Summary: This study reports the outcomes and pharmacokinetic parameters in patients who massively ingested DOACs. No bleeding was observed despite elevated plasma concentrations. No patient required specific haemostatic agents. Charcoal administration may contribute to limit toxicity.

Summary: Factors influencing the initiation and choice of oral anticoagulants in Australian general practice patients newly diagnosed with atrial fibrillation were investigated. The study found a marked increase in the proportion of newly diagnosed AF patients initiated on OAC, especially following the introduction of DOACs. The use of DOACs also increased significantly, with the majority of OAC initiations transitioning to DOACs by the end of the study period.

Summary: The review examined the use of DOACs for VTE treatment and prevention in patients with obesity, as well as their application in bariatric surgery. Updated guidance recommendations on the use of DOACs in patients with obesity for VTE treatment and prevention were provided.

Summary: The case highlights the high risk of thromboembolic complications in patients with atrial fibrillation, necessitating timely and effective pharmacological treatment. Furthermore, the use of direct oral anticoagulants in atrial fibrillation patients with bioprosthetic heart valves remains controversial due to an unclear definition of nonvalvular atrial fibrillation.

Summary: This study aimed to describe the baseline characteristics of patients with oral anticoagulant-related intracerebral hemorrhage (OAC-ICH) in Sweden and identify predictive variables associated with receiving hemostatic treatment. The study found that only 48% of patients with non-Vitamin K oral anticoagulant associated ICH received reversal treatment. Age, previous stroke, comatose LOC, pre-stroke dependency, and type of anticoagulant drug were factors associated with lower odds of receiving reversal treatment.

Summary: Little is known about the post-transplant use of direct oral anticoagulants (DOACs) in patients after heart transplantation (HTX). We investigated the frequency, indications, and complications of DOACs and vitamin K antagonists (VKAs) in HTX recipients. The results showed that DOACs were associated with significantly fewer bleeding complications compared to VKAs, but there were no differences in the risk of ischemic stroke, thromboembolic events, or OAC-related death between the two groups.

Summary: The adherence to direct oral anticoagulants (DOACs) among Dutch patients with atrial fibrillation (AF) was suboptimal, and determinants negatively affecting adherence included conflicting information, lack of shared decision making, and patients with higher educational level. Efforts towards identifying suboptimally adherent DOAC patients, adherence counselling, and reinforced education of both patients and healthcare providers are key in improving adherence.

Summary: Studies comparing the efficacy and safety of direct oral anticoagulants (DOAC) versus vitamin K antagonist (VKA)/low molecular weight heparin (LMWH) in the treatment of venous thromboembolism (VTE) in obese patients showed similar VTE recurrence rates between the two treatment groups, but DOAC were associated with a reduced risk of major bleeding.

Summary: This study translated the Jessa Atrial fibrillation Knowledge Questionnaire (JAKQ) into Chinese and evaluated its reliability and validity. The results showed that the Chinese version of JAKQ has good reliability and validity and can be used to guide educational activities to improve the effectiveness and safety of atrial fibrillation oral anticoagulation (OAC) treatment. Chinese patients with atrial fibrillation have insufficient knowledge about atrial fibrillation and OAC. Lower JAKQ scores are associated with bleeding, indicating the need for targeted education.

Summary: This study presents a series of cases involving seven patients diagnosed with HIT and treated with DOACs at four healthcare centers in Paris. The patients recovered their platelet count within an average of 3.3 days after discontinuation of heparin, and no major bleeding or thrombotic events related to DOAC treatment were reported during follow-up. These findings support the potential and safe use of DOACs in clinically stable patients with acute HIT.

Summary: This study proposes a novel approach to train medical students' diagnosis ability, called Virtual Standard Patient (VSP). By constructing an oral knowledge graph and designing personalized templates, VSP provides realistic and relevant disease clues, interacts accurately with dentists, and expresses symptom characteristics in a natural style. The results of user studies demonstrate that VSP satisfies the requirements of medical students' diagnosis practice in terms of naturalness, realism, and topic relevance.

Summary: This study evaluated patients' knowledge about their oral anticoagulants and the potential role for community pharmacists in optimising safety. Findings indicated that patients had reasonable knowledge of their anticoagulation therapy, but areas for improvement were identified. Community pharmacists are well positioned to play a vital role in promoting the safe use of anticoagulants.

Summary: Direct oral anticoagulants (DOACs) are commonly used for stroke prevention in atrial fibrillation patients. However, the association between medication adherence, patient satisfaction, and treatment knowledge is not well known. This study found suboptimal adherence to DOACs and no association between treatment satisfaction, knowledge, and DOAC adherence over a 6-month period. Patient education and follow-up are needed to address knowledge gaps.

Summary: The study evaluated the outcomes of patients with atrial tachyarrhythmia undergoing elective electrical cardioversion (ECV) with DOAC or VKA treatment in a tertiary referral center. Results showed that the rates of thromboembolism and major bleeding events were low after ECV, even without routine TEE, in patients treated with DOAC or VKA.
