Summary: Recurrent pregnancy loss is a complex health challenge with no universally accepted definition. The true incidence is difficult to estimate due to inconsistency in definitions and criteria applied by international guidelines. The exact etiology remains questionable and up to 75% of cases are unexplained. This review aimed to summarize accumulated knowledge on etiology, risk factors, diagnostic options, and management approach to recurrent pregnancy loss.

Summary: Recurrent pregnancy loss (RPL) is defined as two to three spontaneous pregnancy terminations before 12 weeks of gestation, potentially caused by factors like gamete quality, uterine issues, endometrial receptivity alterations, immunologic factors, and thrombophilia.

Summary: This study showed that criteria aPL were absent in the majority of patients with recurrent early miscarriage, fetal loss, and extra-criteria recurrent pregnancy loss. However, aPS/PT antibodies were positive in a significant portion of these patients. The presence of aPS/PT antibodies posed a higher risk for recurrent pregnancy loss, but successful live births were achieved in patients who received appropriate treatment.

Summary: Unexplained recurrent spontaneous abortion (URSA) is a challenging issue in obstetrics that may be related to prethrombotic state and placental vascular thrombosis, leading to embryonic dysplasia or abortion. Anticoagulant therapy has been of interest in the obstetrics community, but research findings are inconsistent.

Summary: The study showed that there was no significant difference in pregnancy outcomes between fondaparinux and LMWH in the treatment of recurrent spontaneous abortion caused by PTS. However, fondaparinux had a lower occurrence rate of adverse reactions and higher safety compared to LMWH.

Summary: Antibody typing and treatment can improve live birth rate in Chinese patients with APS-related RSA. The combination of anticoagulant and anti-inflammatory treatment leads to a higher live birth rate in the treatment of APS-related RSA.

Summary: Both congenital and acquired uterine anomalies are associated with recurrent pregnancy loss. Initial evaluation of uterine anatomy is crucial for women with RPL. Surgical management of certain uterine anomalies, such as uterine septa, endometrial polyps, intrauterine adhesions, and submucosal myomas, may improve subsequent pregnancy outcomes in specific circumstances.

Summary: The relationship between polycystic ovarian syndrome (PCOS) and recurrent pregnancy loss (RPL) is still poorly understood and requires further research. Factors that should be further investigated include the contribution of PCOS phenotypes, BMI, obesity, insulin resistance, hyperandrogenemia, SHBG, hs-CRP, CTRP6, adiponectin, plasma leptin, homocysteine, AMH, and thrombophilia to the risk of miscarriage. Other factors that need to be explored in relation to the risk of miscarriage in PCOS patients with RPL include sOB-R, PAI-Fx, and Factor-V-Leiden mutations.

Summary: The treatment of RSA is challenging due to its heterogeneous etiology and lack of specific clinical manifestations. Available treatments target hypothetical risk factors, but their effectiveness is controversial.

Summary: The study found that environmental or lifestyle factors more prevalent in late summer may be associated with an increased risk of early spontaneous abortion. This conclusion was based on the analysis of data from the Pregnancy Study Online, a preconception cohort study of pregnancy planners from the United States and Canada, who reported the month and gestational week of their pregnancies.

Summary: The study found that there was no significant association between a history of asthma and the incidence of spontaneous abortion (SAB), but severe asthma (based on medication use) was associated with a higher risk of SAB.

Summary: The study found that each additional week of alcohol exposure during the first trimester increases the risk of spontaneous abortion, even at low levels of consumption and when excluding binge drinking. Alcohol use during weeks 5 through 10 from last menstrual period was associated with increased spontaneous abortion risk, with risk peaking for use in week 9.

Summary: This study reveals that metastasis-associated lung adenocarcinoma transcript 1 (MALAT1) regulates methyl-CpG-binding domain protein 4 (MBD4) expression in antiphospholipid antibody-positive recurrent spontaneous abortion (RSA) by recruiting CREB binding protein (CREBBP), leading to a decrease in the occurrence of RSA in mice.
