Increased intra- and extracellular granzyme expression in patients with tuberculosis

Tuberculosis (TB) is an important cause of morbidity and mortality worldwide. Granzymes (gzms) are proteases mainly found in cytotoxic lymphocytes, but also extracellularly. While the role of gzms in target cell death has been widely characterized, considerable evidence points towards broader roles related to infectious and inflammatory responses. To investigate the expression of the gzms in TB, intracellular gzms A, B and K were measured by flow cytometry in lymphocyte populations from peripheral blood mononuclear cells from 18 TB patients and 12 healthy donors from Bangladesh, and extracellular levels of gzmA and B were measured in serum from 58 TB patients and 31 healthy controls. TB patients showed increased expression of gzmA in CD8(+) T, CD4(+) T and CD56(+) T, but not NK, cells, and of gzmB in CD8(+) T cells, when compared to controls. GzmK expression was not altered in TB patients in any lymphocyte subset. The extracellular levels of gzmA and, to a lesser extent, of gzmB, were increased in TB patients, but did not correlate with intracellular gzm expression in lymphocyte subsets. Our results reveal enhanced intra- and extracellular expression of gzmA and B in patients with pulmonary TB, suggesting that gzms are part of the host response to tuberculosis. (C) 2015 Elsevier Ltd. Summary: Leptospirosis is a global zoonotic and waterborne disease that poses a significant public health problem. It is often difficult to diagnose due to its similarity to other tropical diseases, and there is increasing evidence of chronic symptoms that can go unrecognized. Kidney involvement is common in leptospirosis, and early recognition of severe cases is crucial for reducing morbidity and mortality. This review highlights the spectrum of characteristics seen in leptospirosis kidney disease, as well as the diagnostic and treatment methods.

Summary: In patients with uncomplicated P. falciparum malaria, the nadir hemoglobin occurs 2 days after starting treatment. Factors associated with moderately severe anemia on day 7 in Africa and Asia include severe anemia at baseline, young age, high parasitemia, and delayed parasite clearance. In Asia, artemisinin-based treatments are associated with a greater risk of anemia during recovery compared to non-artemisinin-based treatments.

Summary: This study evaluated the operational aspects of biosensor deployment in the hands of village malaria workers in Cambodia. The results showed that skills using the biosensor improved progressively, and VMWs expressed confidence and enthusiasm in using the biosensor. Providing G6PD testing at the point of first contact can overcome barriers for patients and deploying biosensors in routine work of VMWs can expand and strengthen their role as healthcare providers in the community. However, VMWs also reported some practical difficulties in using the biosensor.

Summary: As a disaster-prone country, snake bites are a major public health concern in Bangladesh. Late presentation to the hospital, low efficacy of antivenom, and a lack of adequate management facilities are the primary reasons for mortality from snakebite. This study evaluated the neutralization efficacy of Incepta antivenom against the highly venomous Bungarus caeruleus snake in Bangladesh. The venom was found to possess phospholipase A2 activity, hemolytic activity, and edema inducing activity, while hemorrhagic activity was absent.
