Growth factors, silver dressings and negative pressure wound therapy in the management of hard-to-heal postoperative wounds in obstetrics and gynecology: a review

The last two decades witnessed the development of numerous innovative regimens for the management of patients with abnormally healing and infected wounds. Growth factors, negative pressure wound therapy (NPWT) and antiseptic dressings containing silver are examples of methods with best documented efficacy, being widely used in the treatment of acute and chronic post-traumatic wounds, burns and ulcers of various etiology. As far as obstetrics and gynecology are concerned, prevention and treatment of infected, hard-to-heal postoperative wounds is of crucial importance. This article reviews the available literature to discuss the possibilities for use, efficacy and cost-effectiveness of growth factors, NPWT and silver dressings in the treatment of difficult-to-heal postsurgical wounds in obstetrics and gynecology. An extensive search of the English and Polish literature via PubMed and EMBASE databases was undertaken for articles published between January 1960 and April 30, 2014 to identify articles that described and assessed use, efficacy and cost-effectiveness of growth factors, silver dressings and NPWT in patients with hard-to-heal postoperative wounds following obstetric or gynecological surgery. Literature review regarding the use of growth factors, NPWT and silver dressings suggests that these methods may play an important role in the management of wounds after invasive obstetric and gynecological procedures. Obese patients, patients after vulvectomy or prior radiation therapy may benefit most, however, due to non-numerous randomized reports, prospective studies on the use of above-mentioned methods in the treatment of postsurgical wounds following obstetric and gynecological interventions are required.

Does HPV infection have impact on results of photodynamic treatment of vulvar lichen sclerosus?

Summary: The association between lichen sclerosus (LS) and human papilloma virus (HPV) infections remains unclear, but photodynamic therapy (PDT) may be a promising, effective, and safe method for the treatment of LS regardless of HPV infection. Patients with HPV-negative results had a longer mean remission period compared to those with positive HPV results.

Perinatal Outcomes in a Population of Diabetic and Obese Pregnant Women-The Results of the Polish National Survey

Summary: This study compared perinatal outcomes in diabetic and obese Polish women, finding that GDM was the most common type of glucose intolerance in both groups. There was a slight increase in induced deliveries for obese women, and a significant decrease in fetal birth weight. The risk of GDM was significantly higher in women over 35 years and those who were overweight.

Summary: Ultrasound-derived measurements of fetal soft tissue, heart, liver and umbilical cord are significantly increased in women with gestational diabetes treated with insulin and type 1 diabetes mellitus. In addition to standard biometric measurements, parameters such as abdominal fat mass may be useful in the management of diabetes-complicated pregnancies.

Summary: This study evaluated the placental expression of glucose transporters in term pregnancies complicated by GDM and PGDM. It found a significant increase in GLUT-1 expression in PGDM patients, while no significant differences were observed in other GLUT isoforms. Positive correlations were noted between fetal birthweight and the expression of GLUT-1 in PGDM group and GLUT-12 in the control group.

Maturity-onset Diabetes of the Young (MODY) in Pregnancy: A Review

Summary: Hyperglycaemia in pregnancy, commonly diagnosed as gestational diabetes mellitus (GDM), may not always match the clinical manifestations of GDM. Misdiagnosis of maturity-onset diabetes of the young (MODY) as GDM is suspected in 1-2% of cases. Proper diagnosis is crucial as misdiagnosis of MODY increases complications for both mother and fetus. This literature review provides current knowledge on the diagnosis, treatment, and complications of the most common types of MODY in pregnancy.

Is There an Association between the Use of Epidural Analgesia during Labor and the Development of Autism Spectrum Disorder in the Offspring?-A Review of the Literature

Summary: Autism spectrum disorders (ASDs) are complex neurodevelopmental conditions diagnosed in early childhood. The etiology of ASDs is said to be a combination of genetic predisposition and environmental impact. Epidural analgesia (ELA) is a commonly used method of pain relief during labor, but a study in 2020 suggested a potential link between its use and the risk of ASD development in newborns. However, a review of the literature indicates that there is no significant association between the use of ELA during labor and the risk of ASD.

Influence of COVID-19 infection on placental function

Summary: This meta-analysis study aimed to examine the impact of COVID-19 on the placenta. The results showed that SARS-CoV-2 infection led to pregnancy complications in pregnant women, including caesarean delivery, preterm delivery, fetal distress, premature rupture of the membranes, and stillbirth. The study also found that placental damage was associated with SARS-CoV-2 infection. Further research is needed to better understand how SARS-CoV-2 infection affects the placenta.

Summary: This study evaluated the levels of circulating 20S proteasome and 20S immunoproteasome in plasma and peritoneal fluid of women with and without endometriosis. The results indicate that these markers are not useful in diagnosing the disease but suggest a potential role in its pathogenesis.

Flash glucose monitoring in gestational diabetes mellitus (FLAMINGO): a randomised controlled trial

Summary: This study aimed to assess the efficacy of flash glucose monitoring (FGM) in the management of gestational diabetes mellitus (GDM). The results showed that the FGM group significantly reduced fasting and postprandial glycaemia during the first 4 weeks following GDM diagnosis, and the incidence of fetal macrosomia was significantly lower in the FGM group compared to the control group.
