Summary: This study explored the relationship between pulmonary regurgitation and quality of life and health status in adults with Tetralogy of Fallot (TOF), and found that with advancing age, patients reported more problems in mobility and pain or discomfort. Factors associated with superior patient-reported outcomes include race, syndrome, employment, and left ventricular function, while being asymptomatic was found to have the greatest impact on quality of life and health status.

Summary: Systemic juvenile idiopathic arthritis (sJIA) is a complex inflammatory disorder involving innate and adaptive immunity. Research shows that early use of IL-1 or IL-6 blockade as first-line treatment is effective. This study reviewed the medical records of sJIA patients over a 16-year period and examined clinical presentation, treatment interventions, and remission rates. The results indicate that patients who received early biologic treatment achieved higher rates of clinical remission.

Summary: The prevalence and determinants of tricuspid regurgitation (TR) in patients with repaired Tetralogy of Fallot (rTOF) remain incompletely understood. This study found that despite substantial right ventricular (RV) dilatation in rTOF patients, the prevalence of TR is surprisingly low and has weak correlation with factors such as right atrial volumes and tricuspid annular diameter. These findings question the commonly held notions regarding the pathophysiology of functional TR in these patients.

Summary: This study aimed to assess the 2-year neurodevelopmental outcomes for neonatal cardiac surgery survivors with tetralogy of Fallot and pulmonary atresia with ventricular septal defect. The results showed that these patients had below normative values in cognitive, language, and motor skills, with a high prevalence of delays. Chromosomal anomalies and postoperative complications were identified as factors associated with worse neurodevelopmental outcomes.

Summary: This study retrospectively compared the outcomes of primary repair (PR) and staged repair (SR) in neonates with tetralogy of Fallot and pulmonary atresia (TOF/PA). The results showed that SR was more common in higher-risk patients. The mortality rate was similar between SR and PR, but PR had advantages in perioperative morbidities, reintervention rates, and right pulmonary artery growth, while SR allowed for larger initial conduit implantation.

Summary: Children with congenital heart defects (CHD) in England have a higher risk of being born preterm, but recent trends in long-term survival based on gestational age (GA) are not well understood. This study aimed to quantify changes in survival to age 5 of children with severe CHD by GA. The findings showed that overall survival to age five increased from 87.5% in 2004-2008 to 89.6% in 2012-2016. However, the improvement in survival was driven by increased survival in term children. The study highlights the need for additional support for school-aged children with severe CHD. Rating: 8/10

The Physio4FMD trial aims to evaluate the effectiveness and cost effectiveness of specialist physiotherapy for FMD. The trial was impacted by the COVID-19 pandemic, leading to disruptions in the treatment of participants. The analysis plans have been adjusted to account for these disruptions and maintain the integrity of the trial while providing meaningful results.

Summary: This cohort study examined hospital admissions associated with SARS-CoV-2 infection in children and adolescents in England. The results showed that most admissions were due to SARS-CoV-2 or SARS-CoV-2 was a contributory factor. Boys, younger children, and those from ethnic minority groups or areas of high deprivation were more likely to be admitted to hospital. These findings have implications for public health initiatives and further research.
