Summary: This study compared the surgical volumes and exposure areas of different surgical approaches to Meckel's cave and found that endoscopic approaches can provide adequate exposure, especially for the medial portions, while microsurgical transcranial approaches still seem to be the gold standard for optimal exposure and surgical volumes in the most lateral portion of Meckel's cave.

Summary: The study focuses on designing a miniature steerable digital endoscope for high-resolution imaging of the middle ear via a trans-nasal approach, aiming to provide less-invasive diagnosis and surveillance of middle ear disease. The endoscope features a steerable tip with a 1 Megapixel image sensor and fiber-optic illumination, enabling visualization of critical middle ear structures through the Eustachian tube. The experimental validation on cadaveric temporal bone specimens demonstrates high-quality middle ear visualization and the potential for non-surgical diagnosis of middle ear disease.

Summary: This study found that radiographic evidence of cholesteatoma in the retrotympanum is overestimated compared to intraoperative endoscopic findings, indicating limited diagnostic value of high-resolution computed tomography (HRCT) in preoperative evaluation.

Summary: This study aimed to investigate the variability of the retrotympanum in patients undergoing surgical treatment for cholesteatoma. The most frequent variant of retrotympanic pneumatization in relation to the facial nerve was type A in all subsites in cholesteatoma patients. The variability among patients with cholesteatoma is different to previously published results in healthy subjects, and the pneumatization of the retrotympanum is associated with mastoid pneumatization.

Summary: This study aimed to evaluate the surgical outcomes of cholesteatoma treated with endoscopic ear surgery (EES) or a combined endoscopic-microscopic approach (cEMA) based on various classification and staging systems. The results showed that EES or cEMA is an effective treatment for middle ear cholesteatoma. The STAM classification and the EJS have prognostic value, with certain stages associated with better outcomes. Mastoid involvement is a risk factor for recurrence, while attic localization is associated with residual disease.

Summary: This study evaluated the postoperative hearing and vestibular function after various semicircular canal surgeries to treat patients with labyrinthine fistulas caused by middle ear cholesteatoma. The results showed that different types of labyrinthine fistulas require different surgical techniques, and these surgeries can effectively improve postoperative hearing and vestibular function.

Summary: The study assessed the outcomes of transcanal endoscopic ear surgery (TEES) for congenital cholesteatoma and found that it is feasible and effective for cases not extending into the mastoid. Recidivism rates were lower with TEES approach, with advanced-stage disease being the primary risk factor for recidivism and worse hearing outcomes. Minimally invasive TEES in young children allows for early identification and intervention.

Summary: The study compares the accuracy of unstructured preoperative CT reports from non-tertiary diagnostic centers with intraoperative findings in patients with COM, suggesting discrepancies may be due to limited expertise of general radiologists in ENT neuroimaging. A structured checklist is proposed for optimizing communication with surgeons and providing crucial information for accurate surgical planning.

Summary: This study developed a fiber-based optical coherence tomography catheter for visualizing the Eustachian tube anatomy, proving its feasibility through experiments on swine cadaver heads. The OCT technique successfully obtained cross-sectional images of the tube, demonstrating its potential for assessing Eustachian tube anatomy.

Summary: Both postauricular microscopic and transcanal endoscopic approaches were found to have limited applicability when the tympanic cavity was located very close to the cochlea, with a distance less than 3 mm. They both provided a similar dissection area and presented detailed anatomy and measurements of the topography of the third portion of the facial nerve from the tympanic cavity.

Summary: This study describes a case in which simultaneous surgical treatments for vestibular schwannoma and middle ear cholesteatoma were performed on a female patient, followed by cochlear implantation. The surgical approach was successful, with preservation of the auditory nerve and cochlea, and positive postoperative results were observed with the cochlear implant in both silent and noisy environments.
