Summary: This study explored the efficacy and safety of an individualized preconditioning regimen for ABO-incompatible (ABOi) kidney transplantation. The results showed that the regimen achieved comparable graft and patient survival rates as ABO-compatible (ABOc) kidney transplantation, and effectively prevented acute rejection (AR). However, monitoring antibody titer rebound is necessary.

Summary: High preoperative and postoperative peak Ig M isoagglutinin titers were significantly associated with the risk of acute kidney injury (AKI) in ABO-incompatible liver transplantation. However, no significant differences were observed in other clinical outcomes based on isoagglutinin titers. This suggests that high isoagglutinin titers may serve as a warning sign for AKI risk after liver transplantation.

Adding bortezomib to the desensitization regimen can mitigate the risk of isoagglutinin rebound and prevent clinical AMR in patients with high isoagglutinin titers.

Summary: This study reports the first ABO-incompatible deceased donor kidney transplantation (ABOi DDKT) in an infant. The recipient was diagnosed with a Wilms tumor gene 1 (WT1) mutation and had received peritoneal dialysis before the transplant. No antibody removal treatment was performed, and the recipient had good graft function and normal development at 11 months post-transplant. ABOi DDKT without antibody removal treatment is a feasible option for kidney transplantation in small infants.

Summary: This study reviewed the experience of ABO-incompatible living donor liver transplantation (ABOi-LDLT) for acute liver failure (ALF) in a single center. The results showed that despite the delayed preoperative administration of RTx, the ALF group had a good immune response and an acceptable long-term survival rate. Therefore, ABOi-LDLT seems to be a viable treatment option for ALF.

Summary: This article reports a successful case of ABO-incompatible living-donor lobar lung transplantation in a 14-year-old girl with bronchiolitis obliterans due to graft-versus-host disease after hematopoietic stem cell transplantation. The patient received a right lower lobe from her blood type B father and a left lower lobe from her blood type O mother. Prior to transplantation, desensitization therapy with rituximab, immunosuppressants, and plasmapheresis was implemented to reduce the production of anti-B antibodies and prevent acute antibody-mediated rejection.

Summary: ABO-incompatible kidney transplantations require preemptive antibody reduction. A study on ABO-O recipients of ABO-A kidneys revealed that ABO-A trisaccharide is sufficient for IgM binding to all A-subtypes, with A-II being the major, but not the sole, antigen relevant to treatment and immune modulation. IgG binding to different A-subtypes exhibited varying degrees of specificity, with A-II showing the most reduced antibody levels posttransplantation.
