Respiratory syncytial virus persistence -: Evidence in the mouse model

Several studies have described a clear association between respiratory syncytial virus (RSV) lower respiratory tract infection in infancy and the subsequent development of persistent wheezing in children. Using the mouse model we demonstrated that RSV induces long-term airway disease characterized by chronic airway inflammation and airway hyperreactivity (AHR). The RSV murine model offers, great advantages to study the immunopathogenesis of RSV-induced long-term airway disease. Mice can be challenged with aerosolized methylcholine to determine the presence of AHR. We can apply the reverse transcription-polymerase reaction assay (RT-PCR) to detect RSV RNA in the respiratory tract and we can perform lung gene expression analysis to further characterize the chronic changes induced by RSV infection. Compared with sham-inoculated controls, RSV-infected mice developed chronic airway disease characterized by AHR and persistent airway inflammation. Forty-two days after RSV infection, a time point when RSV could no longer be isolated, RT-PCR demonstrated, quite unexpectedly, the presence of RSV RNA in the lower respiratory tract of mice. The presence of genomic RNA persisted for months after inoculation. Furthermore, preliminary studies also demonstrated that on day 42 there were a number of genes differentially expressed in RSV-infected mice compared with controls. RSV-infected mice with persistent AHR exhibited presence of abnormal chronic inflammatory changes, altered gene expression profiles, and persistence of RSV RNA, which may contribute to long-term airway disease Induced by RSV. Future Studies are needed to define the significance of persistent RSV RNA in the mouse model, and its potential role in the pathogenesis of RSV-induced persistent wheezing in children.

Drug resistance in children and adolescents with HIV in Panama

Summary: This study evaluates the impact of antiretroviral resistance mutations in an HIV-infected paediatric cohort in Panama. The results show that a significant proportion of treated patients carry resistant strains, with most of them having resistance mutations to NRTIs and NNRTIs. Routine resistance monitoring is necessary for optimal rescue therapy selection in this vulnerable population.

Summary: By using an electronic health record-based algorithm, researchers identified serology-positive children with COVID-19 between March 2020 and April 2022. Compared to PCR-positive children, serology-positive children were more likely to be hospitalized, have chronic conditions, and be diagnosed with multisystem inflammatory syndrome in children. Identifying asymptomatic or paucisymptomatic children who were not tested is crucial for understanding the burden of post-acute sequelae of SARS-CoV-2 infection in children.

Infection with SARS-CoV-2 variant Gamma (P.1) in Chile increased ICU admission risk three to five-fold

Summary: The 2021 wave of SARS-CoV-2 infection in Chile saw a significant increase in ICU admissions, particularly among individuals younger than 60 years. This wave was accompanied by a surge in Gamma (P.1) variant detections and a mass vaccination campaign. By using a queueing model that incorporates various factors, including genomic surveillance and vaccine effectiveness, researchers discovered that infection with the Gamma variant led to a 3.5-4.7-fold increase in ICU admissions for people under 60. Importantly, the vaccines used in Chile were found to effectively reduce the risk of Gamma variant-related ICU admissions.

Clinical manifestations of Rickettsia rickettsii in a familial outbreak in Panama

Summary: We report an isolated outbreak of Rickettsia rickettsii in a jungle and mountainous area of Western Panama, where seven members of a family were simultaneously infected, resulting in four deaths. Previous studies have described family outbreaks as responsible for 4-8% of the cases, and the simultaneous onset of symptoms in the affected population group is extremely unusual, but should not be disregarded by clinicians in considering Rickettsia rickettsii infection.

Summary: This study evaluated immunomodulatory treatments for multisystem inflammatory syndrome in children (MIS-C) and found that glucocorticoids or glucocorticoids plus intravenous immunoglobulin were not significantly different from intravenous immunoglobulin alone in terms of clinical efficacy. Initial treatment with glucocorticoids may be a safe alternative to immunoglobulin or combined therapy.
