Place of Preoperative Treatment of Acromegaly with Somatostatin Analog on Surgical Outcome: A Systematic Review and Meta-Analysis

Context: Transsphenoidal neurosurgery is the accepted first-line treatment of acromegaly in the majority of patients. Previous studies addressing preoperative somatostatin analog (SSA) treatment and subsequent surgical cure rates are conflicting, reporting either benefits or no significant differences. Objective: The aim of this study, based on a meta-analysis of all published reports, was to investigate whether treatment with SSA before surgery improves the surgical outcome of acromegaly. Data Sources: All studies of preoperative treatment of acromegaly with SSA were systematically reviewed up to December 2011. We searched the Medline, Embase, Cochrane and Google Scholar electronic databases. Study Selection: The primary endpoint was the biochemical postoperative cure rate. We identified 286 studies, out of which 10 studies (3.49%) fulfilling the eligibility criteria were selected for analysis; five retrospective studies with a control group, two prospective non-randomized trials, and three prospective controlled trials. The meta-analysis was conducted using the random-effects model. Data Extraction: Data were extracted from published reports by two independent observers. Data Synthesis: A borderline effect was detected in the analysis of all of the trials with control groups, with a pooled odds ratio (OR) for biochemical cure with SSA treatment of 1.62 (95% CI, 0.93-2.82). In the analysis of the three prospective controlled trials, a statistically significant effect was idenfified OR: 3.62 (95% CI, 1.88-6.96). Conclusions: Preoperative treatment with SSA og GH-secreting pituitary adenomas shows a significant improvement on surgical results. This meta-analysis suggests that in centers without optimal results all patients with a GH-secreting pituitary macroadenoma should be treated with a long-acting SSA prior to surgical treatment.

Trends in the presentation and management of traumatic spinal cord lesions above T6: 20-Year experience in a tertiary-level hospital in Spain

Summary: This study analyzed the changes in demographic and lesion characteristics of persons with acute traumatic spinal cord injury (ATSCI) above T6 over a 20-year period and evaluated their impact on ICU resource use, length of stay, and mortality. The study found that despite an increase in age, comorbidity, and APACHE II score, the length of ICU stay decreased significantly, with no significant changes in mortality rates.

The results suggest that CIM can be an effective option for selected patients, but further research and validation are needed.

Association of rhinitis with asthma prevalence and severity

Summary: Asthma and rhinitis often co-exist in the same patient. This study aimed to describe the prevalence of rhinitis in children and adolescents and to analyze its relationship with the prevalence of asthma. The results showed a significant increase in the prevalence of asthma in both age groups for those with rhinitis, especially for exercise-induced asthma and severe asthmatics.

Thyroid Function Alteration in Obesity and the Effect of Bariatric Surgery

Summary: The relationship between obesity and thyroid function has some controversial aspects, including the effect of obesity on thyroid function and the impact of weight loss surgery. In morbidly obese patients, there is a mild increase in TSH and a mild central resistance to thyroid hormone, which can be reversed with weight loss. After weight loss, the levothyroxine dose/kg of ideal weight remains unchanged but increases when calculated based on actual weight. Diagnosing mild hypothyroidism is difficult in morbid obesity, and bariatric surgery can improve abnormal thyroid function, with better adaptation of thyroid hormone treatment based on ideal weight.

Predictors of Intensive Care Unit Stay in Patients with Acute Traumatic Spinal Cord Injury Above T6

Summary: In this study, factors associated with the length of stay (LOS) of intensive care unit (ICU) patients with acute traumatic spinal cord injury were identified. After adjusting for variables, a lower American Spinal Injury Association motor score, injury level C5-C8, higher Sequential Organ Failure Assessment score, positive fluid balance, and the onset of an infection were found to be independently associated with a longer ICU LOS.

Relationship between time from symptom's onset to diagnosis and prognosis in patients with symptomatic colorectal cancer

Summary: This study investigated the relationship between diagnostic delay and survival in colorectal cancer patients and found that the duration of diagnostic delay had no significant effect on patient outcomes. The most important determinant of diagnostic delay was determined to be the biological profile of the tumor. However, community health centers and authorities still have a responsibility to minimize diagnostic delays and implement initiatives for early diagnosis and better outcomes.

However, more research is needed to confirm the potential benefits in other clinical situations like thoracic surgery, bladder cancer, gynecological surgery, hip fracture, liver pathology, and COVID-19.
