Background: Alteration of the immune system is one of the major mechanisms responsible for complications in severe acute pancreatitis (AP). The aim of our study was to provide a complex evaluation of peripheral blood monocyte subsets, natural killer cells (NK cells) and cytotoxic T lymphocytes in patients with different severity forms of AP. Methods: 20 patients with mild AP and 15 with severe AP (S-AP) were included in our study. Peripheral blood mononuclear cells were studied on days 1-3, 5, 10 and 30, by means of flow cytometry. Results: In peripheral blood of patients with pancreatitis, we found a marked increase in total monocyte count. In S-AP, circulating monocytes were significantly activated, which was presumed from increased expression of HLA-DR, CD54, CD69 and CD25. Concurrent increased expression of CD95 (FasR) may indicate enhanced susceptibility of these cells to apoptosis. In patients with S-AP, a dramatic depletion of circulating NK cells (CD16/56 and CD3-CD8+) was found along with a reduction of circulating CD3+ CD8+ lymphocytes (cytotoxic T lymphocytes). Conclusion: Our findings suggest profound disturbances of innate cellular immunity in patients with S-AP. Copyright (C) 2008 S. Karger AG, Basel and IAP.

Summary: The systematic review and meta-analysis showed that endoscopic techniques, including self-expandable stents, clipping, and tissue sealants, are effective in closing post-bariatric surgery leaks and fistulas. The success rates were high for stents (92%), clips (67.1% for OTSC system) and fibrin glue (92.8-100%). Complications were relatively low, indicating that these endoscopic methods are suitable for properly selected patients.

Summary: This retrospective study evaluated the effectiveness of endoscopic therapy for UGI postsurgical leaks in 206 patients between 2014 and 2019. It found that multimodal therapeutic endoscopy can achieve leak closure in a significant proportion of patients with a low rate of severe adverse events.

Summary: The study found that exposure to BPA led to reduced chemotactic capacity in neutrophils, increased phagocytic activity in female cells, decreased phagocytic activity in male cells, and elevated expression of CD14, CD284 (TLR4), and NET formation in both sexes. Additionally, BPA intensified NO production and iNOS expression in both sexes, and increased expression of PI3K-Akt pathway proteins in male neutrophils.

Summary: Acute-on-chronic liver failure (ACLF) is a severe syndrome with a high short-term mortality rate. This study found that the risk of ACLF remains high in cirrhotic patients hospitalized for acute gastrointestinal bleeding, even after successful control of bleeding. Elevated creatinine level, MELD score, and the presence of hepatic encephalopathy (HE) on admission were identified as the best predictors of ACLF in these patients during hospitalization.

Summary: Magnification endoscopy and methylene blue chromoendoscopy can effectively detect intestinal metaplasia (IM). Methylene blue staining is more accurate in indicating IM compared to pit pattern and vessel evaluation. Methylene blue chromoendoscopy can serve as a screening method for detecting advanced OLGIM stages.
