Local control of nasopharyngeal carcinoma has substantially improved with advancing radiotherapy technology and appropriate combination with chemotherapy. However, when local recurrence occurs, this is one of the most difficult challenges. Aggressive treatment is indicated because long term salvage is achievable particularly for early recurrence, but high risk of complications is a serious concern. Treatment options include different methods of surgery and/or re-irradiation with/without chemotherapy. Available information in the literature is grossly inadequate; most reports compose of small series of highly selected patients with heterogeneous characteristics and treatment. No randomized trials have been performed to evaluate the therapeutic ratio of different treatment methods. This article reviews available treatment options, their therapeutic benefits and risks of complications; the objective is to provide information for developing treatment recommendations and suggestions for future improvement. Summary: Proton therapy is a promising radiation therapy method for improving outcomes in head and neck cancer patients. It can minimize radiation dose to surrounding vital structures, reducing toxicity. However, current evidence on its effectiveness is limited, leading to scarce reimbursement indications in most countries. Despite this, there are approximately 100 proton therapy centers worldwide, and initial experiences for head and neck cancer are being reported. This review aims to summarize the early clinical experience and challenges of proton therapy for head and neck cancer.

Prognostic Biomarkers for Survival in Nasopharyngeal Carcinoma: A Systematic Review of the Literature

Summary: This study investigates molecular biomarkers for prognostic prediction in patients with nasopharyngeal carcinoma (NPC). Seventeen important markers are identified, including genetic polymorphisms, methylation, microRNAs, mutation, and gene expression panels. These biomarkers can be used to predict patient survival and guide personalized therapy. The study also highlights the clinical need for a comprehensive evaluation of multiple types of biomarkers in a multicenter study, and these biomarkers are involved in various signaling pathways that could be potential therapeutic targets.

Summary: The projected increase in life expectancy is expected to lead to a rise in age-related diseases, including cancer. This report critically reviews the literature on managing older patients with head and neck cancer (HNC). Older adults, especially those who are frail or have comorbidities, are more prone to complications and toxicities from HNC treatment. Screening older patients for predispositions and managing comorbidities is essential. Age alone should not be a determining factor for less intensive treatment, and older patients should be treated according to the best standard of care to avoid increased treatment failure rates and mortality. A multidisciplinary tumor board should establish the treatment plan, involving shared decision-making with patients and family, and treatment modifications should be considered for patients with severe comorbidities, frailty, or who refuse recommendations.

Summary: This study compared the efficacy and safety of hyperfractionation with standard fractionation in intensity-modulated radiotherapy. Summary: This study aimed to investigate the optimal time for early therapeutic response prediction with multi-parametric magnetic resonance imaging (MRI) in patients with nasopharyngeal carcinoma (NPC) receiving concurrent chemo-radiotherapy (CCRT). The results showed that DW-MRI on week-1 and DCE-MRI on week-2 can serve as the optimal time points for early therapeutic response prediction.

Comprehensive clinical, genome and transcriptomic analysis of primary ghost cell odontogenic carcinoma

Summary: This study presents a comprehensive clinical, genomic, and transcriptomic analysis of primary ghost cell odontogenic carcinoma (GCOC). The results provide the first comprehensive molecular atlas for primary GCOC, including previously identified CTNNB1 mutation and novel alterations in MAP3K, EP300, and the 22q11.21 region. The transcriptome analysis reveals significant involvement of cytokine-cytokine receptor interaction and the PI3K-Akt signaling pathway. More GCOC cases should be compared to validate these findings for accurate clinical guidance.

Summary: This study explores the potential of galectin-3 binding protein (LGALS3BP) as a novel target for antibody-drug conjugate (ADC) therapy in adenoid cystic carcinoma (ACC). The results show that LGALS3BP is highly expressed in ACC tissues and treatment with anti-LGALS3BP ADC leads to durable tumor growth inhibition in animal models.

Transoral robotic surgery in oropharyngeal squamous cell carcinoma: A comparative study between da Vinci Single-Port and da Vinci Xi systems

Summary: This study compares the outcomes of transoral resections using the da Vinci SP and da Vinci Xi systems for oropharyngeal squamous cell carcinoma (OPSCC). The results indicate that the da Vinci SP system has advantages in terms of reduced docking and console times, as well as improved rates of wide negative resection margins. However, there were no significant differences in complication rates or postoperative functional outcomes between the two systems.
