Summary: In patients who underwent explantation of implants due to infection in the context of implant-based breast reconstruction, both implant-based and free flap breast reconstruction had similar rates of overall complications. However, implant-based reconstruction had higher rates of infection and reconstructive failures, as well as lower patient-reported satisfaction scores for the breasts.

Summary: This study assessed partner satisfaction with breast reconstruction surgery and compared it to patient satisfaction. The results showed a high correlation between partner satisfaction and patient satisfaction, but partners tend to overestimate patient's psychosocial well-being. Therefore, partner inclusion in regular psycho-oncological support should be encouraged to raise awareness of the high psychological burden of a breast cancer diagnosis and stabilize patients' support system.

Summary: This study compared satisfaction, aesthetic outcomes, and cancer-related outcomes in women who underwent breast removal surgery with nipple removal and reconstruction versus those who had surgery with preservation of the nipples. The results showed that nipple projection was shorter in the nipple reconstruction group, but there were no significant differences in overall symmetry, satisfaction regarding nipple preservation, or overall nipple satisfaction. The study also found no diagnoses of breast cancer in the long-term follow-up period.

Summary: The present study aims to systematically summarize the evidence on abdominal-based free flap inset for breast reconstruction and provide principles and classification to guide the surgeon's choice. Four main breast anatomical features, including breast width, breast ptosis, breast projection, and upper pole fullness, were identified as the focus of the insetting technique. This study provides a framework to guide the surgeon's decision-making and optimize the aesthetic outcomes of abdominal-based free flap breast reconstruction.

Summary: This study compared patient-reported outcomes between autologous breast reconstruction (ABR) and implant-based breast reconstruction (IBR) patients using propensity score matching. The results showed no significant differences in breast satisfaction and well-being between ABR and IBR patients who did not receive radiotherapy.

Summary: This study conducted a meta-analysis of previously published studies and found that neoadjuvant chemotherapy (NACT) does not significantly impact the complication rate after breast reconstruction in breast cancer patients. In terms of minor complications, single-stage direct-to-implant (DTI) appears to be a safer procedure than two-staged tissue expander/implant (TE/I).

Summary: Implant-based reconstruction is the most commonly used method for breast reconstruction, while autologous breast reconstruction requires advanced microsurgical skills and anatomical understanding. The choice of reconstruction is made by the patient after discussing all available options with the plastic surgeon. The advantages, disadvantages, and long-term oncologic outcomes of each technique are reviewed.

Summary: This study compared patients' satisfaction with aesthetic outcomes following muscle or fasciocutaneous free flap reconstruction to the lower limb, with patients who underwent fasciocutaneous flap reconstruction being more satisfied with flap texture. However, there were no significant differences in other aspects of satisfaction between the two groups, highlighting the importance of aesthetic restoration in lower limb reconstruction.

Summary: The study investigated the feasibility and aesthetic outcome of free perforator flap breast reconstruction via small lateral chest wall incision after R-NSM. Results showed that patients in the R-NSM group had smaller scars and better aesthetic outcomes in terms of breast symmetry and scar visibility, indicating potential benefits of this approach compared to C-NSM.

Summary: This cross-sectional study comprehensively analyzed autologous and implant-based breast reconstruction procedures, and found that despite the high probability of secondary procedures for aesthetic improvement, autologous breast reconstruction (ABR) was associated with higher patient satisfaction compared to implant-based breast reconstruction (IBR). IBR patients experienced more major complications, while ABR patients had better breast sensitivity. In bilateral reconstruction, IBR tended to have superior aesthetic results. Therefore, the laterality of reconstruction should be considered in the decision-making process.

Summary: This study systematically reviewed the characteristics of perforators in deep inferior epigastric perforator (DIEP) flap breast reconstruction and found that a single dominant perforator and medial row perforators may be associated with a higher risk of fat necrosis. Adopting a standardized perforator selection algorithm could help surgeons make operative decisions and improve postoperative outcomes.

Summary: This study compared the treatments and time intervals between treatments of breast cancer patients diagnosed during and before the COVID-19 pandemic, and the number of treatments started during and before the pandemic. The results showed that patients diagnosed during the transition period were less likely to receive neo-adjuvant chemotherapy for stage I, stage II, and hormone receptor+/HER2- tumors compared to 2018/2019. The time between diagnosis and first treatment decreased for patients diagnosed during the lockdown period. Additionally, there was an increase in the number of neo-adjuvant endocrine therapies and surgeries starting in week 14 of 2020, while the number of adjuvant chemotherapies decreased in week 15 and increased in week 22.

Summary: In this population-based study in the Netherlands, the incidence of unplanned revision surgery was lower in immediate implant-based breast reconstruction using a direct-to-implant approach compared to a two-stage approach. Additionally, more breasts were reconstructed within the planned number of operations in the direct-to-implant group.

Summary: This systematic review compared patient-reported outcomes (PROs) and complication rates after nipple-sparing mastectomy (NSM) and skin-sparing mastectomy (SSM). The study found that NSM resulted in higher scores in sexual and psychosocial well-being, with no differences in overall complication rates between the two groups. Therefore, the decision for NSM or SSM should be made on an individual basis, considering factors such as patient preferences and safety.

Summary: This study investigated the techniques and opinions of Dutch plastic surgeons and oncological breast surgeons regarding the preservation of pectoral fascia (PF) during mastectomy. The results showed that approximately one in five plastic surgeons and breast surgeons routinely preserve the PF, and even more surgeons preserve the PF in specific cases. There were significant differences in surgical techniques and opinions regarding PF preservation among the surgeons. Future studies should clarify the impact of PF preservation on oncological safety, complication rates, postoperative pain, cosmetic outcomes, and patient satisfaction.

Summary: This study aimed to explore the trends in emotional functioning and psychosocial wellbeing of breast cancer survivors, as well as the demographic and treatment characteristics that may influence these trends. The findings showed a decline in psychosocial wellbeing and a steady improvement in emotional functioning over time. Women who underwent breast reconstruction experienced a more significant improvement in emotional functioning, while women with no partner or children showed a marginal decline in psychosocial wellbeing.

Summary: This study aims to reduce the occurrence of fat necrosis by using ICG-NIR-FA technology to evaluate the perfusion of DIEP flaps. A total of 280 females undergoing autologous breast reconstruction with DIEP or msTRAM flaps will be randomly assigned to the intervention or control group. The main endpoint of the study is the difference in clinically relevant fat necrosis between the two groups evaluated at two weeks and three months after reconstruction.

Summary: This study compared the postoperative outcomes of pectoralis major myocutaneous (PMMC) and myofascial (PMMF) flaps for partial hypopharyngeal defect reconstruction. It was found that PMMF flaps had non-inferior results compared to PMMC flaps in terms of postoperative complications and also had some inherent advantages. Therefore, PMMF flaps can be considered as a reliable choice for reconstruction.

Summary: This study examined the factors influencing plastic surgeons' choice of breast implants and found substantial variation in the weighing of these factors among different surgeons. It also revealed that the choice of breast implants is not solely based on scientific evidence, but brand reputation plays a significant role. Therefore, it is crucial for patients to have comprehensive knowledge about all aspects of breast implants during shared decision making.
