Summary: The study suggests that prescribing opioids at surgical discharge does not reduce pain intensity but increases the risk of adverse events, particularly vomiting. These findings are limited to patients undergoing minor and moderate surgeries, highlighting the need for more research on patients undergoing major procedures.

Summary: This study systematically reviewed 45 randomized controlled trials involving 7364 patients and found no significant differences in mortality and complications between robotic and laparoscopic surgery. Robotic surgery was associated with longer operative times and higher overall costs, while laparoscopic surgery was more advantageous in terms of operative time and cost. Reliable quality assessment and studies of long-term outcomes are still needed.

Summary: There is currently no clear definition of High Energy Devices (HEDs), and their proper indications for use are unclear. However, the flourishing market of HEDs increases the difficulty of choosing the right device in clinical practice, potentially leading to improper use and impacting the healthcare system economically. This study aims to assess the efficacy and safety of HEDs compared to electrocautery devices during laparoscopic cholecystectomy (LC).

Summary: Non-surgical therapies, such as rubber band ligation and coagulation, have lower postoperative pain and complication rates compared to surgical therapies. In this study, the clinical outcomes of rubber band ligation and coagulation for haemorrhoids were compared. The systematic review identified nine trials that met the inclusion criteria, showing no significant difference between the two therapies in terms of efficacy rate, recurrence rate, and postoperative urine retention rate. However, patients undergoing rubber band ligation had higher postoperative pain rates and lower postoperative bleeding rates than those undergoing coagulation.

Summary: This study aimed to investigate the effectiveness and safety of surgery compared with non-surgical treatment for sciatica. The results suggest that surgery, such as discectomy, is more effective in reducing leg pain and disability compared to non-surgical treatment or epidural steroid injections, but the benefits may decline over time. Therefore, surgery may be considered as an option for patients with sciatica who prioritize rapid relief over the associated risks and costs.

Summary: The COVID-19 pandemic has led to a shift towards telepsychiatry as the primary means of treatment in some countries. This comprehensive meta-analysis compared the efficacy of telepsychiatry and face-to-face treatment for psychiatric disorders and found that telepsychiatry was equally effective in improving symptoms compared to face-to-face treatment overall, but there were some variations in efficacy depending on the specific psychiatric disorder.

Summary: This systematic review and meta-analysis aimed to evaluate the effectiveness of probiotics/synbiotics in reducing infectious complications after colorectal surgery and explore their potential as perioperative measures. The results showed that probiotics/synbiotics significantly reduced infectious complications, particularly in pulmonary and urinary infections. However, further research is needed to address practical questions regarding the formulation and duration of probiotics/synbiotics before their inclusion in enhanced recovery programs.

Summary: This study conducted a systematic review and meta-analysis to evaluate the effectiveness of antimicrobial prophylaxis in preventing surgical site infections. The results suggest that more liberal use of antimicrobial prophylaxis is associated with a small reduction in the risk of surgical site infection, although the harms of antimicrobial treatment are poorly reported. Further research is urgently needed to assess the risks of antimicrobial prophylaxis and inform current practices.

Summary: In elderly patients with stage 1 non-small cell lung carcinoma, lobectomy is superior to sublobar resection in terms of survival and cancer recurrence. While there is no difference in 5-year overall survival rates between sublobar resection and lobectomy for stage 1A tumors, lobectomy is favored for stage 1B tumors. Further randomized controlled trials are needed in this area.

Summary: Pulmonary nodules are commonly encountered by respiratory physicians, radiologists, and thoracic surgeons. A collaborative effort by the European Association of Cardio-Thoracic Surgery and the European Society of Thoracic Surgeons aims to provide a comprehensive review of the management of pure ground-glass opacities and part-solid nodules. The literature suggests an increasing detection of early-stage lung cancer, emphasizing the importance of surgical resection and the need for guidelines. Multidisciplinary decision-making is recommended, considering radiologic characteristics, lesion evolution, patient fitness, and co-morbidities.

Summary: This study conducted an online survey to explore the e-learning experience within the medical community in the MENA region. The results showed that 78 vascular surgeons and trainees from 16 countries participated, with 88% being male. More than half of the activities did not lead to official certification, and the primary determinant for attending an activity was the topic. National societies and social media played a major role in disseminating activity-related information. Lack of time, increased workload, differences in time zone, and technical issues were the main obstacles cited. Overall, 84.7% of the participants had a positive impression towards e-learning. Novel ways of quality control are required as a shift was observed in the learning mode and new leadership skills were called upon in vascular surgery due to the COVID-19 pandemic.
