Young adults and acceptance of the human papillomavirus vaccine

Objectives: To determine whether young Dutch adults had ever heard of human papillomavirus (HPV) and whether they would accept vaccination, and to assess the factors influencing their decision. Study design: Cross-sectional survey. Methods: Six hundred participants aged 18-25 years were recruited from two university departments and one non-university technical college. Results: One hundred and six (17.7%) participants had heard of HPV and 536 (94%) had heard of cervical carcinoma. Women had significantly more knowledge of cervical carcinoma than men. A medical education, knowledge of HPV, knowledge of cervical cancer and knowledge of the cervical screening programme were not significantly associated with acceptance of HPV vaccination, whereas gender and age did show a significant relationship. In total, 61% of the female participants and 48% of the mate participants were willing to accept a 'catch-up' HPV vaccination. Conclusions: This study found that average knowledge levels of HPV and cervical cancer were low. Despite this lack of knowledge, a small majority of the study population would accept a 'catch-up' HPV vaccination. Women and younger participants were significantly more willing to accept HPV vaccination. However, in these subgroups, acceptance of HPV vaccination seems to be affected by other, still unidentified, factors. These factors could be evaluated in a more qualitative orientated study. An educational campaign is needed to cover knowledge about HPV and cervical carcinoma, and beliefs and behaviours associated with the acceptance of vaccination. Summary: The introduction of the revised FIGO 2018 staging system for cervical cancer has improved the accuracy of predicting survival outcomes for early stage patients. In addition to stage, factors such as LVSI and invasion depth should be considered in treatment decisions. The FIGO 2018 staging system accurately reflects prognosis and allows for better prognostic differentiation.

Summary: In the Netherlands, self-collected samples have lower high-risk human papillomavirus (hrHPV) positivity but higher cervical intraepithelial neoplasia (CIN) 2+ detection compared to clinician-collected samples. The differences between the two sampling methods are likely related to sociodemographic factors as well as differences in screening test accuracy and background risk. Self-sampling can be used as a more convenient screening tool for underscreened women.

Sentinel Lymph Node Mapping in Presumed Low- and Intermediate-Risk Endometrial Cancer Management (SLIM): A Multicenter, Prospective Cohort Study in The Netherlands

Summary: This study aims to investigate the incidence of sentinel lymph node (SLN) metastases and the contribution of SLN mapping in the management of presumed low- and intermediate-risk endometrial cancer (EC). A total of 152 patients were enrolled, with overall and bilateral SLN detection rates of 91% and 61%, respectively. The final results showed that SLN metastases were present in 11.2% of patients, and 5.9% of patients had adjuvant radiotherapy based on SLN metastases. The study emphasizes the importance of SLN mapping in presumed early-stage, grade 1 and 2 EC.

Evaluation of DNA methylation biomarkers ASCL1 and LHX8 on HPV-positive self-collected samples from primary HPV-based screening

Summary: This study evaluated the use of DNA methylation analysis as a triage method for high-risk HPV-positive self-collected cervicovaginal samples in cervical cancer screening. The methylation markers ASCL1 and LHX8 showed a sensitivity of 73.3% and specificity of 61.1% for detecting CIN3+. Compared to clinician-collected samples, self-collected samples had a relative sensitivity of 0.95 and relative specificity of 0.82. Therefore, the ASCL1/LHX8 methylation marker panel is a feasible triage method for HPV-positive women participating in routine self-sampling.

The Siderophore Ferricrocin Mediates Iron Acquisition in Aspergillus fumigatus

Summary: Aspergillus fumigatus, a common airborne fungal pathogen, utilizes two high-affinity iron uptake mechanisms, reductive iron assimilation (RIA) and siderophore-mediated iron acquisition (SIA), to maintain iron homeostasis and enhance virulence. This study focused on understanding iron acquisition during germination and revealed that ferricrocin, a siderophore, plays a crucial role in this process. Ferricrocin was found to be secreted and taken up during early germination, regardless of iron availability, indicating developmental regulation of this iron acquisition system. These findings provide insights into the mechanisms of iron acquisition and suggest potential targets for the diagnosis and treatment of fungal infections.

Acceptability of risk-based triage in cervical cancer screening: A focus group study

The Transcriptome Response to Azole Compounds in Aspergillus fumigatus Shows Differential Gene Expression across Pathways Essential for Azole Resistance and Cell Survival

Summary: This study compared the adaptational response to azole drugs from A. fumigatus isolates with different azole susceptibility and genetic backgrounds. Through RNA sequencing, it was found that there were many similarities in up-regulation of ergosterol biosynthesis across isolates, except for the pan-azole-resistant isolate which showed very little differential regulation compared to other isolates. Additionally, differential regulation of membrane efflux transporters, secondary metabolites, iron metabolism, and various stress response and cell signaling mechanisms were observed.
