Summary: Segmental mandibular advancement (SMA) is a combination of various surgical procedures that aims to improve tongue base obstruction in OSA patients and minimize any negative aesthetic changes. This pilot study showed that SMA is safe and effective for moderate-to-severe OSA patients, with the majority of patients achieving significant improvement in AHI and airway volume, and no major complications occurring.

Summary: Both continuous positive airway pressure (CPAP) and mandibular advancement splint (MAS) therapy did not significantly improve blood pressure variability (BPV) in patients with moderate to severe obstructive sleep apnoea (OSA). However, there was a slight improvement in BPV for CPAP-treated patients with effective treatment. Further research on the long-term effects of OSA therapy on BPV is warranted.

Summary: This study evaluates the effect of mandibular advancement splint (MAS) therapy on cardiac autonomic function in patients with obstructive sleep apnoea (OSA) using heart rate variability (HRV) analysis. The findings suggest that successful MAS treatment correlates with changes in HRV, specifically the lengthening of NN intervals, a marker for improved cardiac autonomic adaptability.

Summary: This study aims to evaluate the relative efficacy of mandibular advancement device (MAD) and continuous positive airway pressure (CPAP) in treating blood pressure in patients with moderate-to-severe obstructive sleep apnoea (OSA). A randomized controlled trial will recruit 220 Asian patients with hypertension and high cardiovascular risk to compare the outcomes of MAD and CPAP treatment.

Summary: This study aimed to investigate how mandibular advancement enlarges the upper airway via posterior tongue advancement in people with obstructive sleep apnea (OSA), and whether this is associated with mandibular advancement splint (MAS) treatment outcome. The results showed that responders had the greatest tongue and upper airway movements, which indicated a satisfactory treatment response to MAS.

Summary: This study assessed the sleep quality and therapeutic effects of a custom-made mandibular advancement device (MAD) in male Japanese elite rugby athletes. The results showed that a majority of the participants had poor sleep quality and were diagnosed with obstructive sleep apnea (OSA). MAD therapy significantly improved sleep quality and reaction times.

Summary: Obstructive sleep apnoea (OSA) is a common disease characterized by upper airway collapse during sleep, leading to chronic intermittent hypoxaemia and systemic inflammation. OSA is associated with increased risk for cardiovascular morbidity and mortality, affecting coagulation and fibrinolysis processes. Further research is needed to understand the mechanisms and clinical implications of OSA-related changes in these processes.

Summary: This systematic review summarized the objective and subjective outcomes of different available Mandibular Advancement Device (MAD) designs in treating Obstructive Sleep Apnea (OSA). It found that Monoblock appliances performed better than bilateral thrust, midline traction appliances performed better than other designs, and custom appliances performed better than thermoplastic appliances. However, there were no clinically relevant differences between MAD designs in reduction of ESS, compliance, preference, side effects, and cost effectiveness.

Summary: In this Review, the authors summarize the shared risk factors and causal links between obstructive sleep apnoea (OSA) and cardiovascular disease (CVD), as well as the emerging knowledge on the heterogeneity of OSA. They also explore the potential role of new biomarkers for cardiovascular risk stratification in patients with OSA.

Summary: Patients with OSA had higher AIP and triglyceride, and lower HDL-C levels. AIP was significantly correlated with the Epworth Sleepiness Scale score, the apnoea-hypopnoea index, and oxygen desaturation index, but not with markers of sleep quality.

Summary: The aim of this study was to compare the effects of maxillomandibular advancement (MMA) on respiratory function and facial aesthetics between OSA patients with and without maxillomandibular deficiency. The study found that there was no significant difference in the improvements in respiratory parameters and facial profile measurements between the two groups after MMA.

Summary: This review provides a summary of the interactions between pregnancy and breathing during sleep, and highlights the changes in respiratory function that can increase the incidence and severity of sleep-disordered breathing. The increased risk of snoring and obstructive sleep apnea hypopnea syndrome during pregnancy is discussed, and the potential impact of increasing obesity rates on sleep-disordered breathing in pregnant women is considered.

Summary: This study evaluated the efficacy of low-level light therapy (LLLT) in improving pain, oedema, and neurosensory disorders after orthognathic surgery. Based on 15 randomized clinical trials, the study found a positive effect of LLLT in controlling pain and neurosensory disorders, while limited evidence showed a positive effect on reducing oedema. However, due to the heterogeneity across studies, a meta-analysis was not possible.

Summary: This study aimed to evaluate the efficiency of maxillomandibular advancement using aggregated individual patient data from multiple studies. Through the analysis of 12 systematic reviews, it was found that bimaxillary advancement surgery can improve respiratory indicators, sleepiness data, and increase upper airway size, but it is necessary to standardize the surgical criteria for measurable efficiency of the procedure.

Summary: TMJ chondrosarcomas are more prevalent in women with a male:female ratio of 1:1.4. Compared to other chondrosarcomas, TMJ chondrosarcomas have low recurrence and higher survival rates. The synovial subtype, absence of reconstructive surgery, and presence of local recurrence or distant metastasis are associated with poorer prognosis for patients.
