Polar Effects on Ion Transport and Cell Proliferation Induced by GC-C Ligands in Intestinal Epithelial Cells

Guanylin receptor guanylate cyclase (GC-C) peaks in neonatal intestine and is involved in either enterocyte proliferation or chloride secretion. The latter is more potent when GC-C activator guanylin, or its analog Escherichia coli heat-stable enterotoxin (ST), is added to the mucosal rather than serosal side of intestinal monolayers. By using Ussing chambers, we investigated transepithelial ion transport and enterocyte proliferation and their mechanisms in response to the addition of guanylin or ST to the mucosal or serosal side of Caco-2 monolayers and in ileal specimens from neonates. GC-C activation showed a polar pattern of the effects. GC-C mucosal activation resulted in a potent cGMP-chloride secretion activation and in a marginal enterocyte proliferation. Conversely, serosal GC-C activation induced a potent enterocyte proliferation, through MAP kinase ERK 1/2. Finally, the inhibition of ERK 1/2 enhanced the Isc increase in response to serosal but not to mucosal ST stimulation, indicating that ERK 1/2 also acts as a brake of chloride secretion. These data suggest that the guanylin/GC-C system plays a key role in early postnatal intestinal adaptation exploiting the polar structure of enterocyte. (Pediatr Res 69: :17-22, 2011)

Summary: This study aimed to assess the impact of COVID-19 lockdown on the rate of disease flare in children with juvenile idiopathic arthritis (JIA). The results showed that the rate of joint inflammation in JIA during the COVID-19 lockdown was significantly higher compared to the previous year. These findings highlight the need for rearrangement in the home and health care management of children with JIA during lockdowns.

Pediatric Inguinal Hernia Repair, Laparoscopic Versus Open Approach: A Systematic Review and Meta-analysis of the Last 10-Year Evidence

Summary: In this study, a systematic review and meta-analysis were performed to compare outcomes between laparoscopic herniorrhaphy (LH) and open herniorrhaphy (OH) in pediatric patients. The results showed that LH had similar operative time, lower complications, and comparable recurrence rate to OH. Closure of contralateral patent processus vaginalis resulted in a significant decrease of contralateral metachronous hernia.

Synbiotics in the Management of Pediatric Gastrointestinal Disorders: Position Paper of the ESPGHAN Special Interest Group on Gut Microbiota and Modifications

Summary: This article aims to provide recommendations for the use of synbiotics in the management of pediatric gastrointestinal disorders. Based on the current evidence, there is no sufficient support or opposition for the use of synbiotics in the treatment of acute gastroenteritis, prevention of necrotizing enterocolitis, Helicobacter pylori infection, inflammatory bowel disease, functional gastrointestinal disorders, and allergy in infants and children. More well-designed randomized controlled trials are needed to study the role of synbiotics in gastrointestinal disorders.

Summary: The European Society for Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN) provides updated recommendations for the use of probiotics for the management of selected pediatric gastrointestinal disorders. Specific probiotic strains are recommended for the treatment of acute gastroenteritis, prevention of antibiotic-associated diarrhea, nosocomial diarrhea and necrotizing enterocolitis, management of Helicobacter pylori infection, and management of functional abdominal pain disorders and infant colic. However, further studies are needed to confirm the effects and determine the type, dose, and timing of probiotics, and the use of probiotics without documented health benefits should be discouraged.

Peculiar Ca2+ Homeostasis, ER Stress, Autophagy, and TG2 Modulation in Celiac Disease Patient-Derived Cells

Summary: In this study, the response of skin-derived fibroblasts from CD and control subjects to the ER stress inducer thapsigargin was investigated, revealing molecular features of the CD cellular phenotype. It was found that CD cells had lower cytosolic and ER Ca2+ levels compared to control cells, and showed slightly different responses in terms of ER stress activation and autophagic marker expression. Overall, this study identified further molecular features of CD cells and highlighted their reduced ability to adapt to stress conditions and respond in a physiological manner.

Clinical Presentation and Severity of SARS-CoV-2 Infection Compared to Respiratory Syncytial Virus and Other Viral Respiratory Infections in Children Less than Two Years of Age

Summary: The study found that the spread of SARS-CoV-2 and restrictive measures led to a decrease in RSV occurrence and rare bronchiolitis caused by SARS-CoV-2. The study compared the occurrence and severity of SARS-CoV-2 bronchiolitis with other respiratory viral infections in children under two years of age. The results showed a higher risk of oxygen therapy and intravenous hydration in children with RSV infection and co-infection compared to children with SARS-CoV-2 infection. Pediatricians should pay attention to bronchiolitis caused by SARS-CoV-2, which can have a severe clinical course in younger children.
