Summary: The study compared the concentrations of proinflammatory mediators of arachidonic acid (ARA) and linoleic acid (LA) in patients with CD and UC, and identified differences in inflammation levels between the two diseases. Active CD patients had higher concentrations of 15S-HETE than active UC patients, and levels of TXB2, LTB4, and 9S-HODE were higher in active CD compared to inactive CD.

Summary: Indeterminate colitis (IC) is a type of inflammatory bowel disease that can be difficult to differentiate from ulcerative colitis (UC) or Crohn's disease (CD) in the early stages, but most patients will ultimately be diagnosed with one of the two diseases after a period of observation.

Summary: The study identified that IgG4 subclass deficiency is common in patients with IBD and associated with markers of disease severity. Treatment with intravenous Ig replacement therapy was beneficial in patients with IgG4-deficient severe IBD. Further research is needed to understand the mechanistic role of IgG4 deficiency in this subset of severe IBD patients.

Summary: The safety and effectiveness of surgical treatment for hemorrhoidal disease in patients with inflammatory bowel disease (IBD) is currently poorly supported by evidence. However, cautious surgical management of non-responsive CD patients with inactive disease is still recommended, though further studies are needed to assess the potential benefits of non-excisional techniques in this population.

Summary: Despite being an important issue for patients, the impact of diet on adult inflammatory bowel disease (IBD) is not well explored, with limited guidance. Current evidence on various diets used in the treatment of IBD is summarized, with potential applications of dietary data from related immune-mediated inflammatory diseases (IMIDs) to inform providers. Multiple diets, including Mediterranean, vegetarian/vegan, and reduced-calorie/fasting diets, have shown positive results in other inflammatory diseases, suggesting potential applicability to IBD, but further well-designed clinical trials are needed for guidance.

Summary: This study aimed to develop and externally validate a prediction model for new-onset chronic uveitis in children with juvenile idiopathic arthritis (JIA). Data from the international Pharmachild registry were used to develop a multivariable Cox proportional hazards model. The model was subsequently validated and recalibrated in two inception cohorts. The results showed that the model had acceptable performance and could be used for risk estimation in clinical practice.

Summary: This study reports the outcomes of an enhanced recovery after surgery (ERAS) protocol in pediatric inflammatory bowel disease (IBD) surgery. Implementation of the ERAS protocol resulted in a significantly lower postoperative complication rate and earlier first defecation in children with IBD. However, there was no significant difference in the length of hospital stay between the ERAS group and the non-ERAS group. Further pediatric studies are needed to validate these findings and support the application of ERAS in children with IBD.

Summary: This study analyzed the treatment of pediatric patients with recurrent pericarditis using anti-IL-1 therapy. The study found that most patients received inadequate first-line treatment and only a few achieved drug-free remission. The effectiveness of anakinra was supported, but the different response rates to anakinra and canakinumab may suggest a role of IL-1 alpha in the pathogenesis of recurrent pericarditis.

Summary: Inborn errors of metabolism (IEM) can manifest as emergencies during acute metabolic decompensation, necessitating specific treatments to prevent morbidity and mortality. Metabolic imbalance is often triggered by diet or concurrent illnesses, with nonspecific symptoms that increase the risk of missed diagnosis. The principle of acute management is to halt further decompensation and ensure adequate metabolic requirements are met.
