Summary: This study retrospectively analyzed the association between complications of sleeve gastrectomy and patient-specific and perioperative factors. The results showed no significant difference in postoperative bleeding or staple line leak with different energy devices, staple height, or stapling techniques. Patient selection, especially considering age and coagulopathic comorbidities, was found to be crucial in reducing postoperative complications.

Summary: Sleeve gastrectomy is a popular bariatric surgery procedure, and this study aims to evaluate the occurrence of postoperative staple line leakage (PSLL) and identify risk factors. The study found that 6.46% of patients developed PSLL, and there were no significant differences in age, weight, and BMI. Abnormal drain amylase levels were associated with earlier detection of PSLL. Nonoperative management with percutaneous drainage and endoscopic stenting was recommended as the safest and most efficient approach.

Summary: Staplers are commonly used in clinics, but complications like bleeding and leakage are still challenging for surgeons. To address this issue, using buttress materials is recommended. This Review provides a systematic summary of the characteristics and applications of buttress materials. The analysis of numerous research data suggests that buttress materials play a crucial role in increasing staple line strength and reducing the probability of complications. Non-crosslinked small intestinal submucosa (SIS) matrix material is the preferred candidate due to its bioactivity, degradability, and biosafety requirements. However, further studies are needed to confirm its effectiveness.

Understanding and improving the management of staple line leakage, a common complication, is crucial. Increasing education and collaboration between healthcare professionals is recommended.

Summary: Our study showed that a median staple loading pressure of 15 g/mm(2) was preferred by surgeons in laparoscopic sleeve gastrectomy. Increasing the minimum and maximum pressure values within each cartridge zone by 10 g/mm(2) resulted in a reduction in bleeding rate by 59.8% and 38.7% respectively. Stapling protocol 2 demonstrated lower intraoperative bleeding rates, higher usage of preventive hemostatic techniques, and more hemostatic treatments compared to stapling protocol 1.

Summary: This study compared the early complications of stapling line using oxidized regenerated cellulose (ORC) and fibrin sealant (FS) in laparoscopic sleeve gastrectomy (LSG). The results showed that both ORC and FS had a similar impact on stapling line bleeding. There were no significant differences in the need for blood transfusion, hemoglobin level, white blood cell count, and drain output volume between the two groups on postoperative day 1 and 2. The ORC group had a significantly higher white blood cell count, but this had no clinical relevance.
