Laparoscopic sleeve gastrectomy (SG) is an accepted bariatric procedure, with an advantaged by a low complication rate. A feared complication is stapler line leak. Buttressing materials have been suggested as a means of reducing staple line leak rates. We analyzed the leak rates from published series to help in demonstrating a potential cause. The study was institutional review board (IRB) approved retrospectively. A Medline search using the key words sleeve gastrectomy and bariatric surgery obtained 54 articles. Attention was restricted to 11 articles written in English that listed numbers of gastrectomy procedures and leaks. Poisson regression assessed the possibility that patients who received buttressing materials had a reduced rate of leaks. Thirty-five patients were evaluated from Greece (15) and the United States (20); two patients developed staple line leaks that appeared to be related to problems associated with buttressing materials. Eleven prior studies and the present series yielded 1,589 procedures, 15 (0.94%) of which were complicated by leaks. The leak rate for patients who were known to have received reinforcement of some sort was 1.45 (95% confidence interval 0.41-3.43) times that for other patients. To detect a difference between 1% and 0.5% as statistically significant in 80% of cases, with a two tailed test and alpha set at 0.05, would require 9,346 procedures. There is no reason to believe, at this point, that reduction in leak rates occur because reinforcement is used. Because the leak rate is small, the routine reinforcement of the staple line after sleeve gastrectomy is questionable at best, although a decrease in hemorrhage has been reported.

Summary: Adrenocortical carcinoma is a rare disease with limited treatment options. Registry-based clinical trials are seen as a cost-effective approach with high scientific potential. ENSAT is considering transforming their current registry into a clinical trial infrastructure to conduct randomized registry-based trials for ACC patients.

Medullary thyroid carcinoma in children: current state of the art and future perspectives

Andreas Kiriakopoulos, Anastasia Dimopoulou, Constantinos Nastos, Dimitra Dimopoulou, Konstantina Dimopoulou, Evangelos Menenakos, Nikolaos Zavras

Summary: MTC is a distinct type of malignant thyroid tumor, accounting for 1.6% of all thyroid cancers, with both sporadic and hereditary forms. The hereditary form is mainly caused by the RET gene, and most pediatric cases are discovered through genetic testing. Early diagnosis requires consideration of genetic status, calcitonin levels, and ultrasound findings, with prophylactic surgical intervention being an important treatment approach.

Summary: The pyramidal lobe is present in 50.6% of patients with variable size, and its larger size correlates strongly with big thyroids, male gender, and Delphian nodes presence. Thorough anatomic knowledge and meticulous surgical technique are required for complete excision of the pyramidal lobe during thyroid surgery.

Summary: This article reviews the current concepts in diagnosing and managing pheochromocytoma and paraganglioma (PPGL), emphasizing the importance of personalized genetic testing and the use of next-generation sequencing to expand knowledge of PPGL. The article also introduces new methods and markers that can aid in PPGL diagnosis. It highlights the diverse presentation of PPGL, with symptoms mainly derived from catecholamine excess or mass effect on the cardiovascular system. Accurate diagnosis and timely treatment are essential, especially for high-risk individuals.

Calcitonin: current concepts and differential diagnosis

Andreas Kiriakopoulos, Periklis Giannakis, Evangelos Menenakos

Summary: Calcitonin is a hormone produced by the parafollicular cells of the thyroid gland that acts through the calcitonin receptor. In clinical practice, it is used for the diagnosis and follow-up of medullary thyroid carcinoma. Elevated basal calcitonin levels can indicate the presence of this type of carcinoma. This review provides current information on the synthesis and physiology of calcitonin, as well as its differential diagnosis in various clinical situations.
