Summary: Traction techniques play an important role in endoscopic submucosal dissection, providing adequate visualization of submucosal tissue and vasculature for safe and efficient operations. These techniques can be categorized into internal and external traction, allowing personalized traction based on the location of the lesion. Mastering these techniques requires structured training and understanding of their advantages and disadvantages. Future research should focus on training pathways and curriculums, as well as the development of new traction modalities.

Summary: This study retrospectively analyzed 15 patients with gastric adenocarcinoma of the fundic gland type (GA-FG) and evaluated the efficacy and safety of endoscopic submucosal dissection (ESD) for GA-FGs. The results showed that ESD is effective and safe for the treatment of early-stage GA-FGs.

Magnifying endoscopy revealed that GA-FGM lesions met the diagnostic criteria for cancer, while GA-FG lesions did not, suggesting the potential usefulness of magnifying endoscopy with NBI for diagnosing GA-FGM.

Summary: This study aimed to elucidate the endoscopic causes and characteristics of missed gastric cancers (MGCs) after endoscopic submucosal dissection (ESD) for early gastric cancer (EGC). The results showed that the endoscopic causes of MGCs included perceptual errors, exposure errors, sampling errors, and inadequate preparation. Logistic regression analysis revealed the risk factors for perceptual errors and the sites of exposure errors.

Summary: Magnifying and nonmagnifying image-enhanced endoscopy (IEE) show comparable effectiveness in diagnosing gastric intestinal metaplasia (IM). Nonmagnifying IEE demonstrates similar performance as magnifying IEE in evaluating light-blue crest (LBC), white opaque substance (WOS), and endoscopic grading of gastric IM (EGGIM). The presence of high EGGIM score is correlated with gastric cancer (GC) in patients.

Summary: This study compared standard endoscopy and NBI-M in the diagnosis of H. pylori infections, atrophic gastritis, and intestinal metaplasia. Although NBI-M had better sensitivity, the diagnostic accuracy was similar between the modalities. Future studies may help guide endoscopists in choosing the most effective and cost-effective diagnostic modality for difficult cases of H. pylori-related gastritis and precancerous conditions.

Studies have shown significant dysregulation of miRNA expression in superficial ESCC, particularly with miR-21-5p, miR-146b-5p, and miR-210-3p. These miRNAs are associated with ESCC invasion depth and lymphovascular invasion.

Summary: The study evaluated the efficacy of a novel fully covered self-expandable metal stent (SEMS) for the palliation of distal biliary obstruction (DBO) caused by unresectable pancreatic cancer (UPC). The results showed that the incidence of recurrent biliary obstruction (RBO) was lower and the time to RBO (TRBO) was longer in the HFS group compared to the other two groups. Additionally, the incidence of complications was lower in the HFS group. Therefore, the use of HFS stent is recommended for the palliation of DBO due to UPC.
