Health-Related Quality of Life Compared With Cardiopulmonary Exercise Testing at the Midterm Follow-up Visit After Tetralogy of Fallot Repair: A Study of the German Competence Network for Congenital Heart Defects

This nationwide study aimed to evaluate health-related quality of life (QoL) experienced by children after tetralogy of Fallot repair and to compare self-reported physical ability with objective exercise performance. This prospective nonrandomized, government-funded multicenter study enrolled 168 patients (70 girls; ages 8-16 years) after tetralogy of Fallot repair at eight German heart centers. Health-related QoL was analyzed by the self-reported KINDL-R quality-of-life questionnaire. The patients' actual exercise capacity was evaluated by a cardiopulmonary exercise test. Health-related QoL and cardiopulmonary exercise capacity were compared with those of an age-matched German standard population. Correlation of health-related QoL with self-estimated physical rating and cardiopulmonary exercise capacity were analyzed. Health-related QoL in children and adolescents after tetralogy of Fallot repair is without limitation. Compared with the standard population, all the items evaluated by the KINDL-R questionnaire showed better or similar values, whereas objective exercise capacity compared with that of the standard population was impaired. Peak oxygen uptake correlated significantly with the physical well-being (p = 0.002) and the total score (p = 0.01) of the KINDL-R questionnaire. Health-related QoL experienced by children and adolescents after tetralogy of Fallot repair is comparable with that of the healthy standard population. However, closer inspection shows that self-estimated physical functioning is significantly overestimated compared with actual exercise capacity. Quality-of-life instruments and exercise tests, therefore, should be used in a complementary manner with children to avoid eventually fatal misinterpretation of patient-estimated physical ability.

Impacts of stage 1 palliation and pre-Glenn pulmonary artery pressure on long-term outcomes after Fontan operation

Summary: This study aimed to assess the impact of high mean pulmonary artery pressure before bidirectional cavopulmonary shunt on late adverse events in low pulmonary artery pressure patients before total cavopulmonary connection (TCPC). Results showed that high mean pulmonary artery pressure remained a significant risk factor for adverse events after TCPC, even if the pressure decreased below 10mmHg before TCPC. Various factors such as previous atrial septectomy and pulmonary artery banding were associated with different pressure levels before TCPC.

Factors influencing length of intensive care unit stay following a bidirectional cavopulmonary shunt

Summary: Reduced ventricular function was significantly associated with death during the ICU stay, prolonged ICU stay and a dominant right ventricle increased the risk of death, and an ICU stay longer than 19 days was a significant risk factor for mortality. High pulmonary artery pressure at BCPS was also a significant risk factor for prolonged ICU stay.

Preoperative risk factors influencing inter-stage mortality after the Norwood procedure

Protein-Losing Enteropathy and Plastic Bronchitis Following the Total Cavopulmonary Connections

Summary: This study evaluated the incidence, outcomes, and predictors of protein-losing enteropathy (PLE) and plastic bronchitis (PB) in a cohort of patients undergoing total cavopulmonary connection (TCPC). The results showed that 41 patients developed PLE/PB, and dominant right ventricle and prolonged pleural effusion after TCPC were independent risk factors. Surgical interventions were performed in 10 patients, and 9 of them achieved recovery from PLE.

Longitudinal analysis of systemic ventricular function and atrioventricular valve function after the Fontan procedure

Summary: This study aimed to determine the longitudinal change of systemic ventricular function and atrioventricular valve (AVV) regurgitation after total cavopulmonary connection (TCPC). In a retrospective evaluation of 4219 longitudinal echocardiographic examinations, it was found that there were no transplant-free survival difference and no difference in ventricular function between dominant right ventricle (RV) and dominant left ventricle (LV) for the first 10 years after TCPC. However, ventricular function in dominant RV was inferior to that in dominant LV thereafter, and AVV regurgitation was more frequent in dominant RV, positively associated with ventricular dysfunction.

Changes in pulmonary artery index and its relation to outcome after stage II palliation in patients with hypoplastic left heart syndrome

Summary: This study aimed to determine whether the size of the pulmonary artery (PA) affects survival and if an additional aortopulmonary shunt (APS) promotes left PA growth in patients with hypoplastic left heart syndrome who underwent bidirectional cavopulmonary shunt (BCPS). The study found that preoperative PA index did not affect mortality after BCPS, and the additional APS was unable to improve left PA size in patients with inadequate BCPS.

Summary: This study evaluates the clinical outcomes and hemodynamic variables of patients after the Bj & ouml; rk procedure, based on their pulmonary flow patterns. The results suggest that patients with pulsatile systolic pulmonary flow have better survival rates, higher cardiac index, larger right ventricle, and improved exercise capacity compared to those without pulsatile systolic pulmonary flow.

Aortopulmonary collaterals in single ventricle: incidence, associated factors and clinical significance

Summary: The incidence and associated factors of aortopulmonary collaterals (APCs) in patients with univentricular heart undergoing staged palliation were examined in this study. The presence of APCs before Glenn shunt was highly associated with hypoplastic left heart syndrome, while lower Nakata-Index and younger age at Glenn shunt were associated with the development of APCs before Fontan procedure. However, the presence of APCs or intervention for APCs did not affect intensive care unit stay or mortality after total cavopulmonary connection.
