Clinical Characteristics of Young Type 2 Diabetes Patients with Atherosclerosis

Objective The prevalence of type 2 diabetes is increasing rapidly in the young population. The clinical characteristics and risk factors for young type 2 diabetes patients with atherosclerosis are not fully explicated. The aim of the present study was to investigate various clinical and biochemical characteristics of young type 2 diabetic patients with atherosclerosis. Design and Methods This was a cross-sectional study. The study involved 2199 hospitalized patients with type 2 diabetes. The young patients were classified into the atherosclerotic group or the non-atherosclerotic group, and we also enrolled an older group with peripheral atherosclerosis disease and an age of at least 45 years. Comparisons were made between the different groups to investigate the cardiovascular and metabolic risk profiles of young type 2 diabetes patients with atherosclerosis. We also used logistic regression models to assess the atherosclerosis risk factors for young patients. Results Compared to older type 2 diabetes patients with atherosclerosis, young patients with atherosclerosis had more deleterious profiles of weight and hyperlipidemia. Only age and diabetes duration were found to be significant independent risk factors for atherosclerosis in young patients. The ratio of the presence of atherosclerosis in the lower extremity arteries alone was significantly higher in young patients than older patients (26.4% vs. 14.0%, P = 0.000). Conclusion Young type 2 diabetes patients with atherosclerosis have more adverse cardiovascular risk profiles and inadequate control of these risk factors. Lower extremity examination is of high importance in young patients.

The Biological Disease-Modifying Antirheumatic Drugs and the Risk of Cardiovascular Events: A Systematic Review and Meta-Analysis

Furthermore, bDMARD users with follow-up over one year had significantly reduced risks of heart failure, cardiovascular death, all-cause mortality, and 3P-MACE.

Population attributable fractions of modifiable risk factors for microvascular complications of type 2 diabetes in China: An analysis using national cross-sectional data

Summary: This study aimed to assess the population attributable fractions (PAFs) for modifiable risk factors for microvascular complications of type 2 diabetes (T2D) in China. Data from the China National HbA1c Surveillance System were analyzed to calculate PAFs for diabetic microvascular complications, including diabetic retinopathy, diabetic kidney disease, and distal symmetric polyneuropathy. The study found that suboptimal glycemic and blood pressure control were the main contributors to these complications, while the impact of unmet LDL-C and BMI control targets was limited.

Anti-inflammatory therapies were associated with reduced risk of myocardial infarction in patients with established cardiovascular disease or high cardiovascular risks: A systematic review and meta-analysis of randomized controlled trials

Summary: This study evaluated the association between anti-inflammatory therapies and the incidence of cardiovascular events in patients with established cardiovascular disease or high cardiovascular risks. The results showed that anti-inflammatory therapies were associated with a decreased incidence of myocardial infarction, especially those targeting the central IL-6 signaling pathway. IL-1 inhibitor treatment was associated with a reduced risk of heart failure, while colchicine treatment was associated with a lower incidence of stroke. The risk reduction of myocardial infarction by anti-inflammatory therapies appeared to be more prominent in older patients with long follow-up duration.

Sensitivity to thyroid hormone and risk of components of metabolic syndrome in a Chinese euthyroid population

Summary: This study evaluated the association between sensitivity to thyroid hormone and metabolic syndrome and its components in a Chinese population. The results showed that reduced thyroid hormone sensitivity was significantly associated with metabolic syndrome and its components. This suggests that impaired thyroid hormone sensitivity may affect the distribution of adipose tissue and muscle.

Summary: This study aimed to investigate the association between proprotein convertase subtilisin/kexin type 9 inhibitors (PCSK9i) therapy and the risk of fracture. The results showed that PCSK9i therapy was not associated with major osteoporotic fracture, hip fracture, osteoporotic non-vertebral fracture, and total fracture over a period of 6-64 months.

Baseline eGFR, albuminuria and renal outcomes in patients with SGLT2 inhibitor treatment: an updated meta-analysis

Summary: The aim of this study was to investigate the association between baseline renal characteristics and disparities in renal outcomes among patients with SGLT2i treatment. Event-driven randomized controlled trials were included, and sensitivity analyses on prespecified eGFR and UACR subgroups were conducted. The use of SGLT2i was consistently associated with improved renal prognosis, with renal benefits independent of baseline eGFR and more pronounced in patients with mildly or severely increased albuminuria.

Summary: This study evaluated the reductions of CRP and LDL-C in different lipid-lowering drugs and the relationships between these reductions and cardiovascular events. The results showed that statins, ezetimibe, and PCSK9-mAbs are effective in reducing LDL-C levels. Statins and ezetimibe also had a significant effect on CRP. The traditional lipid-lowering strategy, including statins and ezetimibe, showed similar benefit on CV outcomes compared with PCSK9-mAbs treatment.
