Summary: Epilepsy and depression often coexist, and there is a bidirectional relationship between the two disorders. However, current evidence on the effectiveness of antidepressant therapy in patients with epilepsy and comorbid depression is limited. Vortioxetine, a multimodal antidepressant, shows promise as a treatment option for depressed patients with cognitive dysfunctions, anhedonia, and anxiety.

Summary: Psychotic depression is considered an independent trait that may accompany mood disorders of varying severity, with acute treatment typically involving a combination of antidepressants, antipsychotic drugs, or electroconvulsive therapy. Limited information exists on maintenance treatment for unipolar psychotic depression and on acute and chronic treatment for psychotic bipolar disorder, leading to a reliance on clinical experience for treatment decision-making.

Summary: Lithium may play an important role in treating comorbid anxiety in bipolar disorder, both as adjunct and monotherapy. Lower doses of lithium may provide equivalent efficacy and enhance tolerability and compliance.

Summary: This study found that evening-types show more pronounced mental health symptoms, diagnoses, and hospital treatments, especially when assessed by circadian type. Insufficient sleep mediates most but not all of the associations between eveningness and mental health.

Summary: This study evaluated the impact of individual genes in the response to treatment in bipolar depression, finding that pharmacogenetic variables can predict patient response to medication and tolerability. These predictors could be useful for guiding pharmacological treatment in bipolar disorder, but further clinical studies are needed to confirm these findings.

Summary: This article discusses the variability of antidepressant effectiveness in treating major depressive disorder among patients, with genetic factors playing a significant role in individual responses. The integration of genomics and pharmacology can provide guidance for personalized treatment of depression.

Summary: This study conducted a meta-analysis to explore the comorbidity between mood and anxiety disorders, finding consistent evidence of a substantial association regardless of diagnostic criteria, study timeframe, or adjustments. Clinicians should be aware of the high prevalence of this common comorbidity and ensure prompt identification and treatment.

Summary: This systematic review on the impact of disease progression in the treatment of bipolar disorder patients found that efficacy decreases with clinical progression, supporting early intervention. However, tailored recommendations cannot be made based on disease stages currently, highlighting the need for further research.

Summary: This study confirmed the presence of a depressive continuum with sadness being a common symptom in the general population and sharing correlations with Major Depressive Disorder (MDD). They have shown a continuum where sadness serves as an intermediate state between well-being and psychiatric disorders. With high sensitivity and specificity, sadness appears to be a clear MDD prodrome and an at-risk state, potentially indicating a symptom of a transdiagnostic distress process.

Summary: This study found a possible association between genotype related to dopamine neurotransmission and antidepressant treatment response. DRD4 and MAOB genotypes were linked to improved HAMD 17 scores, but the preliminary nature of the results is noted due to the limited sample size. Further research into the role of habenular dopamine D4 receptors in antidepressant response is recommended.

Summary: The gut microbiota plays a crucial role in maintaining homeostasis, growth, and digestive processes. It also influences the intercommunication system between the gut and the brain, potentially impacting the pathogenesis and clinical management of Bipolar Disorder. Current literature suggests a relationship between compositional alterations in the gut microbiota and BD, with potential benefits in treating BD symptoms through modifications in the microbiota composition. Further research is needed to explore the potential of correcting gut microbiota alterations as a novel strategy in BD management.

Summary: This review examines the genetic overlap between various traits and antidepressant treatment response in patients with unipolar MDD using polygenic risk scores (PRS). The results suggest that PRSs for several traits, including attention-deficit hyperactivity disorder and coronary artery disease, may be associated with antidepressant treatment response. However, the studies varied in sample size and outcome measures, highlighting the need for larger cohorts and standardized assessment methods.

Summary: The study found that intravenous ketamine can significantly improve cognitive function in TRD patients, especially in tests like DSST and PDQ-5-D. This improvement is mainly mediated by reduced depressive symptoms, indicating that ketamine has independent and rapid effects in promoting cognition.

Summary: The study focused on investigating the expression levels of genes involved in drug disposition and action in tissues of pharmacological importance in patients treated with psychotropic drugs. Promising findings include the role of genes in inflammation, neurotransmission, and mitochondrial function. However, studies on antipsychotics or mood stabilizers often had smaller sample sizes and lacked replication compared to studies on antidepressants.

Summary: This study investigated the associations between LEPR gene polymorphisms and treatment response to valproate (VPA) in patients with bipolar disorder (BD). The results showed that LEPR gene polymorphisms significantly affected changes in disease severity after VPA treatment in BD patients, and the LEPR CAGG haplotype was associated with a better treatment response.
