Summary: This study analyzed the benefits and risks of ex vivo liver resection and autotransplantation (ELRA) in end-stage hepatic alveolar echinococcosis (AE). The long-term overall survival of the ELRA group was superior to that of the nonsurgical group, and patients with a predicted 12-month mortality risk >75% would significantly benefit from ELRA.

Summary: This case report describes a patient with hepatic alveolar echinococcosis who developed renal invasion 2 years after liver transplantation. The study suggests that patients with hepatic alveolar echinococcosis and definite metastatic lesions should be monitored for the presence of cryptogenic lesions. Immunosuppressive drugs after liver transplantation may contribute to the development of occult lesions into active ones. In clinical practice, particular attention should be given to patients with hepatic alveolar echinococcosis who have long-term unexplained urinary tract infections, as this may be an early clinical feature of cryptogenic renal alveolar echinococcosis.

Summary: Primary sclerosing cholangitis is a significant cause of liver transplantation. Hepatic alveolar echinococcosis, caused by Echinococcus multilocularis, can be detected by ultrasound or CT scan of the liver showing characteristic calcified conglomerates. Symptoms of alveolar echinococcosis occur after a long period of infection, accompanied by cholestasis or cholangitis. This report presents a case of presumed autoimmune hepatitis and primary sclerosing cholangitis in a patient, and post-transplantation diagnosis revealed alveolar echinococcosis in the explanted liver.

Summary: This study aimed to compare the real efficacy of laparoscopic hepatectomy, radiofrequency ablation, microwave ablation, and medication in the treatment of hepatic alveolar echinococcosis, showing that laparoscopic hepatectomy has a significant effect in the treatment of early-stage hepatic AE, especially in reducing recurrence compared to other treatment methods.

The RHIVC reconstruction methods should be selected appropriately depending on the defect degree of AE lesions in IVC lumen. The RHIVC resection without any reconstruction method should be considered with caution.

Summary: We reported a case of a 26-year-old patient with hepatic AE who underwent complex inferior vena cava reconstruction during autologous liver graft using disease-free IVC, autologous portal vein fragments, and umbilical vein within the ligamentum teres hepatis. The patient had a good surgical recovery without vascular-related complications during long-term follow-up.

Summary: This study evaluated the incidence and predictors of non-transplantable recurrence in patients with single hepatocellular carcinoma (HCC) <= 5 cm treated with liver resection (LR). The results showed that HCC size >= 4 cm and high alpha-fetoprotein (AFP) level at the time of LR were independent predictors of recurrence beyond established criteria. Microvascular invasion and microsatellite lesions were identified as additional independent risk factors for non-transplantable recurrence.

Summary: This case study presents a case of a 33-year-old female patient with leiomyosarcoma at zone II-III of inferior vena cava with thrombi in three hepatic veins. The patient underwent ex vivo liver resection and autotransplantation (ELRA) and hepatic vein thrombectomy, with successful management of post-operative complications. Despite the aggressiveness of the procedure, ELRA could be considered in similar cases of advanced leiomyosarcoma with Budd-Chiari syndrome and hepatic vein thrombi.
