Objectives: To describe a post platelet-rich plasma (PRP) injection, exercise-based physical therapy program, investigate feasibility and report the first results of patellar tendinopathy patients treated with PRP injection combined with the physical therapy program. Study Design: Case-series. Setting: A PRP injection followed by a physical therapy program seems promising for the treatment of patellar tendinopathy. However, descriptions of physical therapy programs are often limited and incomplete. Participants: Five patellar tendinopathy patients (six tendons) in the degenerative phase. Main outcome measure: VISA-P score. Results: Muscle strength, endurance, power and retraining sport-specific function form the basis for the physical therapy program aiming to improve the load capacity of the knee. The program is characterised by gradually increasing intensity and difficulty of exercises. Five of the six tendons showed an improvement of at least 30 points on the VISA-P after 26 weeks. Conclusions: This study extensively describes, based on current knowledge, a physical therapy program after PRP injection for patellar tendinopathy patients. The combination treatment reported in this study is feasible and seems to be promising for patients in the late/degenerative phase of patellar tendinopathy. (C) 2012 Elsevier Ltd. Validity and reliability of the German translation of the Diabetes Foot Self-Care Behavior Scale (DFSBS-D)

Linda Lecker, Martin Stevens, Florian Thienel, Djordje Lazovic, Inge Van den Akker-Scheek, Gesine H. Seeber

Summary: To improve diabetic foot syndrome prevention, a valid and reliable instrument for measuring daily foot-care routines in patients with diabetes is needed. This study translated and adapted the Diabetic Foot Self-Care Behavior Scale into German, and evaluated its validity and reliability.

Cost-effectiveness of ACL treatment is dependent on age and activity level: a systematic review

R. Deviandri, H. C. van der Veen, A. M. T. Lubis, I Van den Akker-Scheek, M. J. Postma

Summary: This study systematically reviewed health-economic evaluations of anterior cruciate ligament (ACL) injury and compared the cost-effectiveness of reconstruction surgery (ACLR) and non-operative treatment (NO). The results suggest that early ACLR may be the more cost-effective strategy for athletes and young populations with high-activity levels, while non-operative treatment with optional delayed ACLR may be more cost-effective. However, for middle-aged individuals with moderate activity levels, early ACLR may not be more cost-effective than delayed ACLR.

Clinical relevance of joint line obliquity after high tibial osteotomy for medial knee osteoarthritis remains controversial: a systematic review

Tianshun Xie, Reinoud W. Brouwer, Inge van den Akker-Scheek, Hugo C. van der Veen

Summary: This study aimed to review the literature on the association between knee joint line obliquity (KJLO) and clinical outcome after high tibial osteotomy (HTO) for medial knee osteoarthritis and summarize the KJLO cut-off value used in these studies. The results showed conflicting findings on the associations between postoperative KJLO and patient-reported outcome, medial knee cartilage regeneration, and 10-year surgical survival. The clinical relevance of KJLO after HTO remains controversial.

High-demand tasks show that ACL reconstruction is not the only factor in controlling range of tibial rotation: a preliminary investigation

Mark Zee, Michele Keizer, Jos van Raaij, Juha Hijmans, Inge van den Akker-scheek, Ron Diercks

Summary: The study aimed to determine whether the range of tibial rotation (rTR) increases after ACL injury and whether it returns to normal after ACL reconstruction (ACLR). The results showed that rTR increased during high-demand tasks compared to low-demand tasks. Before ACLR, rTR was smaller in the ACL-deficient knees compared to the contralateral knees. After ACLR, rTR increased compared to pre-operative but was still smaller than the contralateral knees.

Summary: This study aimed to translate the ACL-RSI scale into Indonesian and evaluate its validity and reliability in Indonesian-speaking patients after ACL reconstruction. After a forward-and-backward translation procedure, the I-ACL-RSI was found to have good construct validity and test-retest reliability in Indonesian patients.

Assessment of joint line obliquity and its related frontal deformity using long-standing radiographs

Tianshun Xie, Hugo C. van der Veen, Inge van den Akker-Scheek, Reinoud W. Brouwer

Summary: This study investigated the influence of radiographic techniques and osteoarthritis grade on measurements of knee joint line obliquity (KJLO) and KJLO-related frontal deformity. It was found that measurements of JLOAF, JLOAM, JLOAT, MJLA, JLCA, and HKA were influenced by single-leg/double-leg standing, while JLOAF, JLOAM, and JLOAT were also affected by bipedal distance in double-leg standing, and JLCA was affected by osteoarthritis grade. MPTA measurement of knee joint obliquity was not affected by standing position, bipedal distance, or osteoarthritis grade, and had excellent reliability. Thus, MPTA is proposed as the preferable KJLO measurement method for clinical practice and future research.

Summary: This study aimed to identify barriers and facilitators perceived by primary and secondary healthcare professionals in implementing lifestyle-related treatment modalities in patients with hip/knee osteoarthritis. The findings suggest that focusing on societal factors rather than individual factors could improve the implementation of these treatment modalities.

Joint line obliquity after lateral closing-wedge high tibial osteotomy does not adversely affect clinical and radiological outcome: a 5-year follow-up study

Tianshun Xie, Maarten R. Huizinga, Inge van den Akker-Scheek, Hugo C. van der Veen, Reinoud W. Brouwer

Summary: This study aimed to analyze the association between change in knee joint line obliquity (KJLO) and patient-reported outcome, radiological progression of osteoarthritis, and surgical survival after lateral closing-wedge high tibial osteotomy (HTO). The results showed that increased KJLO (postoperative MPTA ≥ 95.0°) or MPTA change ≥ 8.0° after HTO did not adversely affect patient-reported outcome, radiological progression of osteoarthritis, or surgical survival at an average 5-year follow-up.

Responsiveness of the Indonesian Versions of the Anterior Cruciate Ligament-Return to Sport After Injury Score, the International Knee Documentation Committee Subjective Knee Form, and the Lysholm Score in Patients With ACL Injury

Romy Deviandri, Hugo C. van der Veen, Andri M. T. Lubis, Maarten J. Postma, Inge van den Akker-Scheek

Summary: The purpose of this study was to determine the responsiveness of ACL-RSI, IKDC, and Lysholm scores in an Indonesian-speaking population with ACL injury. The results showed that all three scores had good responsiveness and can be used for follow-up of patients after ACL injury. At the individual level, IKDC was found to be more efficient than ACL-RSI or Lysholm scores in detecting clinically important changes over time after ACL treatment.
