Summary: Some surgical failures after temporal lobe epilepsy surgery may be caused by the presence of extratemporal epileptogenic zones, particularly in the medial parietal lobe. Seizures originating from this area may not cause obvious symptoms before spreading to the temporal lobe.

Summary: Temporal lobe epilepsy (TLE) is a common form of epilepsy, and about 30% of patients have drug-resistant seizures. This study evaluated the seizure outcomes of patients with drug-refractory TLE who underwent resective temporal lobe surgery (rTLS), finding that the lack of MRI lesions and placement of depth electrodes before surgery were associated with unfavorable postsurgical seizure outcomes.

Summary: Epilepsy is a neurological disorder associated with oxidative stress. Some antiseizure drugs, including levetiracetam, have been found to have neuroprotective and antioxidant properties. This study evaluated the effects of levetiracetam on the oxidant-antioxidant status in the hippocampus of rats with temporal lobe epilepsy (TLE). The results showed that levetiracetam increased antioxidant enzyme activity, reduced hydrogen peroxide levels, and demonstrated scavenging activity against hydroxyl radicals.

Summary: Selective resection in mesial temporal lobe epilepsy surgery has been shown to effectively preserve postoperative memory function, with specific areas of resection correlating with memory decline. This suggests the importance of selective resection to protect memory function in temporal lobe surgery.

Summary: This study analyzed the surgical outcomes of patients with temporal lobe epilepsy (TLE) who underwent temporal and insular SEEG recordings. The findings suggest that insular involvement may be associated with failed anterior temporal lobectomy (ATL) and poorer seizure outcomes. The study also identified temporal-insular spreading time (TIST) as an independent predictor of postoperative seizure-free outcomes in these patients.

Summary: This article summarizes the attention given to feline temporal lobe epilepsy (TLE) in recent years, stating that epileptic discharges in certain parts of the temporal lobe result in similar symptoms, justifying their grouping under one name. Feline TLE patients often exhibit histopathological changes in the hippocampus. The article aims to not only summarize the clinical features and possible etiology, but also to classify TLE within the veterinary epilepsy classification.

Summary: P2X7R overexpression in nerve terminals of MTLE-HS patients is associated with seizures, glial cell activation, cytokines production, GABAergic rundown, and unrestrained glutamatergic excitation. Furthermore, miR-22 serum levels are diminished, especially in patients requiring multiple AED regimens.

Summary: The study examined the safety and efficacy of Transcranial Focused Ultrasound (tFUS) neuromodulation in patients with refractory temporal lobe epilepsy undergoing surgery. Results showed that tFUS at intensities up to 5760 mW/cm(2) may be safe, with no detectable tissue damage and minimal changes in neuropsychological testing. Further research is needed due to methodological limitations and inconclusive findings.

Summary: Sotos syndrome is a genetic disorder caused by NSD1 gene haploinsufficiency, with some patients experiencing epilepsy, including drug-resistant seizures. A 47-year-old female patient with Sotos syndrome had focal-onset seizures in the left temporal lobe, left-side hippocampal atrophy, and cognitive impairment. After undergoing left-side temporal lobe resection, the patient achieved complete awake seizure control and significant improvement in quality-of-life during the 3-year follow-up. Resective surgeries may be beneficial for improving seizure control and quality of life in selected patients with Sotos syndrome.

Summary: The authors investigated the microvascular anatomy of the hippocampus and its implications for medial temporal tumor surgery, revealing the anatomical variability of the arterial supply and venous drainage of the hippocampus. The study found rich anastomoses between hippocampal arteries and highlighted the importance of understanding the vascular variability and network of the hippocampus for surgery in this region, including anterior temporal lobectomy with amygdalohippocampectomy and transsylvian selective amygdalohippocampectomy.

Summary: This study aims to characterize seizure types and phases based on electrophysiological characteristics. By analyzing intracranial EEG recordings, network and spectral measures were calculated and used to identify and classify seizure types and phases. The results showed that seizure types could be classified with high accuracy using a multidimensional feature space and a support vector machine. The study also revealed specific measures and patterns related to seizure severity during different seizure phases.

Summary: This study aimed to assess the role of prehospital point-of-care N-terminal probrain natriuretic peptide in predicting sepsis, septic shock, and in-hospital sepsis-related mortality. The study found that N-terminal probrain natriuretic peptide outperformed other scoring systems in estimating in-hospital sepsis-related mortality and its application improved the identification of infectious diseases in low-risk scoring groups.

The results showed significantly higher fractional anisotropy in widespread areas of the cerebral white matter in PD patients treated with bilateral subthalamic DBS, suggesting that neuromodulation produces connectivity changes in different neural networks.

Summary: Deep brain stimulation requires precise localization, especially in the hypothalamus. This study characterized the neurophysiological properties of the posteromedial hypothalamus (PMH) using electrical stimulation. The results showed significant differences between PMH, upper PMH (uPMH), and lower PMH (lPMH) in cell composition and characteristics. Additionally, atypical extracellular action potentials were observed for the first time.

Summary: Temporal lobe epilepsy (TLE) is the most common form of focal epilepsy, and it is associated with structural and psychological alterations. When using resected brain tissue for research, it is important to consider the clinical, anatomical, and psychological characteristics of the patients. Unfortunately, this information is often incomplete or not fully understood by the neuroscientists analyzing the brain samples.

Summary: In this pilot study, we used numerical methods to define the normative structure of resting-state EEG (rsEEG) in 37 healthy subjects aged 10-74. We employed a double-banana bipolar montage and visually identified artifact-free epochs of 120-200 s, which were divided into 1 s windows with a 10% overlap. By computationally analyzing the power spectrum, coherence, and other parameters, we characterized the rsEEG in different age groups and brain lobes. Importantly, we demonstrated the feasibility of a normative rsEEG structure obtained using a double-banana montage.

Summary: In this study, a prehospital early warning score was developed to predict 2-day mortality and ICU admission rates, incorporating components of lactic acidosis other than lactate. The results showed that the prehospital lactic acidosis (PLA) score predicted 2-day mortality better than individual components of lactic acidosis alone, and the inclusion of PLA score in the prehospital setting could improve decision-making in emergency medical services.

Summary: Science and philosophy have long sought to understand the origins of emotions, but it is only in the past 150 years that we have started to study them from a neuroscientific perspective. Emotions involve physiological changes in various systems, as well as conscious experiences embedded in memory. The cortico-striato-thalamo-cortical circuit, limbic system, and prefrontal circuit are crucial in the perception, thought, and memory of emotions. This review focuses on the anatomy and physiology of these brain structures and their implications for psychiatric pathologies and deep brain stimulation (DBS) treatment outcomes.

Summary: Osteogenesis Imperfecta (OI) is a disease that affects bone fragility and deformities, including the cranial base and craniocervical junction, potentially leading to neurological disorders. This study examined the cranial base in patients with OI compared to age-matched controls using cephalometric analysis, lateral skull radiographs, and MRI. Variations in the craniocervical junction were found to be significantly different across various age groups. Disease severity influenced the measurements of the skull base, indicating potential indicators for early detection and prevention of related alterations in patients with OI.
