Objectives To study the association between CD4/CD8 ratio and morbidity in HIV-infected patients on antiretroviral therapy (ART). Methods The APROCO/COPILOTE cohort enrolled patients initiating a protease inhibitor-containing ART in 1997-1999. The association between occurrence of first non AIDS-defining severe events (NADE) and time-dependent measures of immune restoration was assessed by 4 Cox models with different definitions of restoration, CD4+ cell counts (CD4), CD4/CD8 ratio, both CD4 and CD4/CD8 ratio, or a composite variable (CD4<500/mm(3), CD4 > 500/mm(3) and CD4/CD8 ratio < 1, CD4 > 500/mm(3) and CD4/CD8 ratio > 1). Models adjusted on baseline characteristics and time-dependent viral load were compared using Akaike Information Criterion. Results We included 1227 patients. Median duration of follow-up was 9.2 years (IQR: 4.2-11.4). Median CD4 was 530/mm(3) at 9 years. Median CD4/CD8 ratio was 0.3 (IQR: 0.2-0.5) at baseline and 0.6 (IQR: 0.4-0.9) after 9 years. Incidence of first NADE was 7.4/100 person years, the most common being bacterial infections (21%), cardiovascular events (14%) and cancers (10%). For both bacterial infections and cardiovascular events, the CD4/CD8 ratio did not add predictive information to the CD4 cell count. However, low CD4/CD8 ratio was the best predictor of non-AIDS cancers (adjusted HR = 2.13 for CD4/CD8 < 0.5; 95% CI = 1.32-3.44). Conclusions CD4/CD8 ratio remains < 1 in most HIV-infected patients despite long-term CD4+ cell counts restoration on ART. A CD4/CD8 ratio < 0.5 could identify patients who require a more intensive strategy of cancer prevention or screening.

No barrier to care, yet disparities in the HIV care continuum in France: a nationwide population study

Lise Cuzin, Clotilde Allavena, Laurent Cotte, Cyrille Delpierre, Thomas Huleux, Romain Palich, Pierre Delobel, Francois Raffi, Andre Cabie

Summary: Despite free access to care and universal ART in France, disparities remain in the HIV continuum care across age, country of birth and way of HIV acquisition. Clinical and public health interventions targeting specific patients' conditions are needed.

Summary: The presence of drug-resistant viral variants (DRVs) with the M184V mutation decreases over time in HIV DNA, but it persists more frequently in patients with longer treatment experience and past replication under 3TC/FTC.

Summary: A retrospective analysis of PLHIV with a history of KS and controlled HIV replication found that switching from a PI-based regimen to a PI-free one did not increase the risk of KS relapse.

Dolutegravir-based dual maintenance regimens combined with lamivudine/emtricitabine or rilpivirine: risk of virological failure in a real-life setting

Summary: Maintenance ART with dolutegravir-based dual regimens has shown efficacy in HIV-1infected subjects, but real-life data are limited. In this study, virological failure and resistance-associated mutations were assessed in dolutegravir maintenance regimens. Results indicated that careful virological screening is essential before switching to dolutegravir maintenance regimens in virologically suppressed patients.

Prophylactic Antibiofilm Activity of Antibiotic-Loaded Bone Cements against Gram-Negative Bacteria

Andrea Cara, Tristan Ferry, Frederic Laurent, Jerome Josse

Summary: Gram-negative bacilli can cause prosthetic joint infection, and the treatment becomes challenging due to increased antibiotic resistance and biofilm formation. Loading bone cement with antibiotics can help reduce the risk of infection during arthroplasties with cemented prosthesis. The study compared the efficacy of various antibiotic-loaded bone cements in preventing biofilm formation by different strains of Gram-negative bacteria.

Economic impact of generic antiretrovirals in France for HIV patients' care: a simulation between 2019 and 2023

Romain Demeulemeester, Nicolas Savy, Michael Mounie, Laurent Molinier, Cyrille Delpierre, Pierre Dellamonica, Clotilde Allavena, Pascal Pugliesse, Lise Cuzin, Philippe Saint-Pierre, Nadege Costa

Summary: This study assessed the economic impact of generic antiretrovirals in France over a five-year period, finding substantial savings from the use of generics in different scenarios. This could help decision makers anticipate resource allocations and promote innovation in antiretroviral therapies and prevention.

Summary: This study aims to explore the differences in antiretroviral therapy (ART) prescriptions between naive and virally suppressed HIV patients born in France and Sub-Saharan Africa. The findings suggest that the differences in ART prescriptions between these two groups cannot solely be explained by different clinical and virologic situations.

Low-level viral loads and virological failure in the integrase strand transfer era

Lise Cuzin, Philippe Flandre, Clotilde Allavena, Romain Palich, Claudine Duvivier, Agathe Becker, Helene Laroche, Pascal Pugliese, Andre Cabie, Dat'AIDS Study Grp

Summary: This study analyzed the occurrence of virological failure (VF) in patients starting ART with an integrase strand transfer inhibitor (INSTI)-based regimen in recent years and its relationship with previous episodes of low-level viral load (LLVL). The results showed that LLVL was associated with VF, and even in the absence of subsequent failure, LLV episodes have a cost. Therefore, any viral load value above 50 copies/mL should lead to enhanced adherence counseling.
