New Approach in Fibromyalgia Exercise Program: A Preliminary Study Regarding the Effectiveness of Balance Training

Objectives: To determine the effectiveness of balance exercises on the functional level and quality of life (QOL) of patients with fibromyalgia syndrome (FMS) and to investigate the circumstances associated with balance disorders in FMS. Design: Randomized controlled trial. Setting: Physical medicine and rehabilitation clinic. Participants: Patients (N=57) (age range, 18-65y) with FMS were randomly assigned into 2 groups. Interventions: Group 1 was given flexibility and balance exercises for 6 weeks, whereas group 2 received only a flexibility program as the control group. Main Outcome Measures: Functional balance was measured by the Berg Balance Scale (BBS), and dynamic and static balance were evaluated by a kinesthetic ability trainer (KAT) device. Fall risk was assessed with the Hendrich II fall risk model. The Nottingham Health Profile, Fibromyalgia Impact Questionnaire (FIQ), and Beck Depression Inventory (BDI) were used to determine QOL and functional and depression levels, respectively. Assessments were performed at baseline and after the 6-week program. Results: In group 1, statistically significant improvements were observed in all parameters (P<.05), but no improvement was seen in group 2 (P>.05). When comparing the 2 groups, there were significant differences in group 1 concerning the KAT static balance test (P=.017) and FIQ measurements (P=.005). In the correlation analysis, the BDI was correlated with the BBS (r=-.434) and Hendrich II results (r=.357), whereas body mass index (BMI) was correlated with the KAT static balance measurements (r=.433), BBS (r=-.285), and fall frequency (r=.328). Conclusions: A 6-week balance training program had a beneficial effect on the static balance and functional levels of patients with FMS. We also observed that depression deterioration was related to balance deficit and fall risk. Higher BMI was associated with balance deficit and fall frequency. Clinicians should exercise caution when prescribing these drugs.

Summary: This study evaluated the performance of the SLERPI in identifying SLE in a large cohort of UCTD patients. The results showed that the SLERPI was able to identify a significant proportion of SLE patients in the UCTD cohort, which may be useful for the early identification of SLE among patients with signs of CTD without meeting any definite criteria.

A single-center COVID-19 vaccine experience with CoronaVac and BNT162b2 in familial Mediterranean fever patients

Summary: The aim of this study was to determine the frequency of adverse events and attacks related to vaccination in recipients of CoronaVac and BNT162b2 in familial Mediterranean fever (FMF) patients, and to investigate whether a history of prior COVID-19 infection or a booster dose increased the occurrence of adverse events/attacks. The results showed that a considerable number of FMF patients experienced vaccine-related adverse events/attacks, particularly with the BNT162b2 vaccine. However, no serious events or deaths due to vaccination were detected. Demographic, clinical characteristics, and prior history of vaccination did not significantly affect the results. An increased rate of adverse events/attacks was observed with booster dose administration.

Summary: The study aimed to evaluate the impact of sulfasalazine therapy on COVID-19 outcomes in patients with Axial Spondyloarthritis. The results suggest that sulfasalazine may be associated with more severe COVID-19 in AxSpA patients. Therefore, these patients should be closely followed in the presence of COVID-19, and discontinuation of sulfasalazine should be considered.

The relationship between dysphagia and the localisation of brain lesion in stroke: is the involvement of the pons and medulla important?

Summary: This retrospective study examines the relationship between dysphagia in stroke patients and patient demographics, as well as the type and localization of brain lesions. The results show that lesions in the pons and medulla are more likely to result in tube-dependent intake and a higher incidence of pneumonia compared to lesions in the MCA cortex, basal ganglia, and cerebellum.

COVID-19 disease frequency, risk factors, and re-infection rates in patients with autoimmune rheumatic disease receiving rituximab

Summary: This study evaluated the frequency, risk factors, and re-infection rates of COVID-19 in patients with autoimmune rheumatic diseases receiving rituximab. The results showed that hypogammaglobulinemia was a significant risk factor for COVID-19 development, and advanced age was associated with more severe disease. Additionally, a high rate of COVID-19 re-infection was observed.

Summary: Breastfeeding may have positive effects on the course of Behcet's disease, as breastfed patients had a lower need for TNF-i and a lower incidence of sacroiliitis. Additionally, patients who were breastfed were diagnosed with the disease at a later age compared to those who were not breastfed.

Neurological Involvement in Patients with Primary Sjögren's Syndrome: A Retrospective Cross-Sectional Study

Summary: In this study, it was found that one third of symptomatic pSS patients had objective neurological involvement. Patients with neurological involvement had a higher frequency of certain clinical features, including positive Schirmer's test, anti-Ro/SS-A and anti-La/SS-B antibodies, and the presence of interstitial lung disease, articular involvement, lymphadenopathy, anemia, and hypocomplementemia. Articular involvement was identified as a higher risk factor for the development of neurological involvement.

Summary: This study aimed to determine the association between the presence of non-criteria antiphospholipid antibodies (aPLs) and the clinical and laboratory features of patients with antiphospholipid syndrome (APS). 80 patients diagnosed with APS were assessed, and the presence of non-criteria antibodies was documented. The study found that a significant number of APS patients tested positive for non-criteria antibodies, which may be linked to the immunological activity of systemic lupus erythematosus (SLE).
