Summary: This study compared the diagnostic accuracy of the Ki-67 labeling index (LI) in endoscopic ultrasonography-fine-needle aspiration cytology/biopsy (EUS-FNAC/FNB) and surgical specimens of pancreatic neuroendocrine neoplasms (PanNENs). The concordance rate of tumor grade based on Ki-67 LI was higher between EUS-FNAC/FNB and surgical specimens, indicating the potential usefulness of Ki-67 LI in predicting PanNEN tumor grade in EUS-FNAC/FNB.

Summary: This study aimed to evaluate the use of endoscopic ultrasound-guided fine needle biopsy sampling (EUS-FNB) for the preoperative diagnosis and grading of pancreatic neuroendocrine tumors (PanNETs). The results showed that EUS-FNB had high sensitivity for preoperative diagnosis of PanNETs, but the quality of the biopsy samples was relatively poor, making it suboptimal for preoperative grading.

While both methods are safe and offer high diagnostic value, there are potential limitations with EUS-FNA related to cytological aspirations. Comparative trials between EUS-FNA and EUS-FNB for pancreatic mass lesions have shown conflicting results, with the need for further large-scale studies for other types of lesions.

Summary: This study evaluated factors influencing the diagnostic accuracy of pancreatic EUS-FNA, finding that using a new FNB needle and simultaneous sampling of primary and metastatic lesions can significantly improve diagnostic accuracy. Diagnostic accuracy of >80% was observed after 250 procedures even in the absence of ROSE. Operator skill and double EUS-FNA sampling were also associated with high diagnostic accuracy.

Summary: A new microfluidic device has been developed to diagnose pancreatico-biliary (PB) cancers by detecting circulating tumor cells (CTCs) in peripheral venous blood. The study showed that CTCs had the highest diagnostic accuracy in distinguishing between PB cancer patients and controls, as well as in non-metastatic pancreatic cancer patients (cStage I-III). This new method may offer a promising approach for improving the prognosis of PB cancers.

Summary: Endoscopic submucosal dissection (ESD) is the standard treatment for superficial esophageal squamous cell carcinoma (SESCC), but the treatment strategy for SESCC complicated by esophageal varices (EVs) has not been established. This report presents two cases of SESCC in patients with alcoholic cirrhosis complicated by EVs who underwent ESD. EVL and sclerotherapy were performed before ESD, but in one case, severe bleeding resulted in incomplete ESD.

Summary: Pancreatic cancer is a deadly disease that is usually diagnosed at an advanced stage, making chemotherapy the main treatment option. New chemotherapy treatments have significantly improved the prognosis for unresectable pancreatic cancer. However, these treatments often cause chemotherapy-induced peripheral neuropathy (CIPN) as a severe adverse effect. Currently, there are no recommended agents for CIPN in the ASCO guidelines. This article reviews the methods used to treat CIPN caused by pancreatic cancer treatment, including the efficacy of duloxetine and pregabalin, and the potential use of mirogabalin in the future.
