Immunohistochemical analysis of claudin expression in pancreatic cystic tumors

Aberrant expression of the claudin family of proteins has been reported in many human cancers, including pancreatic ductal carcinoma. Intraductal papillary mucinous neoplasms of the pancreas (IPMN) and mucinous cystic neoplasms (MCN) are considered precancerous lesions that are able to progress towards pancreatic ductal adenocarcinoma. We analyzed the expression of several claudin family members using surgical IPMN and MCN specimens to clarify the relationships between claudin expression and clinicopathological features. Twenty-nine and 25 consecutive cases of IPMN and MCN were selected and the expression of claudin-2, -4 and -18 was analyzed by immunohistochemistry. In addition, IPMN and MCN histological grade as well as IPMN subtypes were analyzed in relation to claudin expression. The 29 cases of IPMN comprised of 3 (10.3%) adenomas, 18 (62.1%) borderline malignancies and 8 (27.6%) carcinomas. The 25 cases of MCN comprised of 13 (52%) adenomas, 5 (20%) borderline malignancies and 7 (28%) carcinomas. Claudin-2, -4 and -18 showed strong expression both in IPMN and MCN, with the exception of claudin-4 in MCN. The expression grades of claudin-2 in both IPMN and MCN became weaker with increased histological grade. On the other hand, the expression grades of claudin-4 and -18 became stronger with increased histological grade in both IPMN and MCN. With regard to histological subtype, claudin-4 expression was the strongest in pancreatobiliary type IPMN, and claudin-18 expression was the strongest in gastric type IPMN. The distinct expression patterns of claudin-2, -4 and -18 suggest that claudins may serve as useful molecular markers for tumor differentiation and progression in IPMN and MCN.

Risk Stratification for Sarcopenic Obesity in Subjects With Nonalcoholic Fatty Liver Disease

Summary: This study aimed to determine the relationship between sarcopenic obesity (SO) and the risk of significant liver fibrosis or cardiovascular disease (CVD) in nonalcoholic fatty liver disease (NAFLD). Through a multicenter retrospective study, it was found that high-risk subjects with SO had a higher risk of liver fibrosis and CVD development. The results demonstrated that high-risk subjects with SO had significantly increased risks of liver fibrosis and CVD development.

Sorafenib versus nivolumab after lenvatinib treatment failure in patients with advanced hepatocellular carcinoma

Summary: This study investigated the factors related to spleen length and the diagnostic accuracy of a model using spleen length corrected by related factors for predicting varices needing treatment. The study found that spleen length is influenced by age, sex, and body mass index in the Asian population. The LSRPS model using the measured/estimated spleen ratio showed higher diagnostic accuracy than the LSPS model in predicting VNT in patients with compensated advanced chronic liver disease.

Summary: This study aimed to investigate the differences in incidence of pancreatic cancer between new-onset diabetes mellitus (NODM) and long-standing DM (LSDM). The results showed that the risk of pancreatic cancer was higher in the group with diabetes compared to the non-diabetes group, and the risk was higher in the NODM group than in the LSDM group, especially in men and patients in their 50s.

Summary: The risks of sarcopenia and cardiovascular disease (CVD) are significantly higher in metabolic dysfunction-associated fatty liver disease (MAFLD) group compared to non-metabolic risk (MR) non-alcoholic fatty liver disease (NAFLD) group. However, the risk does not differ according to fibrotic burden in non-MR NAFLD group. The MAFLD criteria might be better for identifying high-risk fatty liver disease than the NAFLD criteria.

Copula graphic estimation of the survival function with dependent censoring and its application to analysis of pancreatic cancer clinical trial

Summary: This article discusses a survival function estimation method for clinical trials of poor prognosis cancer treatments, specifically pancreatic cancer. The copula-graphic estimation is used to handle dependent censoring, and a novel method for choosing the copula parameter is proposed. The method is applied to a clinical trial of GV1001 as a treatment for pancreatic cancer, and the results show statistically significant differences in median survival times between the treatment and control groups.

Impact of cholecystectomy on acute coronary syndrome according to metabolic condition: a nationwide population-based cohort study

Summary: Gallbladder stones (GS) are associated with an increased risk of cardiovascular disease, but the relationship between cholecystectomy for GS and acute coronary syndrome (ACS) is unclear. In this study, the ACS risk in patients with GS and its association with cholecystectomy were investigated. The risk of ACS was higher in the GS group compared to the control group, and individuals without cholecystectomy in the GS group had a higher risk of ACS development. Cholecystectomy was associated with increased ACS risk in the gallstone group, especially in patients without metabolic disorders.
