Patient and System-Related Delays of Emergency Medical Services Use in Acute ST-Elevation Myocardial Infarction: Results from the Third Gulf Registry of Acute Coronary Events (Gulf RACE-3Ps)

Background Little is known about Emergency Medical Services (EMS) use and pre-hospital triage of patients with acute ST-elevation myocardial infarction (STEMI) in Arabian Gulf countries. Methods Clinical arrival and acute care within 24 h of STEMI symptom onset were compared between patients transferred by EMS (Red Crescent and Inter-Hospital) and those transferred by non-EMS means. Data were retrieved from a prospective registry of 36 hospitals in 6 Arabian Gulf countries, from January 2014 to January 2015. Results We enrolled 2,928 patients; mean age, 52.7 (SD +/- 11.8) years; 90% men; and 61.7% non-Arabian Gulf citizens. Only 753 patients (25.7%) used EMS; which was mostly via InterHospital EMS (22%) rather than direct transfer from the scene to the hospital by the Red Crescent (3.7%). Compared to the non-EMS group, the EMS group was more likely to arrive initially at a primary or secondary health care facility; thus, they had longer median symptom- onset-to-emergency department arrival times (218 vs. 158 min; p (<) .001); they were more likely to receive primary percutaneous coronary interventions (62% vs. 40.5%, p = 0.02); they had shorter door-to-needle times (38 vs. 42 min; p =.04); and shorter door-to-balloon times (47 vs. 83 min; p (<) .001). High EMS use was independently predicted mostly by primary/secondary school educational levels and low or moderate socioeconomic status. Low EMS use was predicted by a history of angina and history of percutaneous coronary intervention. The groups had similar in-hospital deaths and outcomes. Conclusion Most acute STEMI patients in the Arabian Gulf region did not use EMS services. Improving Red Crescent infrastructure, establishing integrated STEMI networks, and launching educational public campaigns are top health care system priorities.

Summary: This study analyzed the association between physical activity and renal and cardiovascular outcomes in high-risk patients aged >= 55 years. The results showed that moderate physical activity was associated with improved kidney outcomes, with a threshold of at least two sessions per week. The association between physical activity and renal outcomes did not differ significantly between those with and without diabetes, but the absolute benefit of activity was greater in people with diabetes.

Summary: The International Polycap Study 3 (TIPS-3) trial showed that a polypill containing multiple drugs can effectively reduce the occurrence of cardiovascular events in people without cardiovascular disease. There are regional variations in the costs and affordability of this polypill. Based on the economic classification by the World Bank, the polypill is affordable in countries at different economic levels.

Prevalence and epidemiological trends in mortality due to COVID-19 in Saudi Arabia

Summary: This article presents the prevalence and epidemiological trends of COVID-19 mortality in Saudi Arabia. The study utilized a prospective epidemiological cohort design and collected data from all healthcare facilities in Saudi Arabia from March 23, 2020, to April 30, 2022. The results show a low case fatality rate and death per 100,000 population in Saudi Arabia compared to other developed countries.

The TG/HDL-C ratio was found to be a useful biomarker for guiding treatment and prevention therapy. The results showed a significant association between high LDL levels and ACS events, while the association with the TG/HDL-C ratio was not significant.

Evidence-based Management of Major Bleeding in Patients Receiving Direct Oral Anticoagulants: An Updated Narrative Review on the Role of Specific Reversal Agents

Summary: The use of direct oral anticoagulants (DOACs) has expanded in recent years, with lower bleeding risks and mortality compared to vitamin K antagonists. However, there is still a considerable risk of major bleeding in DOAC users, which can be life-threatening. Current international guidelines recommend specific reversal agents for managing severe bleeding in DOAC users, but these agents are unavailable or limited in many countries, leading to potential negative impacts on clinical outcomes. Limited real-world evidence exists regarding the clinical outcomes of DOAC-associated major bleeding in these countries.
