Summary: The timing of expander-to-implant exchange after radiation therapy has different effects on short-term and long-term complications. The optimal time point for permanent implant placement is 131 days after radiation therapy, reducing short-term complications significantly but not long-term complications.

Summary: Breast reconstruction in women over 60 years old was not independently associated with higher major complication rates in our series. However, a significant relationship was found between a history of ipsilateral breast conservation therapy and adjuvant chemotherapy with major complications.

Summary: The study aimed to determine complications, incidence, and predictors of implant-based reconstruction failure (RF) among mastectomy patients. Infections, delayed wound healing, and receipt of radiation therapy were significant predictors of RF. The newly developed classification tree accurately predicted RF events and non-RF events with over 70% accuracy.

Summary: Complications of implant-based reconstruction are related to body mass index (BMI) and breast size. This study compared the outcomes of skin reducing mastectomy (SRM) with a dermal flap to skin sparing mastectomy (SSM). The results showed that major complication rates were similar between the two groups, and the use of acellular dermal matrix did not adversely affect the complication rate of SRM.

Summary: This study compared the outcomes of immediate autologous breast reconstruction and delayed-immediate reconstruction. The study found that the immediate reconstruction group had more complications such as mastectomy skin flap necrosis, while the delayed-immediate reconstruction group had more complications such as infection and readmission.

Summary: This retrospective study analyzed the effect of timing of radiation therapy on the outcomes of breast reconstruction. The results showed that irradiation of the tissue expander increased the risk of failure, while irradiation of the permanent implant increased the risk of severe capsular contracture. Overall, radiation therapy should be delivered to the tissue expander, and proper capsuloplasty should be performed at the time of implant exchange to correct the deformities caused by radiation-induced capsular contracture.

Summary: This study compared the outcomes of direct-to-implant (DTI) and immediate free flap (FF) breast reconstruction and found no significant difference in ischemic complications between the two methods. Therefore, both DTI and immediate FF reconstruction can be safely offered to patients undergoing nipple-sparing mastectomy (NSM).

Summary: The technique of primary fat grafting before delayed prophylactic mastectomy can minimize early complications and avoid implant loss in high-risk patients. The results showed a low rate of early complications and a majority of patients achieved good aesthetic outcomes.

Summary: The article presents the 9-year oncologic follow-up results of patients who underwent nipple-sparing mastectomy and prepectoral implant reconstruction without acellular dermal matrix. The study shows that the procedure is oncologically safe, with low local recurrence rate and comparable overall survival and disease-free survival rates as total mastectomy.

Summary: This study aimed to identify patient subgroups where acellular dermal matrix (ADM) improved clinical outcomes and patient-reported outcomes (PROs) in expander-implant-based breast reconstruction. Results showed that ADM was associated with a greater risk of major complications, particularly in high-BMI patients, and no significant improvement in patient satisfaction, psychosocial well-being, sexual well-being, and physical well-being. This suggests a more critical approach to the use of ADM in expander-implant reconstruction may be necessary.
