Purpose: To identify factors associated with sustained response to interleukin (IL)-1 inhibition among demographic, clinical and therapeutic data in patients with Behcet disease (BD). Methods: BD patients treated with anakinra or canakinumab were enrolled. Patients were divided into two groups according to the clinical response: group 1 included subjects showing a treatment duration of at least 52 weeks and no secondary inefficacy during the first follow-up year; the remaining patients were included in the group 2. Demographic, clinical and therapeutic data were analyzed to identify significant differences between groups. Results: Eighteen patients were included in group 1 and 18 patients in group 2. A better response to IL-1 inhibitors was significantly more common among patients with BD-related uveitis (p = 0.006) and patients with a longer disease duration (p = 0.03). Conclusion: IL-1 blockade is effective in BD, especially in the subset of patients presenting eye involvement and in those with long-lasting disease.

Pediatric Scleritis: An Update

Maria Tarsia, Carla Gaggiano, Elisa Gessaroli, Salvatore Grosso, Gian Marco Tosi, Bruno Frediani, Luca Cantarini, Claudia Fabiani

Summary: Posterior idiopathic scleritis is the most common type of scleritis observed in childhood, but anterior and necrotizing inflammatory scleritis can also occur. While less common than in adults, systemic disorders can be associated with scleral inflammation in children and should be promptly recognized and treated to prevent complications. A multidisciplinary diagnostic work-up is necessary to rule out infectious and autoimmune causes and tailor treatment to the specific patient.

Vascular Behcet syndrome: from pathogenesis to treatment

Alessandra Bettiol, Fatma Alibaz-Oner, Haner Direskeneli, Gulen Hatemi, David Saadoun, Emire Seyahi, Domenico Prisco, Giacomo Emmi

Summary: Behçet syndrome is a rare chronic inflammatory disease with unknown causes, commonly characterized by mucocutaneous and ocular symptoms. Vascular involvement, including both venous and arterial thrombosis, is a unique feature of this syndrome. The pathogenesis involves impaired immune-inflammatory response and neutrophil-mediated mechanisms leading to thrombogenesis. Treatment primarily focuses on immunosuppressants rather than anticoagulation.

Summary: The aim of this multicenter retrospective study was to investigate the effectiveness and safety of JAK-inhibitors (JAKi) in patients with rheumatoid arthritis (RA) and interstitial lung disease (ILD). The study suggests that JAKi therapy might be a safe therapeutic option for patients with RA-ILD in a short-term follow-up.

Prevalence and clinical associations of ultrasound-confirmed enthesitis in systemic lupus erythematosus

Summary: A retrospective analysis found that enthesitis, defined as inflammation at the sites of tendon and ligament attachments to bone, is fairly common in patients with systemic lupus erythematosus (SLE). Patients with enthesitis had a distinct disease phenotype with less renal involvement, more arthritis, and a low response to anti-CD20 therapy.

Unmet needs in ANCA-associated vasculitis: Physicians' and patients' perspectives

Luca Quartuccio, Elena Treppo, Livio Urso, Giulia Del Frate, Federica Mescia, Federico Alberici, Augusto Vaglio, Giacomo Emmi

Summary: In recent years, there has been significant progress in clinical research and treatments for ANCA-associated vasculitis. However, there are still unanswered questions and unmet needs regarding AAV patients, such as predicting relapse and improving quality of life. Identifying predictors of relapse can optimize therapies and reduce side effects, while recognizing the long-term consequences of the disease and its treatment is crucial. This review aims to highlight achievements, challenges, and the perspectives of physicians and patients.

Summary: Rheumatoid Arthritis (RA) is a complex disease with various clinical phenotypes, categorized by disease duration, seropositivity for rheumatoid factor (RF) and/or anti-citrullinated protein antibodies (ACPA), joint subtype, clinical behavior, and other subgroups. This review provides a summary of the multifaceted aspects of RA, specifically focusing on the association between autoimmune status and clinical outcomes, achievement of remission, and treatment response, as discussed in the 2022 International GISEA/OEG Symposium.

Summary: This study compared the expression of T and B cell subsets in sarcoidosis and B-mediated rheumatic diseases. They found significant differences in 15 cell subsets between sarcoidosis, rheumatic disorders, and controls. Principal components analysis and decision tree analysis were used to distinguish different diseases. This study sheds light on the immune cell profile and provides potential for biological therapies in treating sarcoidosis.

Summary: This study assessed the diagnostic accuracy of anti-cN1A antibodies in patients with suspected IIM and evaluated the agreement between line immunoassay (LIA) and enzyme-linked immunoassay (ELISA). The results showed excellent concordance between LIA and ELISA for detecting anti-cN1A antibodies, suggesting that LIA could be a rapid and useful adjunct, and it could even replace ELISA for cN1A assay.

It found that non-specific interstitial pneumonia (NSIP) was the most common ILD pattern, observed in c-ANCA positive patients. The study also highlights the early complication of ILD in AAV and emphasizes the importance of early diagnosis by both pulmonologists and rheumatologists.

Color Doppler Eye Ultrasonography in giant cell arteritis: differential diagnosis between arteritic and non-arteritic sudden blindness

Edoardo Conticini, Paolo Falsetti, Claudia Fabiani, Caterina Baldi, Silvia Grazzini, Gian Marco Tosi, Luca Cantarini, Bruno Frediani

Summary: The aim of this study was to assess the role of Color Doppler Eye Ultrasound (CDEUS) in the differential diagnosis between arteritic and non-arteritic blindness. The results showed that CDEUS displayed good reliability in distinguishing between arteritic and non-arteritic central retinal artery occlusion (CRAO), but no significant difference was found in arteritic and non-arteritic anterior ischemic optic neuropathy (AION). Therefore, CDEUS is not a reliable procedure in the diagnostic work-up of giant cell arteritis (GCA) and should be restricted to the exclusion of thrombo-embolic occlusions within the territory of central retinal artery.
