Metabolic changes induced by theta burst stimulation of the cerebellum in dyskinetic Parkinson's disease patients

Background: Cerebellar repetitive transcranial magnetic stimulation may be effective in reducing peak-dose levodopa induced dyskinesia in Parkinson's disease patients. It was proposed that the anti-dyskinetic effect could be due to modulation of cerebello-thalamo-cortical pathways. However the neural basis for these clinical effects have not yet been demonstrated. Methods: We investigated the effects of repeated sessions of cerebellar continuous theta burst stimulation in Parkinson's disease patients with levodopa induced dyskinesia on brain metabolism by means of positron emission tomography scan with fluorodeoxyglucose (F-18-FDG) to characterize the specific cerebral network activated by cerebellar stimulation in these patients. Results: We found that five days of bilateral cerebellar continuous theta burst stimulation (cTBS) were effective in reducing levodopa induced dyskinesia. Clinical changes were paralleled by a reduction of F-18-FDG metabolism in the cerebellum as revealed by positron emission tomography imaging. We found a global decrease in the metabolism of the bilateral cerebellar hemispheres, and a significant decrease in F-18-FDG uptake in correspondence of bilateral dentate nucleus. Conclusions: Our study demonstrates the antidyskinetic effect of cerebellar cTBS in Parkinson's disease patients with levodopa induced dyskinesia, is paralleled by modulation of the activity of the pathways connecting the cerebellar cortex with the deep cerebellar nuclei, confirming the hypothesis that the motor cerebellar circuit is involved in the generations of levodopa induced dyskinesia. Summary: Immersive virtual reality can lead to a strong feeling of embodiment over artificial body parts, but the neural mechanisms behind this phenomenon are not yet fully understood. By tracking real-time brain dynamics, researchers found that observing virtual body parts is associated with a rapid decrease in cortical activity in the hand region of the primary motor cortex, as well as rapid changes in activity within a fronto-parietal circuit. These findings suggest that changes in bodily representations are supported by dynamic interactions within a highly plastic network.

Preclinical Investigation of Radiopharmaceuticals: An Accurate and Multidisciplinary Approach

Summary: This pilot study evaluated the capability of a digital autoradiography system to perform biodistribution evaluation of [F-18] choline in a xenograft mouse model of prostate cancer. The results showed that the system could accurately assess the biodistribution of the radiopharmaceutical and demonstrate tumor-specific uptake. Immunohistochemical analysis also confirmed a strong correlation between [F-18] choline uptake and tumor proliferation index.

Summary: This study reviewed the application of radiolabeled-FAPI in patients with pancreatic cancer, discussing its advantages, disadvantages, and future prospects, especially in the field of theragnostic. A literature search was performed in PubMed, EBSCO, and EMBASE databases, and 21 studies were selected for comprehensive analysis. The results showed that FAPI could better detect pancreatic malignant lesions, but further prospective studies are needed to validate its effectiveness.

Radiomics and theranostics with molecular and metabolic probes in prostate cancer: toward a personalized approach

Summary: In the past decade, two new radionuclide-based therapies, (223)Radichloride and radioligand therapy (RLT) targeting prostate-specific membrane antigen (PSMA), have been approved for the management of castrate-resistant prostate cancer (PCa). This article illustrates the basic principles of PCa molecular imaging, with a focus on the combined use of diagnosis and therapy for response prediction and assessment. Radiomics, an emerging discipline based on quantitative features extracted from medical images, is also discussed for its potential in this field.

Estimating the sequence of biomarker changes in Parkinson's disease

Summary: Using the novel data-driven method DEBM, this study determined the sequence of several common biomarker changes in Parkinson's disease (PD). The left putamen was found to be the earliest biomarker to become abnormal, followed by the right putamen, CSF alpha-synuclein, right caudate, left caudate, and serum NfL. The estimated disease stages showed significant differences between PD and healthy controls, and achieved a high accuracy for distinguishing PD from HC.

Incidence and risk factors of institutionalisation in Parkinson's disease and atypical parkinsonism
