Summary: The reoperation rate is higher after microendoscopic discectomy for lumbar disc herniation compared to traditional microdiscectomy or open discectomy at a median follow-up of 4 years. Surgeons should therefore choose microdiscectomy or open discectomy instead of microendoscopic discectomy.

Predictors of the need for rib resection in minimally invasive retroperitoneal approach for oblique lateral interbody fusion at upper lumbar spine (L1-2 and L2-3)

Summary: This study aimed to identify factors predicting the need for rib resection in upper lumbar interbody fusion surgery. It was found that 79% of patients who underwent a two-level procedure required rib resection, while 81.6% of patients who had a single-level procedure did not. Multivariate logistic regression analysis identified the following independent predictors for rib resection: L1-2 procedure, T10-L2 kyphosis > 15.9°, and apex of the coronal curve above L2. Therefore, rib resection should be expected in upper lumbar interbody fusion surgery.

Summary: This study identifies the characteristics and outcomes of surgically confirmed acute non-traumatic spinal subarachnoid hematomas. Most patients were middle-aged and complained of back pain, with the hematoma located in the thoracic spine. After surgery, 70% of the patients showed improvement in neurological symptoms.

Summary: This study evaluated the effectiveness of adding methotrexate (MTX) or iguratimod (IGU) in patients with rheumatoid arthritis who had an inadequate response to Janus kinase inhibitors (JAKis). The results showed that adding MTX or IGU significantly improved clinical disease activity scores and disease activity index, suggesting that it can be considered as a complementary treatment.

The results suggest that vertebral fracture is associated with SLE-associated organ damage, and serum bone-specific alkaline phosphatase level may be a potentially useful marker for osteoporosis monitoring in SLE patients.

Baseline erythrocyte sedimentation rate level predicts long-term inhibition of radiographic progression by tocilizumab: the KURAMA cohort

Summary: The long-term effects of tocilizumab on joint destruction in rheumatoid arthritis patients were evaluated in this study. The results showed that patients who achieved structural remission were relatively younger, had a higher proportion of anti-citrullinated protein antibody positivity, and lower levels of C-reactive protein and erythrocyte sedimentation rate. Multivariate logistic regression analysis demonstrated that the baseline erythrocyte sedimentation rate level was significantly associated with structural remission.

CT-based analysis of sagittal and coronal lumbar endplate morphology: multivariate analysis of factors associated with endplate depth

Summary: This study aimed to analyze the morphology of the lumbar endplate and factors associated with endplate depth using computed tomography (CT). The results showed that endplate depth varies with age, sex, body mass index (BMI), and pelvic incidence (PI). Surgeons should carefully evaluate the preoperative CT image, especially in older, female, low BMI, and high PI patients, and choose the appropriate interbody fusion device.

Summary: Giant cell tumors of bone (GCTBs) are locally aggressive tumors with the histological features of giant cells and stromal cells. Denosumab is a human monoclonal antibody that binds to the cytokine receptor activator of nuclear factor-kappa B ligand (RANKL). RANKL inhibition blocks tumor-induced osteoclastogenesis, and survival, and is used to treat unresectable GCTBs.

Summary: This retrospective study in Japan aimed to assess the retention of biological disease-modifying antirheumatic drugs and Janus kinase inhibitors in a real-world cohort of patients with rheumatoid arthritis, as well as identify factors affecting their retention. The study found that biological disease-modifying antirheumatic drugs and Janus kinase inhibitors had higher discontinuation rates due to ineffectiveness, while anti-interleukin-6 receptor antibodies had lower discontinuation rates. The use of oral glucocorticoids was associated with higher rates of discontinuation due to ineffectiveness and toxic adverse events.

Summary: This study investigated the role of anti-citrullinated osteopontin (anti-cit-OPN) antibodies in the pathogenesis of rheumatoid arthritis (RA). The results showed that approximately 44% of RA patients had anti-cit-OPN antibodies, which aggravated RA and decreased the survival rate of TNF inhibitors.

A ROM-Less DDS with Single Current-Switch Array Using Self-Adjusting Two-Step Integrator

Summary: This paper presents a new low-power ROM-less DDS with only a single current-switch array. The phase interpolator reduces the jitter in the accumulator overflow signal, achieving high frequency purity for the DDS. The self-adjusting two-step integrator generates ramp waves of appropriate amplitude automatically, eliminating the need to change the threshold voltage based on the clock frequency. This DDS reduces the required dual current-switch array in conventional phase interpolation DDSs, making it suitable for low-power consumption. The frequency purity of this DDS is tolerant to PVT variations.
