Summary: Although progress has been made in diagnosing and treating acute coronary syndromes, cardiovascular disease remains a leading cause of death globally. High-sensitivity troponin assays have allowed for rapid diagnosis in suspected cases. Dual antiplatelet therapy and additional preventive measures are recommended. The diagnosis and management of acute coronary syndromes continue to evolve, especially in response to the COVID-19 pandemic.

Summary: This study aimed to report the demographics, angiographic findings, and in-hospital outcomes of COVID-19 ACS patients and compare them with pre-COVID-19 cohorts. The study found that COVID-19-positive ACS patients experienced delayed presentation and had higher in-hospital mortality rates compared to the control subjects before the COVID-19 pandemic. Excessive rates of and mortality from cardiogenic shock were major contributors to the worse outcomes in COVID-19 positive STEMI patients.

Summary: Antithrombotic therapy is a crucial element in the treatment of acute coronary syndromes, with a focus on antiplatelet therapy. However, balancing the benefits and bleeding risks remains controversial. This review examines clinical research and recommended approaches to management in this area.

Summary: This review provides an overview of global mortality from acute coronary syndromes (ACS) and examines the regional and temporal changes in the epidemiology of ACS over the past 20 years. It shows that mortality from ACS is higher in men than in women, and highest in lower-income global regions. High-income regions have seen progressive reductions in mortality from ACS, while Asia and Latin America have stable levels of mortality. There is a need for more complete epidemiological data to identify countries with the greatest burden of death from ACS and the need for preventive strategies.

Summary: Routine implementation of the Global Registries of Acute Coronary Events (GRACE) risk score (GRS) in cardiology services led to an increase in early invasive treatment but did not significantly improve other aspects of care for hospitalized ACS patients. Further larger scale randomized studies are needed due to low event rates and premature study discontinuation.

Summary: This review discusses the current evidence on the epidemiology, pathophysiology, clinical presentation, diagnosis, management, and prognosis of acute coronary syndromes (ACS). ACS is a sudden reduction in blood supply to the heart and includes ST-segment elevation myocardial infarction (STEMI), non-STEMI (NSTEMI), and unstable angina. Each year, more than 7 million people are diagnosed with ACS worldwide. Prompt intervention, such as primary percutaneous coronary intervention (PCI) for STEMI patients and invasive coronary angiography with revascularization for high-risk NSTE-ACS patients, is associated with reduced mortality.

Summary: Multivessel disease (MVD) is common in patients with acute coronary syndromes (ACS) and is associated with poor outcomes and high mortality rates. Complete revascularization is beneficial for hemodynamically stable ACS patients with MVD, but there are still uncertainties regarding optimal timing and guidance strategies.

Summary: Changes in LDL electronegativity are associated with adverse outcomes in patients with ACS, including increased all-cause and cardiovascular mortality. The LDL lipidome is also affected by these alterations, and specific lipid species are independently associated with fatal events.

Summary: This study explores the racial-ethnic differences in reasons for the misuse of prescription medications among US adults. The findings indicate that medical reasons are the primary motivation for misuse, but racial-ethnic minority groups are less likely to report misuse of pain relievers for medical or recreational reasons and more likely to misuse sedatives and tranquilizers for recreational purposes.

Summary: Elevated remnant cholesterol is commonly found in ACS patients and is negatively correlated with age and positively correlated with body mass index. Summary: The loss of skeletal muscle function, strength, and mass is common in older adults, and it has significant socioeconomic impact. Subclinical hypothyroidism, which is prevalent in older adults, has been associated with reduced muscle strength. However, there hasn't been a randomized placebo-controlled trial to investigate the effect of treating subclinical hypothyroidism on muscle function and mass.

Summary: This study analyzed the exclusion criteria of randomized clinical trials (RCTs) on lipid-lowering therapy and found that the majority of trials excluded patients with common diseases such as kidney disease, heart failure, and immunosuppression. This leads to limited applicability of study results and uncertainty about the side-effects and efficacy in these populations. Future trials should focus on improving diversity in recruitment strategies and promoting equity in cardiovascular research.

Summary: Two network meta-analyses were conducted to evaluate all available antithrombotic treatments within or after 12 months following coronary revascularization and/or acute coronary syndrome. Ticagrelor monotherapy showed lower cardiovascular mortality risk within 12 months, and P2Y(12) monotherapy, especially ticagrelor, was associated with lower risk of myocardial infarction beyond 12 months. Aspirin and rivaroxaban 2.5 mg were most effective in reducing stroke risk, with a more acceptable bleeding risk than VKA.

Summary: This study compared the outcomes of STEMI patients undergoing PCI according to total ischemic time. The results showed that PCI performed within 12 hours of symptom onset had better outcomes, while PCI performed later than 12 hours was associated with higher mortality and target lesion failure rates. However, there was no significant difference in outcomes between PCI performed later than 48 hours and PCI performed between 12-48 hours.

Summary: This study examined the acceptability of twelve different regulatory aspects by cannabis users, including those obtaining cannabis from the illicit market and vulnerable populations. The results showed that THC content regulation was the most contentious aspect, with a majority of participants stating a preference for a regulated program if five different THC contents were available, compared to only 54% if only 12% THC was available. Disposal of contact details was the least accepted aspect. Participants obtaining cannabis from the illicit market were more likely to engage in a regulated program. Therefore, a carefully designed program that considers the consumers' perspective is necessary to engage the target population.
