Acupuncture, as a traditional Chinese medicine therapy, has been found to improve CP/CPPS symptoms and reduce pain. This study aims to summarize the characteristics and benefits of acupuncture therapy for CP/CPPS and analyze its related mechanisms.

Recent findings indicate that the autonomic nervous system plays a critical role in the pathogenesis of CP/CPPS. This review summarizes the detailed processes and mechanisms of the involvement of the autonomic nervous system in developing CP/CPPS and provides new perspectives for new therapeutic approaches.

Summary: Acupuncture shows significant efficacy in reducing symptoms and improving quality of life in patients with CP/CPPS, especially in relieving pain and urinary symptoms. Acupuncture is more effective than sham acupuncture and standard medication, with a minimum of four acupuncture sessions required for clinical efficacy. Prolonged acupuncture sessions can continuously improve urinary symptoms and quality of life. The combination of CV3, CV4, BL32, SP6, and SP9 acupoints is commonly used for treatment.

Summary: This study aimed to evaluate the clinical outcomes of using the PDE-5 inhibitor tadalafil 5 mg OD for the management of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). The results showed that tadalafil significantly improved the symptoms in various domains of CP/CPPS compared to placebo. Approximately 50.8% of patients in the tadalafil group showed clinically significant improvement. However, there was weak correlation between tadalafil-induced changes in IIEF-5 scores and the improvement in quality of life.

Summary: Chronic pelvic pain (CPP) refers to the pain in the pelvic region that lasts longer than six months. Monotherapy with medication may not be sufficient for CPP pain management, and multidisciplinary approaches are recommended. This study evaluated the efficacy of acupuncture compared to a control group for managing CPP. A meta-analysis of 17 randomized controlled trials involving 1455 patients showed that acupuncture significantly decreased pain levels in both total pain scores and visual analogue scale/numerical rating scale data compared to the control group. Acupuncture as a monotherapy also resulted in significantly lower pain levels than the control group. These findings suggest that acupuncture may be beneficial for managing CPP pain, even when used as a standalone treatment.

Summary: This study aimed to assess the impact of surgical experience on peri-operative, functional and oncological outcomes in the first 50 cases of Retzius-sparing robot-assisted radical prostatectomy performed by surgeons new to this approach. The results showed that console time, immediate urinary continence recovery, and postoperative complications were optimal in the expert group compared to the initial group, while positive surgical margin rates did not significantly improve.

Summary: This study compared the first-year oncological and functional outcomes between Retzius-sparing robot-assisted radical prostatectomy (Rs-RARP) and standard robot-assisted radical prostatectomy (RARP). The study found that Rs-RARP had a shorter catheter removal time and faster recovery of urinary continence. However, the overall continence rates at the end of one-year follow-up were similar between the two groups.
