate goals of applying artificial intelligence (AI) to digital breast tomosynthesis (DBT) are to reduce reading times, improve diagnostic performance, and decrease interval cancer rates. While AI has shown promising results in improving DBT screening by enhancing diagnostic performance and reducing recall rates and reading times, its ability to reduce interval cancer rates still needs further investigation. Prospective validation studies are needed to assess the cost-effectiveness and real-world impact of AI models assisting DBT interpretation.

Artificial intelligence (AI) in biomedical research: discussion on authors' declaration of AI in their articles title

Summary: Artificial intelligence (AI) has been around for a while, with different approaches such as machine learning and deep learning. This article discusses the frequency of AI declaration in article titles. The percentage of titles declaring AI followed a U-shaped curve, starting at around 30% in the early 1990s, dropping to less than 13% in 2005-2014, and rising to 30% again in 2020-2021. This trend was also observed in AI in medical imaging. The initial decline may be attributed to the establishment of AI methods, while the recent increase may be due to AI's superior performance and unresolved concerns about AI.

Conservative Surgery in cT4 Breast Cancer: Single-Center Experience in the Neoadjuvant Setting

Summary: Screening programs have led to a decrease in the diagnosis of locally advanced breast cancer, but the optimal surgical treatment strategy is still under debate. This study compared conservative breast surgery (CBS) with radical breast surgery (RBS) in cT4 breast cancer patients who underwent neoadjuvant chemotherapy (NA) and surgery. The results suggest that CBS may be a safe alternative to RBS in patients with major or complete response to NA, while RBS remains the best choice for patients with poor response to NA.

A Multicentric, Single Arm, Prospective, Stratified Clinical Investigation to Confirm MammoWave's Ability in Breast Lesions Detection

Summary: In this study, the effectiveness of the MammoWave microwave imaging device for breast cancer detection is investigated through a multicentric European clinical trial. The device uses a Huygens-principle-based radar algorithm for image reconstruction and dedicated image analysis software. Conductivity weighted microwave images are created for each breast, and various image parameters are used to quantify their non-homogenous behavior. Welch's t-test is used to verify the statistical significance between the selected image features and the gold standard radiological study review.

The predictive role of right ventricular late gadolinium enhancement in patients with tetralogy of Fallot undergoing pulmonary valve replacement

Summary: The study aimed to evaluate the correlations between right ventricular late gadolinium enhancement (LGE) at cardiac magnetic resonance (CMR) and post-pulmonary valve replacement (PVR) functional data in patients with tetralogy of Fallot (ToF). The results showed a moderate positive correlation between RV LGE score and the normalized variation of RV stroke volume, as well as a borderline moderate positive correlation between RV LGE score and the normalized variation of RV end-diastolic indexed volume.

Summary: The purpose of the study was to identify clinical and mpMRI factors predicting false positive target biopsy (FP-TB) of PI-RADSv2.1 = 3 findings. A multivariable model was built and internally validated to identify features predicting FP-TB of index lesions. Adjusting PI-RADSv2.1 categories for a multivariable risk of FP-TB is potentially more effective in triggering target biopsy of index lesions.

Triple-negative breast cancer arising in a fibroadenoma in BRCA 1 mutated patient

Summary: We present the case of a 35-year-old woman with a palpable lump on her left breast. Ultrasound-guided core needle biopsy revealed high-grade ductal carcinoma in situ on fibroadenoma. The patient was diagnosed with triple-negative breast cancer arising on fibroadenoma and a BRCA1 gene mutation was detected.

The results showed that patients who had MRI for screening had higher mastectomy rates and lower reoperation rates compared to those who had MRI for diagnostic purposes.

Muscle mass loss in breast cancer patients of reproductive age (=45 years) undergoing neoadjuvant chemotherapy

Summary: The purpose of this study was to evaluate pectoralis muscle area (PMA) depletion in breast cancer patients aged 45 years or younger undergoing neoadjuvant chemotherapy (NAC) and its correlation with clinical and histopathological data. The results showed a decrease in PMA before and after NAC. However, there were no significant associations between PMA changes and other clinical and histopathological data. Therefore, assessing PMA variation on breast MRI during NAC could be a potential tool for monitoring body composition and improving outcomes in young breast cancer patients.

Added value of the EUSOBI diffusion levels in breast MRI
