Safe time limits of aortic cross-clamping and cardiopulmonary bypass in adult cardiac surgery

Objectives: We evaluated the impact of aortic cross-clamping time (XCT) and cardiopulmonary bypass time (CPBT) on the immediate and late outcome after adult cardiac surgery and attempted to identify their safe time limits. Methods: This study includes 3280 patients who underwent adult cardiac surgery of various complexities. Myocardial protection was achieved with tepid continuous antegrade/retrograde blood cardioplegia. Results: Receiver operating characteristics (ROC) curve analysis showed that XCT (area under the curve, AUC: 0.66), CPBT (AUC: 0.73) and CPBT with unclamped aorta (AUC: 0.77) were significantly associated with 30-day postoperative mortality. XCT of increasing 30-minute intervals (Odds Ratio (OR) 1.21, 95% C.I. 1.01-1.52) and CPBT of increasing 30-minute intervals (OR 1.47, 95% C.I. 1.27-1.71) were independent predictors of 30-day mortality. The best cutoff value for XCT was 150 min (30-day death: 1.8% vs. 12.2%, adjusted OR 3.07, 95% C.I. 1.48-6.39, accuracy 91.5%) and for CPBT 240 min (30-day death: 1.9% vs. 31.5%, adjusted OR 8.78, 95% C.I. 4.64-16.61, accuracy 96.0%). These parameters were significantly associated also with postoperative morbidity, particularly with postoperative stroke. Conclusions: XCT and CPBT are predictors of immediate postoperative morbidity and mortality. In our experience, cardiac procedures with CPBT<240 min and XCT<150 min were associated with a rather low risk of immediate postoperative adverse events independently of the complexity of surgery patient's operative risk.

Statin use improves survival of patients with known or suspected lower extremity artery disease on all ankle brachial index levels

Summary: This study analyzed the impact of statin use on survival and cardiovascular mortality in patients with LEAD of different severity depicted by the ABI level. The results showed that statin use improved overall survival and amputation free survival regardless of ABI level. Statin use was also associated with lower mortality from cerebrovascular disease, overall mortality, and cardiovascular mortality in patients with ABI<0.9 or >1.3.

Summary: The aim of this study was to evaluate the outcomes of different surgical strategies for acute TAAD. The study found that replacement of the aortic root and aortic arch did not decrease the risk of aortic reoperation in TAAD patients and should be performed only in the presence of local aortic injury or aneurysm.

Summary: This study aimed to evaluate the impact of individual institutions on the outcome of surgery for Stanford type A aortic dissection. The results showed that certain hospitals had a higher risk of in-hospital mortality, while others had a lower risk. Hospitals with lower in-hospital mortality rates also had a decreased rate of stroke and/or global brain ischemia. The study concluded that hospital volume may be a determinant of poor outcome in surgery for aortic dissection.

Summary: This study evaluated the association between pre-V-A-ECMO arterial lactate level and in-hospital mortality in postcardiotomy patients. The results showed that increased arterial lactate level before V-A-ECMO initiation was associated with higher in-hospital mortality. Therefore, arterial lactate level may be useful in guiding the decision-making process and timing of initiation of postcardiotomy V-A-ECMO.

Surgical Strategy for the Repair of Acute Type A Aortic Dissection: A Multicenter Study

Summary: Type A acute aortic dissection requires prompt referral to tertiary referral centers for urgent imaging and management. Surgery, the choice of which depends on the patient and presentation, and center expertise, plays a significant role. This study compared outcomes of conservative approaches focused on the ascending aorta and hemiarch with the extensive surgery (total arch reconstruction and root replacement) across three European referral centers. The study found no significant difference in surgical mortality between the two approaches, but identified predictors of mortality including age, arterial lactate levels, and emergency or salvage status.

Sex-Based Difference in Aortic Dissection Outcomes: A Multicenter Study

Summary: This retrospective study analyzed data from three departments of cardiac surgery between January 2005 and December 2021 and found a higher proportion of women undergoing aortic arch repair. There were no significant differences in operative mortality and early postoperative neurological complications between men and women. Preoperative arterial lactate levels and mesenteric ischemia after surgery were significantly associated with increased operative mortality in women.

Summary: The aim of this study was to investigate the impact of coronary artery disease (CAD) and different revascularization strategies on clinical outcomes in patients who underwent transcatheter aortic valve implantation (TAVI) with third generation devices. The presence of CAD did not have an impact on overall mortality and major adverse cardiac and cerebrovascular events (MAC-CE) at 1 year after TAVI, but CAD patients had a higher risk for myocardial infarction and need for percutaneous coronary intervention during follow-up.

Incidence and predictors of 30-day and 6-month stroke after TAVR: Insights from the multicenter OBSERVANT II study

Summary: This study aimed to investigate the incidence and predictors of stroke at 30 days and 6 months after transcatheter aortic valve replacement (TAVR). The results showed that stroke occurrence was low but had a significant impact on survival. Predilatation, diabetes, and left ventricle ejection fraction were found to be independent predictors of 30-day stroke, while diabetes, pre-existing neurological dysfunction, bicuspid valve, and critical status were predictive of 6-month stroke. Antiplatelet therapy and anticoagulation were found to be protective factors at both 30 days and 6 months.

Relative Survival After Adult Cardiac Surgery: Analysis of the Italian Nationwide Registry Data

Summary: This study aimed to investigate life expectancy after adult cardiac surgery. It included adult patients who underwent heart valve and coronary artery surgery from a nationwide administrative registry. The results showed that the 10-year survival rate was 58.0%, with a relative survival rate of 0.83. Different surgical procedures had varying relative survival rates, with surgical replacement of heart valves having a relative survival rate below 0.80.
