Overall, radiation therapy should be delivered to the tissue expander, and proper capsuloplasty should be performed at the time of implant exchange to correct the deformities caused by radiation-induced capsular contracture.

Summary: Pre-pectoral implant-based breast reconstruction (IBR) is popular for its advantages, but there are concerns about increased rates of implant rippling. This study identified factors and technical strategies that can be implemented to reduce implant rippling in pre-pectoral IBR. The majority of studies reviewed did not find a significant association between BMI and rippling, suggesting that other factors contribute to implant rippling. Surgical modifications and attention to various technical aspects can potentially reduce the incidence of rippling in pre-pectoral breast reconstruction.

Summary: This meta-analysis compared the incidence of complications and patient-reported outcomes between immediate autologous breast reconstruction (ABR) and immediate implant-based breast reconstruction (IBBR) in breast cancer patients undergoing postmastectomy radiotherapy. The results showed that IBBR had a lower risk of reconstruction failure compared to ABR, while the risk for complications requiring reoperation did not significantly differ between the two groups. However, due to variations in statistical definitions and methodologies, the synthesized result should be critically interpreted. More high-quality studies are needed for further refinement of clinical practice.

This study found that delayed autologous PMBR leads to similar (loco)regional recurrence rates as immediate autologous PMBR. Current evidence suggests that oncological concerns should not be a reason to withhold patients from certain reconstructive timings or techniques, and patients should be offered all technically qualified reconstructive options equally.

The most commonly used outcomes were complications, postoperative pain, radiologic outcomes, and grip strength. There was substantial variation in how these outcomes were measured, highlighting a lack of consensus on critical outcomes.
