Summary: BK polyomavirus-associated nephropathy (BKPyVAN) is associated with an increased risk of graft dysfunction and loss in kidney transplant recipients. This cohort study in Australia and New Zealand found that BKPyVAN occurred in 3% of recipients and was linked to higher rates of graft loss, rejection, and death. Changes in immunosuppression, such as reducing tacrolimus and mycophenolate doses, were commonly made after BKPyVAN. Several factors, including gender, age, blood group, donor characteristics, transplantation era, immunosuppression use, and transplant center volume, were associated with the development of BKPyVAN. The study also found that BKPyVAN increased the risk of graft loss but not mortality. It highlights the need for interventional trials to evaluate strategies for reducing immunosuppression and minimizing adverse outcomes associated with BKPyVAN.

Summary: This study reported the clinical profile and outcomes of dengue infection in renal transplant recipients, showing distinct features such as retro-orbital pain, conjunctival redness, thrombocytopenia, and absence of arthralgia in dengue cases compared to non-dengue cases. The majority of dengue cases experienced allograft dysfunction, but there were no cases of rejection or graft losses in the 1-year follow-up. Successful management of dengue in renal transplant recipients was achieved with no observed mortality in this cohort.

The results showed that high PTH levels during the first year after transplantation were strongly correlated with graft loss. Summary: This study analyzed the association between anemia treatment and vascular access thrombosis (VAT) in dialysis patients. The results showed that proactive IV iron therapy was not associated with an increased risk of VAT. Diabetic kidney disease, AVG use, diuretic use, female sex, and previous/current smoker were independently associated with a higher risk of VAT, while the use of angiotensin receptor blockers was associated with a lower risk.

Summary: Taking care of pregnant patients with parathyroid disorders requires consideration of changes in bone and mineral metabolism. The diagnostic and therapeutic approaches differ from the nonpregnant population. Parathyroidectomy is recommended for primary hyperparathyroidism before pregnancy to reduce complications. Conservative treatment and active surveillance are recommended for hypoparathyroidism. Postpartum monitoring is necessary, as there is an increased risk of hypercalcemia.
