Progressive Increase of Matrix Metalloprotease-9 and Interleukin-8 Serum Levels during Carcinogenic Process in Human Colorectal Tract

Background: Inflammatory reactions, known to promote tumor growth and invasion, have been found associated with colorectal carcinoma (CRC). Macrophages are the chief component of the inflammatory infiltration that occurs early in the progression from non-invasive to malignant tumor, with a switch from the pro-inflammatory phenotype to the tumor-promoting phenotype. Tumor and stroma are additional sources of inflammation-related molecules. The study aimed to evaluate, during colorectal carcinogenesis from benign to malignant phases: i) the trend of serum levels of IL-8, IL-6, TGF beta 1, VEGF and MMPs; ii) the parallel trend of CRP serum levels; iii) derangement of the principal TGF beta 1 receptors (TGF beta 1RI/RII) in tumor tissues. Methodology/Principal findings: 96 patients with colon adenomas or CRC at different stages of progression, and 17 controls, were recruited. Serum IL-8, IL-6, TGF beta 1, VEGF, MMPs and CRP levels were analyzed before endoscopy or surgery. TGF beta 1 receptors were evaluated in adenoma biopsies and surgically-removed colorectal adenocarcinomas. Serum levels of IL-8 in adenocarcinoma patients were increased from stage II, when also the enzymatic activity of MMP-9 increased. Of note, the increasing trend of the two serum markers was found significantly correlated. Trend of serum CRP was also very similar to that of IL-8 and MMP-9, but just below statistical significance. TGF beta 1 levels were lower at stage III CRC, while IL-6 and VEGF levels had no significant variations. In tissue specimens, TGF beta 1 receptors were already absent in about 50% of adenomas, and this percentage of missing receptors markedly increased in CRC stages III and IV. Conclusions: Combined quantification of serum IL-8, MMP-9 and CRP, appears a reliable and advanced index of inflammation-related processes during malignant phase of colorectal carcinogenesis, since these molecules remain within normal range in colorectal adenoma bearing patients, while consistently increase in the blood of CRC patients, even if from stage II only.

Summary: This study compared the diagnostic accuracy and adequacy of macroscopic on-site evaluation (MOSE) with conventional technique in endoscopic ultrasound-guided fine-needle biopsy (EUS-FNB). The results showed that MOSE had comparable diagnostic accuracy and better diagnostic adequacy compared to the conventional technique, and it may potentially reduce the number of passes.

Summary: A ddPCR assay for quantification of HDV-RNA viremia was developed and compared to real-time PCR methods. The study showed that ddPCR and RT-qPCR had highly comparable technical performance, and the absolute quantification method provided by ddPCR may improve the clinical and diagnostic management of delta hepatitis.

Therapy of Chronic Viral Hepatitis: The Light at the End of the Tunnel?

Summary: Chronic viral hepatitis, caused by Hepatitis B Virus, Hepatitis C Virus, and Hepatitis D Virus, is a significant global health issue. Different viruses require different treatment approaches. Long-term suppressive therapy has had a dramatic impact on chronic hepatitis B, but complete clearance of the virus remains difficult. The unique virology of Hepatitis D Virus makes its treatment challenging, but combination therapy with specific antivirals shows promise. The introduction of Direct Acting Antiviral Agents has revolutionized the treatment of chronic hepatitis C, achieving high cure rates.

Summary: This study investigates the factors influencing the intracellular concentration of GS-331007, a metabolite of Sofosbuvir, in peripheral blood mononuclear cells (PBMCs). It finds that the intracellular concentration is associated with the plasma concentration of GS-331007 and the concentrations of daclatasvir at certain time points during therapy. Treatment with ledipasvir and baseline albumin and glomerular filtration rate are significant predictors of intracellular exposure.

Summary: This retrospective study evaluated the improvements in liver synthesis and portal hypertension in cirrhotic patients cured of hepatitis C virus (HCV) infection following antiviral treatment. The results showed that a significant proportion of patients displayed improved liver function and reduced liver stiffness after successful viral eradication. The study highlights the importance of antiviral treatment in improving the condition of cirrhotic patients, and the value of simple clinical tools and liver stiffness measurement in assessing treatment efficacy.

Predictive Factors of Surgical Recurrence in Patients with Crohn's Disease on Long-Term Follow-Up: A Focus on Histology

Summary: This study aimed to identify potential factors associated with an increased risk of surgical recurrence in patients with Crohn's disease (CD) who underwent ileocolic resection. A retrospective observational study was conducted, and the results showed that age, smoking, sex, presence of granulomas at local lymph nodes, and treatment with systemic corticosteroids were significantly associated with the risk of surgical recurrence. These findings support the need for personalized follow-up taking into account different clinical, surgical, and histologic features.

Summary: The expression of extracellular Nicotinamide phosphoribosyltransferase (eNAMPT) and extracellular nicotinic acid phosphoribosyltransferase (eNAPRT) varies in patients with non-alcoholic fatty liver disease (NAFLD). While eNAMPT is similarly distributed across different fibrosis stages, eNAPRT is increased in patients with advanced fibrosis and is associated with its progression. The combination of eNAPRT with other relevant factors can provide better prediction of disease progression in NAFLD patients.

Summary: This study aimed to assess the diagnostic accuracy of PRO-C3, the derivative ADAPT score, and other non-invasive tests (NITs) for advanced fibrosis in NAFLD, as well as changes over a 12-month follow-up. The results showed that serum PRO-C3 levels and ADAPT score significantly increased during the follow-up period, indicating their potential use in monitoring liver disease progression and treatment responses.

Summary: The use of tumor biomarkers for the surveillance of patients at risk of hepatocellular carcinoma (HCC) is common practice in many clinical centers, and a cut-off value of 50 mAU/mL for the biomarker protein induced by vitamin K absence, or antagonist-II, has been found to be appropriate for discriminating between HCC patients and those without tumor. This cut-off value also allows identification of patients without tumor who are at increased risk of developing HCC during follow-up.

Summary: In this cohort study, the fibrosis-4 score (FIB-4) was found to be associated with the development of hepatocellular carcinoma (HCC) in cirrhotic patients with non-alcoholic fatty liver disease (NAFLD). The use of specific cut-offs for FIB-4 allowed for optimal risk stratification of HCC. These findings suggest that FIB-4 could be a useful tool for predicting long-term HCC risk in this population.

Summary: The COVID-19 pandemic has led to a decrease in new diagnoses of hepatocellular carcinoma (HCC) without a worsening of the cancer stage. In the first two years of the pandemic, the number of new HCC diagnoses significantly diminished. It is expected that patients who have escaped diagnosis may present with a more advanced stage of the disease in the next few years.
