Summary: Nonfunctioning pituitary macroadenomas (NFPMA) are benign tumors that cause symptoms of mass effects including hypopituitarism. 80% of patients with NFPMA have pituitary deficits, and nearly half of them will benefit from sustained improvement after surgery. Hyperprolactinaemia at diagnosis and lower tumor dimensions are associated with favorable endocrine prognosis.

Summary: Following gross total resection of nonfunctioning pituitary adenomas, the 10-year chance of recurrence is low and the need for treatment even lower. After subtotal resection, upfront radiation therapy may prevent progression, but even without it, the need for intervention at 10 years is only approximately 20%, warranting a period of observation to prevent unnecessary prophylactic radiation therapy. Tumor volume > 1 cm(3), Knosp score >= 3, and Ki-67 >= 3% may prompt closer follow-up or justify early prophylactic radiation therapy.

Summary: This study retrospectively analyzed the outcomes of the transsphenoidal approach for nonfunctioning pituitary adenomas treated at a single institute. The study found that the transsphenoidal approach is an effective and safe treatment, improving visual impairment and restoring endocrine function. Diabetes insipidus was the most common complication, and complete resection during surgery helped to lower the risk of recurrence.

Summary: In this study, lower zip code-level SES was associated with more severe disease at the time of diagnosis for NFPA patients, but there were no significant differences in short-term postoperative complications. Patients with higher zip code-level SES had higher rates of gross-total resection.

Summary: This study examined the frequency and characteristics of AIP gene mutations in Polish patients with apparently sporadic pituitary macroadenomas. The results showed that 5 out of 131 patients carried AIP gene mutations. However, patients with AIP gene mutations did not differ significantly from other patients in terms of age at diagnosis, tumor diameter, gender distribution, and tumor secretion frequency.

Summary: This study compared the clinical and biochemical characteristics of patients with corticotroph microadenomas and macroadenomas. It was found that macroadenomas had higher levels of ACTH but no association was found between tumor size and cortisol hypersecretion or clinical features of hypercortisolemia. Furthermore, patients with macroadenomas with sphenoid or cavernous sinus invasion had higher biochemical burden, highlighting that extent/location of the adenoma may be more important than size per se.

Summary: This study investigated the use and effectiveness of endoscopic transnasal, transsphenoidal surgery for the treatment of macroadenomas and giant pituitary adenomas. The surgical results of 429 patients were evaluated, and the experiences and lessons learned from treatment complications were assessed. The study found that neuroendoscopic treatment is a safe and effective method, with improved clinical symptoms and hormone levels post-surgery.

Summary: This study evaluated the therapeutic effects of surgical treatment for cystic pituitary prolactin-secreting macroadenomas. The results showed high surgical cure rate and comprehensive remission rate, with a low rate of surgical complications. Long-term follow-up revealed a higher tumor progression rate in the medication plus surgery group, with shorter recovery time for prolactin levels in the surgery alone group.

Summary: The study on the natural history of asymptomatic nonfunctioning pituitary adenomas (NFPAs) with optic nerve compression showed that close follow-up and timely surgical intervention can effectively prevent visual and endocrine issues for cases with optic nerve compression, and conservative management is also acceptable for these tumors. Summary: This study aimed to evaluate alterations in the hypothalamic-pituitary-gonadal (HPG) axis at 1 and 12 months after kidney transplantation (KT) and their association with insulin resistance. LH, FSH, E2/T, and HOMA-IR were assessed in kidney transplantation recipients (KTRs) aged 18-50 years with primary kidney disease and stable renal graft function. The study found that successful KT restores the HPG axis in the first year. Hypogonadism had a negative correlation with insulin resistance in the early period after KT, but it was not significant at 12 months.

Summary: This review evaluates the biological processes underlying malignant transformation and their relevance to the development of pituitary adenomas. It addresses various processes such as genome instability, epigenetic reprogramming, angiogenesis, proliferative signals, immune evasion, and inflammation in relation to pituitary tumors. Additionally, it discusses the existence of oncogene-induced senescence in these tumors. Understanding these processes can provide insights into the resistance of pituitary tumors to malignant transformation and contribute to the development of novel anticancer treatments.

Summary: This study evaluated the clinical and biochemical characteristics of patients with type 1 diabetes (T1D) with and without polycystic ovarian syndrome (PCOS). The results showed a prevalence of 39% for PCOS in T1D patients, and significant differences were found between patients with and without PCOS in terms of insulin usage, glucose levels, ovarian volume, and Ferriman-Gallwey scores. Additionally, PCOS women had a higher incidence of insulin resistance and metabolic syndrome.

Summary: This study aims to evaluate the effect of normalization of serum 25(OH)D concentrations on the PTH/vitamin D/calcium-FGF23/klotho/phosphorus axis in kidney transplant recipients (KTR) treated with cholecalciferol. The results showed that the normalization of 25(OH)D led to a decrease in PTH level and an increase in serum phosphorus level, but no significant changes were observed in calcium, klotho, and FGF23 concentrations. Rating: 7 out of 10.
