Outcome of surgery versus radiotherapy after induction treatment in patients with N2 disease: systematic review and meta-analysis of randomised trials

Objective Chemoradiotherapy is often considered the 'standard of care' for patients with N2 disease. The aim was to evaluate survival outcomes of patients with N2 disease in multimodality trials of chemotherapy, radiotherapy and surgery. Methods Systematic review and meta-analyses (random and fixed effects) were performed. Searches of Medline and Embase (1980-2013) were conducted. Abstracts from thoracic scientific meetings were searched. Reference lists of all relevant studies were reviewed. All studies of patients with N2 disease who received induction chemotherapy or chemoradiotherapy and randomised to surgery or radiotherapy were included. No language restrictions were imposed. The main outcome was overall survival. Results 805 publications were identified. 519 and 281 were excluded because they were not primary results from randomised trials (or did not include N2 disease) or did not compare surgery with radiotherapy, respectively. The final six trials consisted of 868 patients. In four trials, patients received induction chemotherapy and in two trials patients received induction chemoradiotherapy. The HR comparing patients randomised to surgery after chemotherapy was 1.01 (95% CI 0.82 to 1.23; p=0.954) whereas for patients randomised to surgery after chemoradiotherapy was 0.87 (0.75 to 1.01; p=0.068). The overall HR of all pooled trials was 0.92 (0.81 to 1.03; p=0.157). Conclusions Surgery should be considered as part of multimodality treatment for patients with resectable lung cancer and ipsilateral mediastinal nodal disease. In trials where patients received surgery as part of trimodality treatment, overall survival was better than chemoradiotherapy alone.

Lobar versus Sublobar Resection in the Elderly for Early Lung Cancer: A Meta-Analysis

Summary: In elderly patients with stage 1 non-small cell lung carcinoma, lobectomy is superior to sublobar resection in terms of survival and cancer recurrence. While there is no difference in 5-year overall survival rates between sublobar resection and lobectomy for stage 1A tumors, lobectomy is favored for stage 1B tumors. Further randomized controlled trials are needed in this area.

National Rates of Lower Extremity Amputation in People With and Without Diabetes in a Multi-Ethnic Asian Population: a Ten Year Study in Singapore

Summary: This study examined national amputation rates in Singapore from 2008 to 2017 in people with and without diabetes. The results showed that amputation rates remained stable but relatively high in people with diabetes, while decreasing in people without diabetes. Diabetes-related amputation rates were highest in Malays and lowest in Chinese. The proportion of amputations attributable to diabetes increased over time.

Clinical characteristics and outcomes of exertional rhabdomyolysis after indoor spinning: a systematic review

Summary: This systematic review characterizes the clinical features of spin-induced exertional rhabdomyolysis (SIER). The study found that most patients with SIER were healthy females attending their first spinning session. Common symptoms included myalgia, dark urine, and muscle weakness in the thighs. A small percentage of patients developed acute kidney injury or thigh compartment syndrome, but no long-term sequelae or mortality were observed.

Percutaneous decannulation of venoarterial extracorporeal membrane oxygenation using the Manta vascular closure device: A systematic review and meta-analysis

Summary: This study aimed to systematically review and meta-analyze the use of the Manta vascular closure device for percutaneous decannulation of venoarterial extracorporeal membrane oxygenation (VA-ECMO). The technical success rate of percutaneous decannulation with the Manta device was 93.7%, with low incidence of complications such as bleeding, vascular complications, and wound complications. The use of the Manta device for VA-ECMO decannulation was found to be effective and safe.

Summary: This study analyzed 1,644 tumor regions and found various gene mutations and evolutionary processes in lung adenocarcinoma. It was discovered that clonal expansion and whole genome doubling play important roles in the development and relapse of non-small cell lung cancer. The findings also revealed similarities in etiology and pathogenesis between lung adenocarcinoma in smokers and never-smokers.

Outcomes of chest drain management using only air leak (without fluid) criteria for removal after general thoracic surgery-a drainology study

Summary: This study aimed to evaluate the safety of using air leak criteria alone for chest drain removal in thoracic surgery. The results showed that using digital assessment of air leak as the sole criteria for drain removal maintained acceptable levels of safety in selected patients undergoing general thoracic surgery.

Summary: This study conducted an online survey to explore the e-learning experience within the medical community in the MENA region. The results showed that 78 vascular surgeons and trainees from 16 countries participated, with 88% being male. More than half of the activities did not lead to official certification, and the primary determinant for attending an activity was the topic. National societies and social media played a major role in disseminating activity-related information. Lack of time, increased workload, differences in time zone, and technical issues were the main obstacles cited. Overall, 84.7% of the participants had a positive impression towards e-learning. Novel ways of quality control are required as a shift was observed in the learning mode and new leadership skills were called upon in vascular surgery due to the COVID-19 pandemic.
