Summary: This study evaluated the pregnancy outcomes in women with congenital heart disease (CHD) with and without pulmonary hypertension (PH). The results showed that women with CHD-associated mild PH had better pregnancy outcomes compared to those with moderate-to-severe PH, with similar outcomes to women with CHD and no PH.

Summary: This study evaluated maternal and neonatal outcomes of pregnancies among women with congenital heart disease (CHD) in the modern era, finding that pregnancy in women with CHD typically has a successful outcome in both mother and child. However, as maternal CHD severity increases, the risk of numerous morbidities and neonatal mortality also increases. These findings may be helpful in counseling women with CHD, especially those with severe lesions, who plan to become pregnant.

Summary: The incidence of cirrhosis in pregnancies from outside the United States is rising. This study found that pregnancies with cirrhosis more than doubled over the past decade in the United States. Cirrhosis in pregnancies was associated with an increased risk of adverse events, although maternal and perinatal mortality was low.

Summary: The American Academy of Pediatrics National Registry for the Surveillance and Epidemiology of Perinatal coronavirus disease 2019 (COVID-19) (NPC-19) was developed to provide information on the effects of perinatal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. The study found that early in the pandemic, newborns acquired SARS-CoV-2 infection at variable rates and without apparent short-term effects. Additionally, higher than expected numbers of preterm births and maternal in-hospital deaths were observed.

Summary: Emerging evidence suggests that disturbances in early-life gut microbiota can influence adult disease. In this study, the researchers found that maternal chronic kidney disease (CKD) can lead to offspring hypertension, but perinatal supplementation of propionate can prevent this and reverse established hypertension. This protective effect is associated with increased propionate-generating bacteria, elevated plasma propionate levels, higher expression of the SCFA receptor GPR41, improved gut microbiota diversity, and shifts in composition.

Summary: This study systematically assessed the impact of cardiomyopathy on maternal pregnancy outcomes. The results showed that pregnant women with cardiomyopathy have higher risks for adverse maternal outcomes, including cesarean delivery, severe maternal adverse cardiovascular events, and in-hospital mortality, compared to women with no heart disease and women with noncardiomyopathy heart disease. Preconception risk assessments and management by expert teams are crucial for pregnancies affected by cardiomyopathy.

Summary: Obesity in women is a global issue, and bariatric surgery is widely used for its management. Guidelines recommend avoiding pregnancy for a certain period after surgery due to risks. This study investigated the relation between surgery-to-conception time and pregnancy outcomes, considering gestational weight gain. The results showed that shorter surgery-to-conception interval was associated with inadequate gestational weight gain and lower birth weight. Conception should be delayed after bariatric surgery to improve pregnancy outcomes.

Summary: This population-based study reveals a reassuringly low maternal mortality rate in a highly developed healthcare system. Summary: This study found significant regional variation in the characteristics and outcomes of pulmonary arterial hypertension (PAH) patients in the United States. The study analyzed participants in the Pulmonary Hypertension Association Registry (PHAR) and identified differences in age, race, marital status, employment, insurance, smoking, alcohol use, etiology, and treatment characteristics among different census regions. The study also found lower mortality in PHAR participants in the West region. This study highlights the need for further research and improvement in providing equitable care for PAH patients in the United States.

Summary: The relationship between obesity and outcomes in pulmonary arterial hypertension (PAH) is complex. Previous studies suggested that obesity, in conjunction with PAH, may be associated with improved survival. However, our research indicates that obesity prior to the development of PAH is actually associated with worse survival, introducing a novel temporal element to the obesity-paradox.
