Background: The awareness, treatment and control of diabetes mellitus (DM) can effectively reflect on the social status of diabetes conditions. Although several researchers have investigated the awareness, treatment and control rates of diabetes mellitus in China, little is known about their association with risk factors. This study aims to examine the relationship between risk factors and awareness, treatment and control of diabetes mellitus in northeast China. Methods: A cross-sectional survey was conducted in 2012. Multistage stratified random duster sampling design was used to select participants aged 18 to 79 years old. The analysis was based on a representative sample of 1,854 adult subjects. Multivariable logistic regression analysis was used to examine socio-demographic factors associated with the levels of awareness, treatment and control of diabetes mellitus. Results:The awareness, treatment, and control rates of diabetes mellitus were 64.1%, 52.9% and 44.2%, respectively. In the multivariable logistic regression analysis, family history of diabetes was significantly positively associated with awareness (OR, 2.145; 95% CI, 1.600-2.875) and treatment (OR, 2.021; 95% Cl, 1.559-2.619) of diabetes mellitus, while negatively associated with control (OR, 0.671; 95% CI, 0.529-0.951). Cigarette smokers and alcohol drinkers were less likely than nonsmokers and non-drinkers to be aware of their blood glucose levels (OR, 0.895, 0.614; 95% CI, 0.659-1.216, 0.446-0.844, respectively). Participants who frequently exercise were more likely to be aware of their diabetic conditions than people who never or rarely exercise (OR, 2.003; 95% CI, 1.513-2.651). Conclusions: We found that the awareness and treatment of diabetes mellitus were positively associated with age and were high in participants with a family history of diabetes and those who exercise frequently, but low for cigarette smokers and alcohol drinkers. Participants with a family history of diabetes had their diabetic condition poorly controlled.

L-Theanine and Immunity: A Review

Shuna Chen, Jiaxin Kang, Huanqing Zhu, Kaixi Wang, Ziyi Han, Leyu Wang, Junsheng Liu, Yuanyuan Wu, Puming He, Youying Tu, Bo Li

Summary: L-theanine is the main amino acid in tea leaves and has both flavor and health benefits. Its immunomodulatory effects, particularly on reducing immunosuppression and improving immunity, have been studied in clinical and epidemiological research. Numerous studies on cells and animals have also shown that theanine plays a role in regulating inflammation, nerve damage, the intestinal tract, and tumors by regulating certain cellular functions.

Quality Chemistry, Physiological Functions, and Health Benefits of Organic Acids from Tea (Camellia sinensis)

Hongbo Chen, Fei Yu, Jiaxin Kang, Qiao Li, Hasitha Kalhari Warusawitharana, Bo Li

Summary: The composition and contents of organic acids vary in different types of tea. They play important roles in the metabolism of tea plants, nutrient absorption and growth regulation, as well as the aroma and taste quality of tea. However, the research on organic acids in tea is still limited. This article reviewed the progress made in studying organic acids in tea, including analysis methods, physiological functions, composition and influencing factors, sensory contributions, as well as health benefits like antioxidation and digestion promotion. It aims to provide references for further research on organic acids in tea.

Summary: This study aimed to investigate the prevalence of cardiometabolic risk factors (CMRFs), target organ damage (TOD) and its associated factors among adults in Freetown, Sierra Leone. The study found high prevalence of CMRFs such as hypertension, diabetes mellitus, dyslipidemia, obesity, smoking, and alcohol consumption, as well as target organ damage including left ventricular hypertrophy (LVH) and chronic kidney disease (CKD). The findings highlight the need for interventions to improve cardiometabolic health screening and management in Sierra Leonean population.

Sex- and suicide-specific alterations in the kynurenine pathway in the anterior cingulate cortex in major depression

Samara J. Brown, Katerina Christofides, Christin Weissleder, Xu-Feng Huang, Cynthia Shannon Weickert, Chai K. Lim, Kelly A. Newell

Summary: This study found that the kynurenine pathway (KP) in the brains of individuals with major depressive disorder (MDD) undergoes changes, with decreased KYNA levels in female patients. In addition, suicide victims with MDD also had significantly decreased KYNA levels, which may be related to increased KYAT2 mRNA.

Summary: Hand hygiene is crucial for preventing healthcare-associated infections. This study examined the changes in hand hygiene practices and promotion in 13 public hospitals in Sierra Leone following the implementation of recommendations from an operational research study. The findings revealed that the hand hygiene score decreased for secondary hospitals but increased for tertiary hospitals. Training and education, as well as reminders in the workplace, showed a decrease in scores, while system change and institutional safety climate domains had increased scores. Limited resources for hand hygiene promotion and lack of budgetary support and patient engagement programs are persistent gaps that need to be addressed.

Summary: An operational research study in 2021 provided recommendations to improve hand hygiene compliance in two tertiary hospitals. A follow-up study conducted between February and April 2023 showed that overall hand hygiene compliance improved in Connaught Hospital but decreased in 34 Military Hospital. The improvements in Connaught Hospital were attributed to more reminders, better infrastructure, and frequent supervision assessments, while the interventions in 34 Military Hospital were less effective, possibly due to hospital reconstruction.

Summary: Implementing IPC programmes in Sierra Leone has been challenging, with sub-optimal compliance found in three tertiary hospitals in 2021. A 'before and after' observational study conducted in 2023 showed an improvement in IPC compliance, with an increase in overall compliance scores and improved compliance in all core components. Recommended actions were taken, but a dedicated IPC budget and healthcare-associated infection surveillance remained as gaps. Operational research is valuable in monitoring and improving IPC programme implementation, and further actions, such as establishing a dedicated IPC budget and long-term implementation plans, are needed to reach the 'Advanced' level.

Continual expansion of Spartina alterniflora in the temperate and subtropical coastal zones of China during 1985-2020

Xi Zhang, Xiangming Xiao, Xinxin Wang, Xiao Xu, Shiyun Qiu, Lianghao Pan, Jun Ma, Ruiting Ju, Jihua Wu, Bo Li

Summary: Biological invasions, especially by Spartina alterniflora, are significantly impacting the structure and functions of coastal ecosystems in China. Urgent efforts to control the spread of this invasive species are hindered by the lack of time-series data. By using satellite images, this study successfully generated annual maps of Spartina saltmarshes from 1985 to 2020, revealing a continuous expansion of Spartina along the coast. The invasion history and frequency of changes were found to be influenced by latitude, with low latitudes experiencing longer and more frequent invasions. Human interventions have contributed to the decline in certain areas, but natural spread remains the primary driver of its extensive invasion.
