Novel Human Parechovirus 3 Diversity, Recombination, and Clinical Impact Across 7 Years: An Australian Story

Seweryn Bialasiewicz, Meryta May, Sarah Tozer, Rebecca Day, Anne Bernard, Julian Zaugg, Kyana Gartrell, Soren Alexandersen, Anthony Chamings, Claire Y. T. Wang, Julia Clark, Keith Grimwood, Claire Heney, Luregn Schlapbach, Robert S. Ware, David Speers, Ross M. Andrews, Stephen Lambert

Summary: A study found that the recombinant strain of human parechovirus 3 (HPeV3-AR) was the dominant strain in the outbreaks of sepsis-like illnesses in infants in Australia. However, this strain was not associated with increased disease severity.

Reporting of Social Determinants of Health in Pediatric Sepsis Studies*

Kusum R. Menon, Lauren Sorce, Andrew D. Argent, Tellen D. Bennett, Enitan Carrol, Niranjan Kissoon, L. Nelson J. Sanchez-Pinto, Luregn C. Schlapbach, Daniela de Souza, R. Scott L. Watson, James J. Wynn, Jerry Zimmerman, Suchitra Ranjit

Summary: There is low reporting frequency and significant variability in categorization and definition of social determinants of health (SDOH) variables in studies on pediatric sepsis. Consistent and standardized reporting of SDOH is needed to understand the association of SDOH with illness severity and outcomes, compare populations and extrapolate study results, and identify potentially modifiable socioeconomic factors for policy makers.

Validation of a paediatric sepsis screening tool to identify children with sepsis in the emergency department: a statewide prospective cohort study in Queensland, Australia

Patricia Gilholm, Kristen Gibbons, Paula Lister, Amanda Harley, Adam Irwin, Sainath Raman, Michael Rice, Luregn J. Schlapbach

Summary: The study aimed to validate and derive a simplified paediatric sepsis screening tool. The screening tool showed good predictive performance, and a simplified version with 16 criteria achieved comparable results. Implementing sepsis screening tools may improve the timely recognition and treatment of sepsis.

Effect of Saline vs Gluconate/Acetate-Buffered Solution vs Lactate-Buffered Solution on Serum Chloride Among Children in the Pediatric Intensive Care Unit The SPLYT-P Randomized Clinical Trial

Sainath Raman, Kristen S. S. Gibbons, Adrian Mattke, Andreas Schibler, Peter Trnka, Melanie Kennedy, Renate Le Marsney, Luregn J. J. Schlapbach

Summary: This study aimed to compare the effects of balanced solutions and saline on the rise of plasma chloride in critically ill children. The results showed that the use of balanced solutions reduced the incidence of rise in plasma chloride compared with saline, and had less impact on renal function.

Methylprednisolone versus intravenous immunoglobulins in children with paediatric inflammatory multisystem syndrome temporally associated with SARS-CoV-2 (PIMS-TS): an open-label, multicentre, randomised trial

Tatjana Welzel, Andrew Atkinson, Nina Schobi, Maya C. Andre, Douggl G. N. Bailey, Geraldine Blanchard-Rohner, Michael Buettcher, Serge Grazioli, Henrik Koehler, Marie-Helene Perez, Johannes Truck, Federica Vanoni, Petra Zimmermann, Carlos Sanchez, Julia A. Bielicki, Luregn J. Schlapbach

Summary: This study aimed to evaluate the effectiveness of intravenous methylprednisolone compared with intravenous immunoglobulins in children with PIMS-TS. The results showed no significant difference in the length of hospital stay between the methylprednisolone group and the immunoglobulin group, but fewer patients in the methylprednisolone group required respiratory support.

Nitric Oxide on Extracorporeal Membrane Oxygenation in Neonates and Children (NECTAR Trial): Protocol for a Randomized Controlled Trial

Adrian C. Mattke, Kerry Johnson, Kristen Gibbons, Debbie Long, Jeremy Robertson, Prem S. Venugopal, Antje Blumenthal, Andreas Schibler, Luregn Schlapbach

Summary: This article describes a study on the feasibility and safety of mixing nitric oxide (NO) into extracorporeal membrane oxygenation (ECMO) systems. The study aims to assess the impact of NO on inflammation and coagulation system activation during ECMO support. The goal is to improve outcomes for children receiving ECMO treatment.

Machine learning to predict poor school performance in paediatric survivors of intensive care: a population-based cohort study

Patricia Gilholm, Kristen Gibbons, Sarah Brueningk, Juliane Klatt, Rhema Vaithianathan, Debbie Long, Johnny Millar, Wojtek Tomaszewski, Luregn Schlapbach

Summary: A machine learning model was developed to predict poor school outcomes in children surviving intensive care unit (ICU). By using routine ICU information, the model demonstrated high predictive ability. The results of this study are important for prioritizing follow-up and targeting rehabilitative measures.

Operationalizing Appropriate Sepsis Definitions in Children Worldwide: Considerations for the Pediatric Sepsis Definition Taskforce

Enitan D. Carrol, Suchitra Ranjit, Kusum D. Menon, Tellen D. Bennett, L. Nelson J. Sanchez-Pinto, Jerry J. C. Zimmerman, Daniela C. R. Souza, Lauren R. G. Sorce, Adrienne G. Randolph, Paul Ishimine, Claudio Flauzino de Oliveira, Rakesh Lodha, Lori Harmon, R. Scott J. Watson, Luregn J. Schlapbach, Niranjan C. Kissoon, Andrew C. Argent

Summary: Pediatric sepsis definitions are outdated and lack global applicability and validity. To address this issue, the Pediatric Sepsis Definition Taskforce aims to develop a conceptual framework and rationale for future definitions that consider critical aspects and context-specific factors. Locally adaptable solutions are needed to provide individualized care based on available resources and the probability of sepsis.

Swiss Sepsis National Action Plan: A coordinated national action plan to stop sepsis-related preventable deaths and to improve the support of people affected by sepsis in Switzerland

Luregn L. A. Schlapbach, Elisa A. Zimmermann, Sylvain Meylan, Martin M. Stocker, Peter M. M. Suter, Stephan M. Jakob

Summary: Switzerland has not yet taken action to address sepsis, a disease that causes over 10 million deaths worldwide each year. A panel of experts formulated 14 recommendations to improve awareness, prevention, and treatment of sepsis in Switzerland, including raising public awareness, improving healthcare workforce training, establishing standards for rapid detection and treatment, and promoting sepsis research. The report presents a coordinated national action plan to prevent, measure, and reduce the burden, death, and disability caused by sepsis in Switzerland.

Herberg, Myrsini Kaforou, Aubrey J. Cunnington, Oleg Blyuss, Michael Levin

Summary: This study evaluated immunomodulatory treatments for multisystem inflammatory syndrome in children (MIS-C) and found that glucocorticoids or glucocorticoids plus intravenous immunoglobulin were not significantly different from intravenous immunoglobulin alone in terms of clinical efficacy. Initial treatment with glucocorticoids may be a safe alternative to immunoglobulin or combined therapy.
