Maternal, Perinatal, and Postneonatal Outcomes in Women With Chronic Heart Disease in Washington State

OBJECTIVE: To explore the association between the presence of maternal heart disease and maternal, perinatal, and infant outcomes. METHODS: We conducted a population-based retrospective cohort study using Washington State birth certificates linked with hospital discharge records of mothers noted to have maternal congenital heart disease, ischemic heart disease, heart failure, or pulmonary hypertension. Women who gave birth between 1987 and 2009 (n=2,171) were compared with a sample of mothers without these conditions (n=21,710). We described characteristics of pregnant women with heart disease over time. Logistic regression estimated the association between chronic maternal heart disease and small-for-gestational-age (SGA) neonates as well as perinatal, postneonatal, and maternal death. RESULTS: The proportion of births to women with reported heart disease increased 224% between the 1987 and 1994 and 2002 and 2009 calendar periods. Chronic maternal heart disease was associated with increased risk of SGA (62 additional SGA newborns per 1,000 births, 95% confidence interval [CI] 46-78; P<.001), perinatal death (14 additional deaths per 1,000 births, 95% CI 8-20; P<.001), postneonatal death (5 additional deaths per 1,000 births, 95% CI 2-9; P<.001), and maternal death (5 additional deaths per 1,000 births, 95% CI 2-9; P<.001). CONCLUSION: The presence of chronic maternal heart disease is associated with elevated risk for poor maternal, perinatal, and postneonatal outcomes. Summary: This observational study examined the association between thyroid-stimulating hormone (TSH) and mortality in individuals with pulmonary arterial hypertension (PAH). The results showed a strong association between TSH and mortality, regardless of adjustment. However, there was no consistent association between TSH and other markers of disease severity in the PAH cohort.

Metabolomic Signatures Associated With Pulmonary Arterial Hypertension Outcomes

Summary: In this study, metabolic profiling was used to identify the association between plasma metabolomic profiles and right ventricular dilation, mortality, and measures of disease severity in patients with pulmonary arterial hypertension (PAH). The polyamine, histidine, and sphingomyelin metabolic pathways were found to be associated with RV dilation, mortality, and disease severity.

Regional Variation in Pulmonary Arterial Hypertension in the United States: The Pulmonary Hypertension Association Registry

Summary: This study found significant regional variation in the characteristics and outcomes of pulmonary arterial hypertension (PAH) patients in the United States. The study analyzed participants in the Pulmonary Hypertension Association Registry (PHAR) and identified differences in age, race, marital status, employment, insurance, smoking, alcohol use, etiology, and treatment characteristics among different census regions. The study also found lower mortality in PHAR participants in the West region. This study highlights the need for further research and improvement in providing equitable care for PAH patients in the United States.

Summary: The relationship between obesity and outcomes in pulmonary arterial hypertension (PAH) is complex. Previous studies suggested that obesity, in conjunction with PAH, may be associated with improved survival. However, our research indicates that obesity prior to the development of PAH is actually associated with worse survival, introducing a novel temporal element to the obesity-paradox.
